Forty infants with infantile hypertrophic pyloric stenosis had a Ramstedt pyloromyotomy through a circumumbilical incision. Delivery of the pylorus was relatively easy. Mild wound infection occurred in three infants and a further child developed a purulent discharge. There was one instance of abdominal wall dehiscence and all the resultant scars were hardly visible, thus achieving an apparently unscarred abdomen.
Between January 1984 to June 1985, 18 Ph1 positive chronic myeloid leukemia (CML) patients in chronic phase (CP) underwent allogeneic bone marrow transplantation (BMT) from HLA identical and MLC negative siblings. The median age was 32.5 yr and median disease duration of CML at time of BMT was 19.3 months. The pretransplant conditioning regimen consisted of cyclophosphamide (CTX) (120 mg/kg) and 10.20 Gy total body irradiation (TBI) at 6 doses of 1.7 Gy each, administered in 3 daily fractions over 2 days at a dose rate of 15-20 cGy/min. To prevent graft-vs-host disease (GvHD) we used methotrexate (MTX) in one patient and cyclosporin-A (CYA) in the other 17 patients. In addition to CYA, given until day +365, 10 patients received donor marrow depleted of T cells with CAMPATH-1. The residual marrow lymphocytes were always less than 1%. The rate of engraftment was significantly correlated with the number of nucleated cells infused. Neither GvHD nor graft failure were observed among CAMPATH-1 patients. In this group one cytogenetic and one hematologic relapse occurred. The overall actuarial survival at 24 months is 78%. Of the 10 patients treated with donor marrow depleted of T cells, 9 are alive after a median follow-up of 9 months (range 5-18), with an actuarial survival of 90%. Of the other 8 patients transplanted with untreated marrow, 5 are alive after a median follow-up of 19.3 months (range 3.7-24) and the actuarial survival is 63.8%. This pilot study seems to demonstrate that T-cell depletion of donor bone marrow with CAMPATH-1 is effective to prevent GvHD, while the risk of graft failure can be avoided using a "standard" conditioning regimen including a fractionated TBI with a fast dose rate and a prolonged administration of CYA at the maximum tolerable dosage. While the high frequency of relapses suggests the employ of more aggressive anti-leukemic conditioning regimens in CAMPATH-1 treated marrow recipients.
Ofloxacin is a new fluoroquinolone with in vitro activity against the three main urethritis-causing pathogens: i.e. Neisseria gonorrhoeae, Chlamydia trachomatis and Ureaplasma urealyticum. 18 adult males with acute uncomplicated urethritis took 200 mg ofloxacin by mouth twice daily for 7 (non-chlamydial urethritis) or 14 (chlamydial urethritis) days. 12 patients had N. gonorrhoeae; 2 had H. parainfluenzae, 1 had E. coli and 5 had C. trachomatis. Urethral cultures were obtained before treatment and on day 7 (non-chlamydial urethritis) or 28 (chlamydial urethritis). Clinical and microbiological cure was achieved in 17 of the 18 patients. Clinical manifestations failed to resolve in one patient due to the presence of Trichomonas vaginalis recognized on day 7. No side effects were observed. According to these results, ofloxacin is effective and safe against gonococcal and chlamydial urethritis.

Literature data indicate a reduced calcium uptake in synaptosomes prepared from old rat brains. On this line, the present paper investigates the binding of ([3H]NDP) to brain synaptic membranes prepared from rats at different ages from birth up to 24 months of age. The binding is undetectable at birth but reaches within 9-18 day the values observed in adults [3H]NDP binding affinity and sensitivity to calcium were decreased in old rats (24 months). Tritiated dihydropyridines are believed to label voltage-dependent calcium channels (VDC). The observed age-related reduction in binding suggests that the characteristics of VDC in the aged brain may change.
Haemodynamic effects of atracurium at three doses (0.2, 0.6 and 1.2 mg . kg-1) were studied in thirty patients, anaesthetized with thiopentone (5 mg . kg-1) and fentanyl (0.2 microgram . kg-1 . min-1). No cardiovascular side-effects were observed with doses of 0.2 and 0.6 mg . kg-1. However, a 1.2 mg . kg-1 dose induced a transient but significant decrease in mean arterial pressure (-10%; p less than 0.001), maximal in the second minute, associated with an increase in heart rate (+10%; p less than 0.001) and cardiac index (+9%), and a decrease in systemic vascular resistance (-16%; p less than 0.001). The decrease in arterial pressure was constant in every patient and associated with a generalized flush in one of them. Histamine-release induced by atracurium may be one of the possible mechanisms involved in this hypotension.

The following report presents the results of a study conducted on the influence of sorbitol on levels of glycogen, phosphorylase a (E.C. 2.4.1.1.) and phosphorylase kinase (E.C.2.7.1.38) in slices of rat liver. In addition, it presents data concerning the effects of sorbitol on the activities of phosphorylase a and phosphorylase kinase purified from rabbit muscle. The data reveal a limitation in the glycogenolytic process which seems, in part, due to the inactivation of the phosphorylase a and phosphorylase kinase. Gel-filtration techniques showed, in fact, a breakdown of these two oligomeric enzymes into inactive subunits following incubation with sorbitol.
A survey was carried out in 15 leather goods manufactures in the Florence's Province in order to indicate the parameters for the evaluation of the exposure due to solvents by personal samplings. N-esano concentration was correlated to the following parameters: ventilation, volume of the premises, gluing surface of the articles, gluing process and relative methods. Aim of the survey was to point out the more useful parameters, to assess the glue users exposure, in order to effect improvements in the working process and premises of the workshops in the whole area. As a final result of the survey the parameter with the best correlation to n-hexane concentration was the one indicated as "gluing method". Some prevention advises are indicated.
A study of the photosensitizing effects of 8-methoxypsoralen (MOP), neutral red (NR), and proflavine (PF) on the skin of female Swiss albino mice, strain 955, was carried out using fractionated exposure to long ultraviolet light (300-400 nm) and visible light (tungsten emission). The results (1) confirmed MOP photocarcinogenicity, (2) demonstrated that both NR and PF are photocarcinogens, and, further, (3) showed that the above UV light with 2.6% of fluence at 313 nm is a long-term carcinogenic agent even though the total dose of 313 nm was 100 times less than the minimal UV tumorigenic dose in mice. The tumors were mammary adenocarcinomas, carcinomas of skin appendages, carcino-mixo-sarcomas, lymphomas, and one case of thyroid adenocarcinoma. The implications of the above data regarding the controversy about oncogenic risks in photochemotherapy are discussed.
As a model to investigate the pathogenesis of postmenopausal osteoporosis and the involvement of calcitonin in the bone alterations, we chose surgical menopause, studying the influence of castration on blood calcitonin. After surgery, the hormone and calcemia were raised significantly over preoperative values in castrated women, but not so in a control group of patients undergoing hysterectomy without oophorectomy. The elevated concentration of calcitonin was maintained during at least 4 months. It is speculated that this mechanism of control of calcitonin secretion by gonadal hormones might be activated in the pathogenesis of postmenopausal osteoporosis.
