Classification and quantification of abnormal sperm along the epididymal tract. Comparison between adult and aged hamsters.

The types and averages of abnormal sperm were studied in the epididymis of adult and aged golden hamsters. Abnormal spermatozoa represent 14.6-19.6% of the total of spermatozoa in adults, 31.7-42.1% in middled-aged hamsters, and 39.3-50% in advanced-aged hamsters. Twelve abnormal shapes were found, with the lack of an acrosome, the lack of a head, and the coiling of the tail, being the most frequent in the three age groups. An important increase in the number of coiled spermatozoa was found in the corpus and proximal cauda of the epididymis, but a decrease was observed in the distal cauda. Our data suggest that the epididymis produces secondary defects in spermatozoa running from the proximal caput to the middle zone of the duct, but that many of these spermatozoa are eliminated in the distal cauda. Such a result is mainly found in aged animals, in which a higher percentage of abnormal sperm from secondary origin is found with respect to adults.

[Abdominal tuberculosis].

Abdominal tuberculosis develops according to a non-specific clinical picture, with a difficult differential diagnosis with respect to other entities of similar semiology. We present the case of a male who was admitted because of abdominal pain, progressive and notable loss of body weight and a fever of two months evolution. The culture from the colon biopsy showed the presence of Koch bacilli.

[Bilateral chylothorax after gastric surgery].

Chylothorax is a lymphatic effusion of chylous in the pleural space due to thoracic duct obstruction or injury. The most frequent aetiology is cancer; it is also related to chest trauma and iatrogenic. We describe the case of bilateral chylothorax in a 76-year-old woman, right predominant, during the post-operational phase of gastric surgery. This presented itself with respiratory insufficiency and tachycardia without initial haemodynamic compromise. It presented a favourable evolution after conservative treatment, cessation of oral intake and TPN and chest tube during 10 to 14 days.

Developing community-academic partnerships to enhance breast health among rural and Hispanic migrant and seasonal farmworker women.

PURPOSE/OBJECTIVES: To examine effective strategies for building community-academic partnerships for the promotion of breast cancer education and outreach among rural and Hispanic migrant and seasonal farmworker women, mostly from Mexican descent. DATA SOURCES: Published research and education articles and books, community-education models, personal experiences, and community key informant feedback. DATA SYNTHESIS: Effective community partnerships for enhanced education and outreach include a framework based on a network of partners with common goals, communication processes based on trust, and bilingual/bicultural and culturally competent staff. CONCLUSIONS: A sustainable community partnership can be achieved through systematic but flexible approaches to community planning. Involvement of community members in the development and implementation of education and screening activities helps ensure that community needs are met. Relationships based on mutual respect are key. IMPLICATIONS FOR NURSING PRACTICE: Nurses can act as catalysts through community capacity building to create community-academic partnerships to reach medically underserved populations with cancer screening, outreach, and education through the delivery of strategies that are based on common goals.

[Comparison of the effect of alprazolam, imipramine and placebo in the treatment of panic disorders in Cali, Colombia].

A comparison of safety, and efficiency of Alprazolam, Imipramine, and placebo in the treatment of panic disorder shows that both active drugs are significantly superior to placebo as regards therapeutic effectiveness. On a 77 patient sample, 62 completed an 8-week treatment, and 66 were considered as "assessable" for efficiency results after completing a 3-week treatment. Significantly, more placebo-treated patients than either Alprazolam, or Imipramine-treated patients dropped out trial, while the number of panic attacks was significantly reduced in both Alprazolam-, and Imipramine-treated groups. When trial was over, 96% of patients in Alprazolam group, and 95% of patients in Imipramine group were free from panic attacks, if compared to 65% in the placebo group. Generally speaking, drugs were well tolerated, and no serious adverse effects or life-threatening events were observed.

[Evaluation of the cold chain during the national antipoliomyelitis vaccination days. Mexico, 1987-1988].

An evaluation of the cold chain used during the "National Vaccination Days Against Poliomyelitis" in January and March of 1987 and 1988 was performed in 32 states of Mexico, both the potency of the trivalent Sabin vaccine and completion of requirements for the maintenance of the cold chain were evaluated at each level in the Ministry of Health's structure. Only 56 percent of the refrigeration units exclusively stored vaccines, more than 10 percent of refrigerators were broken, and 44 percent of the persons responsible for the cold chain system considered the storage capacity inadequate. A correlation was found between non-fulfillment of maintenance requirements for cold chain and a decreased in vaccine potency.

[Gastric emphysema associated with gastric volvulus].

The gastric volvulus is an infrequent entity requiring surgical treatment in both the acute and chronic cases. The case of an 81-year old female patient attended for an episode of gastric obstruction caused by a volvulus and whose simple stomach radiography showed an image of gastric emphysema is reported. The possible causes of gastric emphysema and its differential diagnosis with emphysematous gastritis are discussed.

[Prevalence of endemic goiter in Sicilian pregnant women in the Alte Madonie region].

The "Alte Madonie" is a hill-mountain community in Sicily with a population of approximately 30,000 inhabitants. It includes the towns of Alimena, Bompietro, Blufi, Castellana Sicula, Fasano, Gangi, Geraci, Petralia Sottana, Soprana and Polizzi generosa. This is an area where goitre is endemic. Between January 1990 and September 1991 out of a population of 400 pregnant women examined at various stages of pregnancy, goitre was found in 1 out of 7.8 women (51 cases). 74.5% (38 cases) of those with goitre had undergone endocrinological tests, in all except two cases, a long time before the start of pregnancy. Three pregnant women (5.8%) were receiving suppressive therapy with L-thyroxine. 58.8% (30 cases) had undergone hormonal controls and laboratory data had revealed cases of hypothyroidism; in the remaining cases hypothyroidism had been excluded on the basis of medical examination. One patient, not included in this series who had received IVG, had been treated a few months before with strumectomy and radio-iodio-therapy for non-differentiated carcinoma which had developed on the goitre. In conclusion, these findings underline the high frequency of goitre in pregnant women from the "Alte Madonie", and the need for greater care in the prevention and treatment of this pathology. There is a clear need for integrated health education programmes and early diagnosis to be carried out in schools and work places. The role played by the gynecologist who first sees the pregnant women with goitre is vitally important, especially in the case of those patients which dedicate little time to the health of their own bodies either for cultural reasons or due to the demands of work both inside and outside the family.

[Knowledge of the utilization of preventive practices through a telephone survey].

OBJECTIVES: To determine knowledge and use of preventive practices (cervical cytology, mammography and taking blood pressure) through a telephone survey. DESIGN: Crossover study. SETTING: Billabona Health Centre (Guipúzcoa). PARTICIPANTS: People over 15 in the Health Centre's catchment area. MAIN RESULTS: The sample size was 800 people. It reached 80% telephone coverage with a reply rate of 86.09% (278 men and 273 women). 76.5% of women stated they had heard of cervical cytology; and 80.2% mammography. 87.5% had had their blood pressure taken on some occasion. CONCLUSIONS: Both cervical cytology and mammography are well-known preventive procedures, although they could always be improved, whereas taking blood pressure is a widespread practice in all age-groups.
