A controlled trial of puncture sites for amniocentesis.

A controlled trial to determine a better site for amniocentesis was carried out over a 6-month period, involving 98 obstetric patients. The suprapubic and nuchal puncture site for amniocentesis were compared. The failure rate was found to be lower when the suprapublic site was used, and no significant difference in morbidity was found in the two groups.

A comparison of maternal serum levels of alpha-fetoprotein in normal and pre-eclamptic pregnancies.

Alpha-fetoprotein (AFP) concentrations were measured by radioimmunoassay in serum from women with normal and pre-eclamptic pregnancies. An analysis of the results obtained in normal pregnancy was made using arithmetic and semi-logarithmic scales, and a statistical conversion of the AFP values in relation to gestational age was introduced to allow an easier interpretation of results. In pre-eclampsia, significantly lower mean AFP values were obtained, with the majority of individual values being lower than the mean for normal pregnancy. These low levels were not associated with fetal death, but appeared to be related to the severity of disease. The significance of these findings remains to be evaluated.

Acute traumatic tetraplegia during pregnancy.

A case of acute traumatic tetraplegia during pregnancy resulting in maternal death is presented, and problems of management are discussed. Difficulties experienced by the mother were intractable urinary tract infection and respiratory insufficiency; those in the neonate were prematurity and the effects of drugs administered to the mother.

Health services needs of children in day care centers.

In order to assess health services needs of children in day care centers, interviews were carried out with health coordinators or directors of 52 licensed day care centers in Berkeley, California. Over one-third of the centers lacked a designated health coordinator; one-fourth of the centers lacked written health guidelines and/or emergency guidelines; almost one-half lacked nutrition education services; and over two-thirds lacked a dental health education program. Only one out of five centers was willing to admit mildly ill children. Health coordinators and directors showed strong preference for care of mildly ill children at home by a trained worker. The presence of a designated health coordinator and of state funds was highly associated with the content and scope of health services provided.

A study of relationships of serum glucose, insulin, free fatty acids, and free and total tryptophan to mental illness.

To explore possible disturbances of cerebral monoamines linked to glucose metabolism, 18 schizophrenics, 21 neurotics, and 21 controls were given a 5-hr glucose tolerance test, and the effect of the glucose load on total and free tryptophan, insulin, and free fatty acids in serum was studied in medication-free patients. The data indicated intragroup differences (fasting to subsequent hours) in serum glucose, insulin, free fatty acids, and free and total tryptophan values. Intergroup differences were noted in insulin values at the first hour, in free fatty acids at fasting with neurotics exhibiting the highest values, and in free tryptophan at fasting with schizophrenics having the lowest values. Schizophrenics with perceptual disturbances (e.g., hallucinations) exhibited lower free tryptophan values when compared with other schizophrenics. The results indicated a glucose-tryptophan interaction in schizophrenics.

Refresher course for general practitioner obstetricians: report of a survey on needs and expectations.

A survey of 52 general practitioner obstetricians attending a refresher course was carried out, the aims of which were to assess their motivation for attending, their expections of the course, and whether the course fulfilled these expectations. The main motivations for attending were interest and preparation for the Dip Obst RCOG examination. The main expectations were to be acquainted with current developments and to have tuition in practical procedures. The general practitioner obstetricians were generally satisfied with the course; however, they would have preferred a greater emphasis on informal tutorials rather than formal lectures, and practical procedures rather than recent advances in physiology and neonatology.

The reduction of the intrapartum still birth rate at the Queen Victoria Memorial Hospital.

A significant reduction in intrapartum anoxic fetal deaths has occurred at the Queen Victoria Memorial Hospital over the last ten years. The possible explanations for this include a variety of factors, which may either improve the health of the population presenting to the hospital, or improve obstetric care. Epidemiological evidence suggests that the use of fetal diagnostic techniques has contributed to the reduction of intrapartum anoxic still births.

Trial of intravenous therapy in women with low urinary estriol excretion.

Estriol excretion in pregnancy is favourably improved following administration of 25% dextrose to patients with persistently low estriol excretion. A double-blind controlled trial was undertaken in 60 patients to assess the efficacy of other regimens of infusion therapy with Hartmann's solution, aminofusin, 10% dextrose, or ritodrine in Hartmann's solution. Estriol excretion rose above the lower limit of normal in 69% of the patients treated. There was no significant difference in success rates between the four solutions studied when subjected to analyses of variance and covariance. Fetal and placental weights were directly related to estriol excretion. Influences of the various therapeutic regimens on metabolic acidosis have been considered and possible reasons for therapeutic success discussed.

The obstetric performance of the grand multipara.

The computerized records of all women delivered between July, 1974, and June, 1975, at the Mater Mothers' Hospital were analysed, and the obstetric complications associated with grand multiparity were examined. Hypertension, preeclampsia, unstable lie, malpresentation and retained placenta are found to be more common in women of high parity. The low morbidity associated with these conditions in the data may have been related to the close supervision and active management carried out. Anaemia remained more frequent in the grand multiparous women despite modern antenatal care. Hypertension in the grand multiparous women could not be explained by age in the data presented. The cause for hypertension in this group of women remains obscure. Newborn babies of grand multiparas developed neonatal jaundice more frequently. The association between jaundice and oxytocic usage in the data only partially accounted for this observation.

Penicillinase-producing strains of Neisseria gonorrhoeae isolated in Rotterdam.

This appears to be the first report of the isolation of a penicillinase-producing strain of Neisseria gonorrhoeae in the Netherlands. The presence of a penicillinase-producing gonococcal strain (PPGS) was demonstrated in two of a group of three patients with gonorrhoea. In addition, a penicillin- and ampicillin-sensitive gonococcus strain was isolated from all three patients. The PPGS was most probably imported into Holland from the Philippines. The epidemiological and clinical findings and the results of treatment of infection with this PPGS are discussed.

The treatment of chronic renal failure by transplantation.

The entire transplantation experience at the Peter Bent Brigham Hospital has been reviewed. This extends between 1951 and 1976, a 25 year period. Information on all patients has been undated through September 1976. Four different periods in the total experience have been compared, as determined by variations in immunosuppressive protocols. Although survival of renal allografts have plateaued over the past several years, the progressive decline in recipient mortality makes renal transplantation an increasingly attractive alternative to dialysis. Transplantation should be used in close conjunction with dialysis in the long-term treatment of patients with chronic renal failure.
