Kaposi's varicelliform eruption in a patient with healing peribucal dermabrasion.

Kaposi varicelliform eruption (KVE) is the name given to a distinct cutaneous eruption caused by Herpesvirus hominis types 1 and 2, vaccinia virus, or coxsackie A16 virus, superimposed on a preexisting dermatosis. A delay in diagnosing this condition may result in intense pain and rapid spread of the cutaneous lesions. We report a patient who underwent perioral dermabrasion for wrinkles who developed KVE secondary to herpes simplex virus infection.

The inheritance of female colour morphs in the damselfly ceriagrion tenellum (Odonata, coenagrionidae)

Female-limited polychromatism is found in many species of Odonata. In Ceriagrion tenellum (Coenagrionidae) one of the morphs is red-coloured, like the conspecific male (androchrome, erythrogastrum morph), whereas most females are red and black (typica morph) or black (melanogastrum morph). Virgin females of this species were mated in the laboratory and their progeny reared (13 crosses). Results of these crosses indicate that colour morphs are controlled by one autosomal locus with female-limited expression. A second laboratory generation (two crosses) confirmed this inheritance system. This locus has three alleles (one per phenotype) and a hierarchy of dominance: typica > melanogastrum > erythrogastrum. The dominance relationships of andro/gynochrome alleles in polymorphic damselflies so far studied are discussed. The frequencies of female morphs in natural populations are highly variable, but in all cases typica females are the commonest.

Treatment of severe mitral stenosis with percutaneous balloon valvotomy in pregnant patients.

BACKGROUND: Pregnancy can cause life-threatening complications in women with mitral stenosis. Frequently, there is an urgent need to increase the mitral valve area mechanically. In selected cases, percutaneous mitral balloon valvotomy (PMBV) has emerged as a safe and effective alternative to surgical commissurotomy. HYPOTHESIS: The study evaluates the effects of PMBV by the Inoue technique in nine pregnant patients with severe symptomatic mitral stenosis. METHODS: The patients were in New York Heart Association (NYHA) functional class II to IV and had echocardiographic scores of < or = 8. The mean gestational age was 24.8 +/- 6.1 weeks. The patient's pelvic and abdominal regions were covered with a lead apron to protect the fetus from radiation. A stepwise dilatation technique was used. Fluoroscopy time was kept to 10 to 15 min. RESULTS: One patient developed severe mitral regurgitation requiring emergency valve replacement. The remaining eight patients showed marked immediate symptomatic and hemodynamic improvement. After dilatation, the transmitral pressure gradient decreased from 20.8 +/- 6.5 to 7.3 +/- 1.4 mmHg (p = 0.001) and the calculated mitral valve area increased from 0.9 +/- 0.1 to 1.8 +/- 0.4 (p < 0.001). All patients had uneventful term deliveries of normal babies. On follow-up they were in NYHA functional class I. CONCLUSIONS: Percutaneous mitral balloon valvotomy is a safe and effective procedure for selected pregnant patients with severe mitral stenosis. The procedure is well tolerated by the fetus. Severe mitral regurgitation requiring immediate surgery may occur occasionally. The possible harmful effects to the fetus from its exposure to radiation during PMBV are unknown.

Mating opportunities and mating costs are reduced in androchrome female damselflies, Ischnura elegans (Odonata)

Female colour polymorphism is a perplexing characteristic of many damselfly species. In Ischnura elegans three female phenotypes occur, one of which has the same blue coloration as the male (androchromes) whilst the others are inconspicuous brown gynochromes (infuscans and infuscans-obsoleta morphs). By marking a natural population near Rome, Italy, we found that all female phenotypes have similar survivorship, but they differ in mating frequency. Androchromes represented 55% of females but were involved in 43% of matings, whereas infuscans females represented 27% of females and 40% of matings and the infuscans-obsoleta phenotype 18% of females and 17% of matings. Old androchromes stored significantly less sperm in their spermatheca than old gynochromes, suggesting that they had mated less often. The majority of mature androchromes were observed alone (54%) when the majority of gynochromes (82-84%) were mating. When live tethered conspecifics were presented to males, blue models (male and androchrome female) were less attractive than brown models (gynochrome females). In contrast, all female colour morphs and males were equally (highly) attractive to males when the models were dead. Androchromes were significantly larger than gynochromes. Our results indicate that androchrome females mate less often than gynochromes, which could be a means of avoiding unnecessary and costly matings, but some androchrome females failed to reproduce (mate or oviposit) probably because they were unable to mate at all. The different explanations for the maintenance of this polymorphism in I. elegans are discussed. Copyright 1998 The Association for the Study of Animal Behaviour.

