Ceftazidime therapy in patients with cystic fibrosis and multiply-drug-resistant pseudomonas.
The in vitro activity of ceftazidime against Pseudomonas aeruginosa and P. cepacia isolates from patients with cystic fibrosis was compared with that of other antipseudomonal drugs. Ceftazidime was as potent as imipenem against P. aeruginosa and the only drug effective against P. cepacia. An evaluation of the elimination kinetics of ceftazidime in 20 cystic fibrosis patients revealed an elimination half-life of 1.76 hours, an apparent distribution volume of 0.27 liters/kg, and a serum clearance rate of 133.9 ml/minute/1.73m2. Urinary recovery of ceftazidime was 87 percent within the first 24 hours after administration of the drug, with 65 percent recovered in the first two-hour fraction. Probenecid administration had no effect on the elimination kinetics of ceftazidime. Forty-three patients who had either shown no response to conventional therapy or had sputum Pseudomonas isolates that were susceptible only to ceftazidime received 75 courses of therapy. In 67 percent of these patients, the clinical response, when evaluated using an objective clinical efficacy scoring system, was considered favorable. Clinical failures were not associated with the development of drug resistance. Thus, ceftazidime can be recommended for the treatment of acute pulmonary exacerbations in patients with cystic fibrosis.

Efficacy and safety of Org OD 14 in the treatment of climacteric complaints.
To study the effect of Org OD 14 [17 alpha, 17 alpha)-17-hydroxy-7-methyl-19-norpregn-5(10)-en-20-yn-3-one) on the endometrium, 9 healthy post-menopausal women were given a daily dose of 2.5 mg of Org OD 14 for 100 consecutive days. In addition, during days 91-100 a daily oral dose of 1 mg of lynestrenol was taken by each volunteer. Endometrial biopsies were obtained before the start of treatment and on day 91. At pre-treatment, the endometrium was atrophic in 7 out of 9 volunteers and weakly stimulated in 2 volunteers. On day 91, the endometrium was found atrophic in 8 out of 9 volunteers and weakly stimulated in 1. Breakthrough bleeding occurred in 1 volunteer. After cessation of the combined treatment with Org OD 14 and lynestrenol, withdrawal bleeding occurred only in 1 volunteer. To assess the efficacy of Org OD 14 in the treatment of climacteric complaints a double-blind cross-over study was performed in 29 post-menopausal women. Patients were randomly allocated to Org OD 14 or to placebo as first treatment. Each period of treatment lasted 4 mth; no wash-out period was introduced. Patients took 1 Org OD 14 tablet (2.5 mg) or 1 placebo tablet per day. The patients scored hot flushes and sweating. At the end of the second treatment period, each patient was asked which of the 2 treatments she preferred. Data sufficient to allow for a conclusion were obtained from 20 patients.(ABSTRACT TRUNCATED AT 250 WORDS)
Depressive symptoms and health service utilization among the community elderly.
The relationship between depression and complaints of physical disease in the elderly is often discussed but has received little documentation. The authors administered questionnaires to 88 elderly volunteers to assess two common psychologic diagnoses, affective disorders and organic brain syndrome, and three common cardiovascular symptoms, pain or numbness in the jaw or arm, pressure in the chest, and difficulty in breathing. Data on utilization of health care professionals and demographic information were also obtained. A significant correlation between reports of depression and complaints of cardiovascular symptoms was seen. While the volunteers with depressive symptoms also visited their physicians more often than did the other volunteers, they did not seek the help of mental health professionals. This raises the larger issue that elderly persons who have mild to moderate depression present to our health care system.
