The soft tissue/cartilage relationship in the laryngeal glottis.

A study of 93 larynges obtained at autopsy was made with regard to the proportion of cartilage and soft tissue in the glottis. The relevant measurements showed that the cartilage formed 48 per cent of the glottic opening and soft tissue (vibrating portion of the vocal cord) formed 52 per cent. The importance of this relationship to the classification and origin of laryngeal cancer is discussed, as is its relation to vocal cord nodules.

The detection of DNA tumour virus-specific RNA sequences in abnormal human cervical biopsies by in situ hybridization.

We have used the technique of in situ nucleic acid hybridization and autoradiography of thin frozen sections of human tissue to search for virus RNA sequences in human cervical tumours. Of cervical biopsies with abnormal cytology, 67% bound herpes simplex virus type 2 (HSV2) 3H-labelled DNA probes and 39% bound adenovirus type 2 (Ad2) 3H-labelled DNA probes, whereas control experiments with phage lambda 3H-labelled DNA probes, under the same conditions, bound to only 7% of cases. In contrast, normal cervical biopsies bound the three probes in only 23%, 17% and 8% of cases respectively.

Androgen metabolism in the epithelial and stromal components of the human hyperplastic prostate.

The reduced and oxidized metabolites of testosterone and dihydrotestosterone were measured in the stromal and epithelial components of 23 human hyperplastic prostates. Our studies indicate differences in the hormonal metabolic patterns of the stroma and epithelium of the resected specimens when compared with tissues obtained retropubically. Testosterone 5 alpha-reductase was evenly distributed between the two components of the specimens obtained retropubically whereas the 3 alpha (beta)-hydroxysteroid dehydrogenase was predominantly located in the stroma. The measurements on the resected specimens suggest, on the other hand, that the bulk of the 5 alpha reductase and 3 alpha (beta)-hydroxysteroid dehydrogenase activities were confined to the stroma although these activities were considerably lower than those measured in the corresponding components of the retropublically obtained specimens. The conversion of testosterone to androstenedione was negligible in all the samples analysed. We therefore conclude that the stroma is the main site for the transformation of dihydrotestosterone to the androstanediol epimers and that the asymmetric distribution of the 3 alpha (beta)-hydroxysteroid dehydrogenase may be instrumental in the development of hyperplasia in the prostate gland. Furthermore, the results of this study indicate that electroresection impairs the enzymatic activities of the tissue.

Gastric pseudolymphoma.

Six histologically proved cases of gastric pseudolymphoma are described. This uncommon benign condition often simulates ulcerated gastric carcinoma radiologically, and pathologically it may cause considerable confusion with malignant lymphoma. Recognition of this condition is important to prevent unnecessary and often hazardous treatment whether by surgery or radiotherapy and to prevent inclusion in series of malignant gastric lymphoma, so that falsely high survival rates are not recorded.

TNM (1978) in bladder cancer: use and abuse.

Modifications to the TNM classification for bladder cancer, introduced in 1979, are described. The use of this classification was evaluated in 106 new patients. It proved to be satisfactory for in situ and papillary tumours but major problems occurred in attempting to separate superficially invasive from deeply invasive tumours. Several aspects of this classification require further consideration and modifications are proposed.

Iron deficiency in children undergoing adenoid-tonsillectomy.

Following a preliminary study of peripheral blood values in 30 children undergoing tonsillectomy and adenoidectomy, a biochemical survey of the iron status of another 108 children also undergoing this operation was made utilizing an automated micro-method. It was shown that 8.4% of these children were frankly iron-deficient but there was no statistical relationship between the levels of serum iron and iron-binding capacity, the incidence of throat infections and the levels of the plasma immunoglobulins. Assessment of all children undergoing this operation for the presence of iron deficiency is advocated as a possible routine screening procedure.

The role of multiple mucosal biopsies in the management of patients with bladder cancer.

The results of multiple biopsies from apparently non-tumour bearing bladder mucosa were studied in 154 patients with bladder cancer; 33% of the biopsies from apparently normal mucosa showed histological abnormalities including 4.5% with carcinoma. When the mucosa appeared red but flat, 52% of the biopsies showed histological abnormalities including 14% with carcinoma. Biopsies from mucosa that had a granular or mossy appearance showed carcinoma in 42%. A more aggressive management policy was adopted in 12 patients (8%) as a direct result of the biopsy findings.

Branching microvilli in the nasal respiratory epithelium.

In an ultrastructural study of the respiratory epithelial covering of 14 human nasal mucous polyps, some of the microvilli overlying the ciliated cells were shown to exhibit a complex form of branching with a resultant increase in surface area. This apparently original observation is of uncertain significance but is not of definite pathological significance.