Acne keloidalis nuchae. Tissue expansion treatment.

BACKGROUND: Acne keloidalis nuchae (AKN) is a chronic idiopathic reaction responsible for a great deal of discomfort in patients, and requires radical treatment to be eradicated. OBJECTIVE: To analyze the technique used for and the results of a case of AKN treated with tissue expansion, which was tracked for 2 years after the initial study. METHODS: A patient with AKN was treated with chronic tissue expansion, total resection, and reconstruction of the injured area by advancing the flap of the expanded scalp. Local radiotherapy was applied to the resultant scar. RESULTS: After 2 years' follow-up we did not observe a recurrence of AKN, and a good cosmetic result was achieved. CONCLUSION: Tissue expansion has a role in surgical management of AKN. Staged excision, skin grafting, and nonsurgical treatments are also options, however they must be individualized to the case at hand.

DNA synthesis and cell generation pattern of chronic lymphatic leukaemia lymphocytes stimulated by phytohaemagglutinin.

A detailed analysis of the cell recruitment and of the cell generation pattern of normal lymphocytes and chronic lymphatic leukaemia (CLL) lymphocytes, simulated by phytohaemagglutinin (PHA), was performed by the bromodeoxyuridiine (BUdR) Hoechst technique. It was found that in normal cultures the majority of cells divide two or three times, producing an early peak of DNA synthesis, while only a few cells grow exponentially and pass through many rounds of replication. On the contrary, the majority of CLL responsive cells grow exponentially, producing a delayed peak of DNA synthesis, while cells which divide only two or three times are scarce or absent. No difference in the minimal cell cycle length of the normal and the CLL exponentially growing population was found. In addition, a cell population recruited into cycle for the first time 5-6 days following PHA stimulation was observed in normal cultures but not in CLL cultures.

Epidermal mast cells.

Tissue from 134 patients with neurodermatitis and prurigo, pseudoepitheliomatous hyperplasia, pemphigus, and urticaria pigmentosa was examined qualitatively for epidermal mast cells. Epidermal mast cells were found in all of the diseases that were studied except dermatitis herpetiformis. Pemphigus vegetans and dermatitis vegetans were frequently associated with the presence of epidermal mast cells. In other pseudoepitheliomatous diseases, such as tuberculosis verrucosa cutis, blastomycosis, and bromoderma, epidermal mast cells were present in many cases. Four of eight patients with acral dermatitis with elevated IgE blood levels had intraepidermal mast cells; the number was much lower in patients with usual neurodermatitis and prurigo nodularis. Only two of ten cases of alopecia mucinosa showed epidermal mast cells. A single epidermal mast cell was found in ten cases of urticaria pigmentosa. Chronic inflammation associated with epidermal cell proliferation appeared to correlate with the presence of epidermal mast cell.

Treatment of skin and soft tissue infections with cefadroxil, a new oral cephalosporin.

Oral cefadroxil in doses of 0-6-1-8 g per day given on twice or three times daily schedules was effective in the treatment of thirty-six patients with infections such as abscesses, carbuncles, cellulitis, furunculosis and impetigo. Staphylococcus aureus strains and beta-haemolytic streptococci, alone or in combination, were cultured from lesions before treatment. In vitro studies with test discs showed that all the organisms were sensitive to cefadroxil, but twenty-three of twenty-nine S aureus strains and one of the seven streptococci strains were resistant to penicillin G. Pre- and post-treatment laboratory tests of renal, hepatic and haematopoietic functions produced no evidence of drug toxicity. The cefadroxil dosage effective in this study is lower than that recommended for currently available oral cephalosporins, which must be given on a four times daily schedule.
