The oral cavity is a rare location for the development of primary malignant melanoma. Two cases of malignant melanoma of the superior alveolar process are described. The necessity of an early recognition and of clarification concerning the differential diagnosis of oral pigmentation as well as the treatment are discussed, together with a review of the literature.

The outline of therapeutic priorities in case of maxillo-facial traumatism may be divided in three different stages according to the need of acute urgent, urgent and necessary intervention. Acute urgent therapy is called for when the bleeding of the arteria carotis externa or the maxillary artery cannot be quenched or when the superior airways are obstructed by the soft tissues of the tongue or the floor of the mouth. A description of the therapeutic attitude in such cases is given. Urgent therapy should be given within 8 hours after the accident and mostly consists in the immobilisation of bony injuries and definite care of soft tissue lacerations. Necessary therapy includes the final surgical treatment such as osteosynthesis by titanium mini-plates. More specific problems like frontobasal fractures and fractures of the orbital floor are described separately.
The frequency and importance of collum mandibulae fractures is indicated, especially in connection with the sequels of conservative-functional therapy, and the different classifications and types of fractures are described. The non-surgical approach is evaluated as a treatment. Its inconvenient and sometimes unsatisfactory results necessitate the surgical treatment of certain types of collum mandibulae fractures. Absolute en relative indications for osteosynthetic treatment of condylar fractures are noted, the proposed surgical technique is discussed. The importance of a universal preauricular incision continued in an incision for parotidectomy and resulting in a standard incision or a combined preauricular-submandibular incision, is pointed out. Surgical technique for intra and extra capsular collum fractures is illustrated. Our preference goes to functional stable osteosynthesis using titanium mini-plates and the often obligatory dissection of the facial nerve. The so called "condylar shave" or remodelling condylotomy in analogy with the DAUTREY-operation is a therapeutic contingency in case of bilateral collum mandibulae fractures with unilateral low condylar fracture where classical mini-plate osteosynthesis is done, and intracapsular multi-fractured condylar head on the other side where the condylar shave or rubble clearing is performed The attitude towards children is discussed. The result of this surgical technique is illustrated through the follow-up of 73 surgical treatments of collum fractures in 59 patients. We conclude that surgical osteosynthetic treatment of collum mandibulae fractures with a strict indication in case of dislocated and luxated fractures should be included in the therapeutic range of modern maxillo-facial traumatology.
The therapy of chronic facial pain poses an interdisciplinary problem. Since 1984 the therapeutical plan of Ulm University was developed in such a way that the intensity of pain and the location as well as former treatments are accommodated. The therapeutical steps don't interfere among one another. In the first four years 204 patients were treated with this scheme. A freedom or at least a reduction of pain was achieved in 187 patients.

Chronic facial pain remains a therapeutic problem. We report on 29 patients (37 nerves involved), who were treated with transcutaneous electric nerve stimulation (TENS). Pain reduction was achieved in 27 patients (93%) while in 5 patients it was possible to stop the treatment because they were constantly free from pain. Two patients did not respond to TENS therapy. It was possible to reduce or discontinue the use of analgesics in 79% of the cases. There were no severe side effects. Therefore, TENS should be seen as a simple, safe and effective method of treatment for many forms of chronic facial pain, as opposed to other invasive surgical procedures.

Combined Therapy of Transcutaneous Electrical Nerve Stimulation and Iontophoresis with Benzydamine and Diclofenac. The combination of transcutaneous electrical nerve stimulation (TENS) and the iontophoresis of benzydamine and diclofenac in a counter-current field is a new method in the treatment of chronical pain of the head and neck region. Prior to a clinical study this method was tested in a preliminary examination in 15 healthy volunteers. Clinical and physical data were investigated from which a theoretical maximum value of the quantity of iontophorised non-steroidal antiphlogistics (NSAI) could be calculated. The actual achievable absorption is discussed. Based on these data it is concluded that a good effective tissue concentration of benzydamine and diclofenac can be achieved with iontophoresis by means of TENS.

This study is based on a questionaire sent to members of the German laryngectomee association. The analyses of the answers provide important positive and negative information concerning the rehabilitation: Generally the patient was reintegrated into the family without incident. 59.2% learned oesophageal voice, but 28% acquired no effective substitute voice. 39% of patients and 51% of their relatives did not become acquainted with first-aid for neck-breathers (mouth to neck resuscitation). 19.5% resumed their former work. During the preoperative course, 36% of the patients waited 3 to 6 months from the beginning of their complaints to the first medical consultation. In 51% of the cases the interval from first complaints to the surgical treatment was more than three months. These results demonstrate shortcomings in rehabilitation which require attention.
BACKGROUND: The melanotic neuroectodermal tumor of infancy is a rare and so far as being classified neoplasm with a high rate of recurrence for one year after diagnosis. Since Krompecher described 1918 the tumor at first, only about 200 cases are reported until today, mostly with manifestation in the maxillary region. CASE-REPORTS: The authors present two infants at the age of six and eight weeks with first clinical manifestation of the tumor in the maxillary region. Although there were no other common signs, the tumor destroyed wide areas of the mid-face. In spite of a treatment with radical surgery, recurrences occur rapidly in the first living year. CONCLUSIONS: Our clinical and histological findings show characteristics of local malignant growth. For these facts the radical resections of the primary tumor and its recurrences are individually the therapeutical consequences. A follow up of seven years of one infant shows a hypoplasm of the mid-face as a result of the inhibition of further growth by the loss of germs after maxillary hemisection.
In the past few years the following methods have been accepted as the treatment of choice in trigeminal neuralgia: transcutaneous electrical nerve stimulation (TENS), drug treatment--especially carbamazepine, controlled thermocoagulation, percutaneous retrogasserian rhizotomy with glycerol, microvascular decompression. The attending physician will prefer the treatment from which he expects the best results. However, the choice of treatment should take into consideration the age and general condition of the patient, the possible side effects, the quality of pain and treatments already performed.

In the framework of differential diagnosis thermography as a non-invasive method has gained in importance. 34 patients suffering from lesions of the trigeminal nerve were examined by our method of thermoprofile analysis of the facial skin. In those lesions where perception disorders were dominant hypothermia occurred, while those lesions where pain was preponderate showed hyperthermic changes. The temperature of the facial skin, however, only shows a small difference compared to the skin of the trunk and the extremities. Therefore, our method of thermoprofile analysis should not be used as a single diagnostic test but as an objective screening procedure for nerve lesions in the facial area.
