Non-invasive method of measuring bilirubin levels in newborn infants.

We present the results of an initial trial of a transcutaneous bilirubinometer under Australian conditions. The method is non-invasive and we obtained an acceptable degree of precision. The meter readings were influences by the site where reading was taken, and by phototherapy. Shading the recording site from phototherapy lights did not improve precision. We constructed regression lines for translation of meter recordings from forehead and abdomen to serum bilirubin levels.

[Pharmacokinetics of Taurolin].

In order to study the pharmacodynamics of taurolin (4,4'-methylene-bis(tetrahydro-2H-1,2,4-thiadiazine-1,1-dioxide) we labelled the substance in two different ways. 3*T contained 3 atoms of 14C, of which one served as a methylene bridge between the two ring systems. 4*T was labelled with 4 14C-atoms, two for every ring within the taurinamide part. Radioactivity was counted in blood, urine, faeces and expiration air of rats. For this purpose both labelled drugs were administered p.o., i.v. and i.p. After using 3*T radioactivity was excreted mainly by expiration, independently of the way of administration. The blood radioactivity diminished very slowly after a first short period. After administration of 4*T radioactivity was found mainly in the urine and only a small portion in the expiration air. Blood radioactivity diminished here very quickly. These results were confirmed by whole-body autoradiographic studies using 3*T as well as 4*T. No plasma-binding was found using the 4*T compound.

The use of antenatal tests of fetal welfare.

The value of antenatal tests of fetal welfare is a constant source of controversy and confusion to obstetricians and general practitioners. The proliferation of such tests indicates that none is ideal and all are relatively imprecise.

Infant passenger safety education in perinatal services in California.

Of 166 hospitals surveyed in California, 53 (31.9 percent) routinely provide some information on infant car-safety to parents of newborns and 115 (69.2 percent) were interested in developing a more comprehensive program of infant passenger safety. More than 80 percent of the hospitals estimated that less than a fourth of the infants were safely restrained on leaving the hospital. Larger hospitals provided more car safety education and expressed greater interest in developing comprehensive programs in the future. Although present services are limited, there is interest in expanding infant passenger safety education not only in perinatal services but in childbirth classes as well.

Analysis of glycosaminoglycan from diabetic and normal Chinese hamster cells.

Diabetic and normal cell lines from Chinese hamster kidneys were cultured in media containing 35SO4 and 3H-glucosamine. Glycosaminoglycans (GAG) were extracted and analyzed from the media, trypsin, and cell pellet by enzymatic and electrophoretic procedures. Significant increases in the hyaluronic acid content were noted in all three fractions of diabetic GAGs when compared with normals. In addition, an increased heparan sulfate content and decreased chondroitin sulfate amounts were noted in diabetic cell lines. These data suggest that in vivo changes in GAG types and amounts in diabetic kidneys seen by others may also be seen in cultured cells.

Care of mildly ill children enrolled in day-care centers: management by parents and by trained home workers.

Because child day-care regulations exclude ill children, parents are frequently faced with problems in finding an alternative care arrangement. This pilot study compares home care of mildly ill children by parents (46 preschool children) with care by trained home workers (23 preschool children) during 20 weeks in 1977. Episodes of illness in children cared for by trained home workers showed a slightly shorter duration (median 3.0 days, range 2 to 11 days versus median 5.2 days, range 2 to 14 days) and a less severe course (fewer and milder signs of illness) when compared with episodes of illness in children cared for by parents. Other selected activities involved in the management of the mildly ill children were comparable. Although larger studies are needed, it appears that if parents desire an alternative care arrangement for their children during mild illness, care by a trained home worker may be convenient and safe.

An incremental-dose combined oestrogen-progestogen oral contraceptive: effects on body weight, blood pressure, and biochemical parameters.

An evaluation is presented of the effects of an incremental-dose combined oestrogen-progestogen contraceptive (0.05 mg ethinyl oestradiol throughout the cycle plus d-norgestrel 0.05 mg for the first 11 days and 0.125 mg for the subsequent 10 days). The majority of women showed weight changes less than 1.0 kg; a weight gain in excess of 2 kg was observed in 11.4% and a similar loss in 14.3%. The net change in blood pressure after 12 months was a slight reduction; no excessive elevation of pressure was recorded. Although no significant change in glucose tolerance was noted, triglyceride levels became progressively elevated; the increase in cholesterol levels were observed mainly in the second half of the cycle. Changes occurred in some but not all haematologic parameters: a significant increase in plasminogen level and decrease in activated prothrombin time; factors 7, 9 and 10 were progressively increased, whereas factors 5, 8, fibrinogen and antithrombin activity remained unaltered. This new preparation provides effective contraception with satisfactory cycle control and body weight changes. It is particularly useful in women who require additional oestrogen.

Obstetric practice amongst Queensland general practitioners: a survey of needs and attitudes.

A survey of Queensland general practitioners was carried out to assess their needs and attitudes regarding obstetric practice. Less than half of the respondents now practise obstetrics, and many general practitioners felt a lack of support from specialist obstetricians, paediatricians, and anaesthetists. Many of the respondents expressed needs for further obstetric training, especially in labour ward management and neonatal care; however, few were able to attend extended courses.

Prolapse of conjunctiva following external levator resection.

Prolapse of the conjunctiva from the superior cul-de-sac is an infrequent complication following external levator resection. In uncooperative patients or children, repositioning of the prolapsed fold of tissue may be difficult and result in a fused conjunctival fold. This complication occurred in 4 of 102 children of the authors' series; its management is outlined.

A survey of women with unplanned exnuptial pregnancies in Brisbane.

The social circumstances of women with exnuptial unplanned pregnancies were surveyed, and the differences between those continuing with their pregnancy and those who decided to terminate their pregnancy were compared. Women who continued their pregnancies were younger, poorer, less educated, and more likely to be employed in unskilled jobs when compared with those who decided to have their pregnancy terminated.

Sterilization: a comparative review.

Publications relating to surgical procedures for sterilization have been reviewed, and the incidences of complications and subsequent pregnancies compared. Laparoscopic sterilization has the lowest incidence of complication, the morbidity rate being lower than that of laparotomy sterilization or hysterectomy, and the mortality rate lower than that of a single pregnancy or taking oral contraceptives for 1 year.

Perinatal death: audit and classification.

Two hundred and thirteen perinatal deaths occurred in a population of 10,539 deliveries over a 4-year period. The associated obstetric complications and circumstances were analysed. The majority of perinatal deaths occurred in fetuses in whom there was a serious malformation, or whose birth weight was less than 800 g. Of the 110 deaths which occurred in normal babies weighing 800 g or more, 54 were antepartum, 5 were intrapartum, and 51 were neonatal. The clinical features surrounding these deaths were classified, and their implications discussed.

A comparison of obstetric morbidity and management between clinic and private patients.

The patterns of morbidity and management among clinic and private patients in a teaching hospital were compared by an audit of computerized case records. The clinic patients were more likely to have a poor obstetric history and antenatal complications, and the private patients were managed more actively. Perinatal mortality in the clinic patients was higher and was related to the higher frequency of low birth weight babies.
