From 1992 to 1999, 58 thyroid gland operations (41 female and 17 male) were performed in ENT Department of the District Hospital in Rzeszów. In 14 (21.4%) cases (9 female and 5 male) thyroid surgery was done for malignant disorders: papillary carcinoma in 11 (79%) patients, follicular carcinoma in 2 patients and medullary carcinoma in 1 patient. There were neck metastases in 9/14 (64.4%) patients. In 3 cases with papillary carcinoma (all with neck metastases) aerodigestive tract was invaded. One patient had neoplasmatic invasion of the larynx and trachea, one patient had invasion of larynx et pharynx and in one patient tumour invaded the esophageal wall. In those patients radical surgery was done: total thyroidectomy with total laryngectomy and radical neck dissection (2 patients) and subtotal thyroidectomy with conservative neck dissection (1 patient). External beam irradiation and radioactive iodine 131 treatment followed surgery. Two patients are still alive 6 years after the treatment free of disease, and 1 patient died of unrelated causes 3 months after the surgery. The authors have presented the video of different stroboscopic pictures of the larynx in patients after thyroid gland surgery.
The authors have presented results of otolaryngologic and phoniatric examination of 504 patients after thyroid surgery. 39 patients (7.74%) with recurrent laryngeal nerve paralysis were qualified for thorough phoniatric investigation.
Laryngological and phoniatric examinations were carried out in 130 alumni from theological seminary. The authors estimated their ability to serve the mass in the future. In 71 (54.6%) cases various pathologies of the ear, nose, oral cavity, pharynx and laryngx were found, which allowed for their early treatment and rehabilitation. It also helped to settle the limitations of their future ministration.
Basing on the own material of 328 patients treated surgically the authors have presented the problem of submandibular space pathology. They have pointed out its interspecialty in diagnosing and treatment. Among 328 patients the presence of inflammatory tumors have been stated in 220 (67.1%) cases, non-specific inflammation in 194 (88.2%) patients and specific inflammation in 26 (11.8%) patients. Neoplasmatic tumors have occurred in 98 (29.9%) patients, malignant tumors in 78 (79.6%) patients and benign ones in 20 (20.4%) patients. Among malignant tumors metastatic ones were observed in 41 (52.6%) cases, while primary tumors in 37 (47.4%) cases. In 3.0% patients developmental anomaly have occurred.
Authors have described a case of bilateral acinic cell carcinoma of parotid gland. The patient has been treated successfully by left superficial parotidectomy and right total parotidectomy followed by complementary Co 60 therapy. During 3 years after surgery the patient revealed no recurrence of the disease. Clinical course, histology and pathology have also been presented. The authors have pointed out the difficulties in diagnosing ACC because of the variety of histological pictures and unclear terminology.
An extremely rare case of primary adenocarcinoma of the tympanic cavity behind the intact tympanic membrane was presented. A radical surgical treatment and postoperative telegammatherapy was used, with no evidence of recurrence for four years. The authors have described the occurrence of this tumor in the temporal bone, clinical picture, diagnosis difficulties, prognosis and ways of therapy. They have paid attention to the guileful course of disease and non-characteristic symptoms in its initial stage.
The authors have presented the description of nasopharyngeal chordoma 28 years after the first diagnose and treatment with Co-60 application of this. Very slow, almost thirty years growth of the tumor has been noticed. The difficulties of the radical treatment, either radiotherapy or surgery have been pointed out in the case of clivus chordoma. The authors have presented too the long-term results of this very rare appearance tumor treatment, based on the review of the literature.
The authors have analyzed 562 cases treated by esophagoscopy in ENT Department of District Hospital Nr 1 in Rzeszów, Poland from 1987 to 1996. There have been 580 esophagoscopy performed during this period: In this number 504 therapeutic procedures and 76 diagnostic ones. The most common indication for therapeutic esophagoscopy has been the suspection or presence of foreign body in the esophagus and rarely other pathology of upper part of digestive tract. Complications of esophagoscopy are rather rare and in the most cases they are the result of perforation of esophageal wall during esophagoscopy. In presented material the complication rate was 0.17%. Impacted parts of false teeth in esophagus should be always removed by experienced otolaryngologist under endotracheal anesthesia and with esophageal muscles relaxation.
The authors presented 4 cases of patients with a foreign body in the esophagus, which has been tried to remove by means of fiberoscopy and rigid esophagoscopy. The fiberoptic endoscopy has failed either as a diagnostic method or as a therapeutic one. The classical esophagoscopy has turned out to be much better in both cases. The fiberoscopy is a valuable diagnostic and therapeutic procedure in a lot of diseases of digestive tract. Especially it is useful in order to remove some foreign bodies from the esophagus, but at the same time it fails when removing big foreign bodies (e.g. parts of false teeth, large bones). In these cases rigid esophagoscopy is the only effective method when performed under endotracheal anesthesia and with esophageal muscles relaxation.
BACKGROUND: Congestive heart failure is associated with blood volume expansion caused by stimulation of the renin-aldosterone system and arginine vasopressin. The use of left ventricular assist devices as bridges to heart transplantation has improved the survival of patients during this critical period. In studying heart failure physiology on support devices, we hypothesized that improvement of cardiac function by a left ventricular assist device is associated with normalization of volume load secondary to normalization of its regulatory substances. METHODS AND RESULTS: We studied 15 patients (13 men, 2 women: age 51 +/- 8 years) with end-stage heart failure who were cardiac transplant candidates eligible for HeartMate implantation. We measured plasma volume and plasma levels of atrial natriuretic peptide, aldosterone, renin, and arginine vasopressin sequentially before HeartMate implantation (baseline), after HeartMate implantation (weeks 4 and 8), and after transplantation. Baseline plasma volume was 123 +/- 20% of normal; it was 122 +/- 22% at week 4 and decreased to 115 +/- 14% at week 8. Atrial natriuretic peptide was 359 +/- 380 pg/mL at baseline, 245 +/- 175 pg/mL at week 4, and 151 +/- 66 pg/mL at week 8. Plasma aldosterone fell from 68 +/- 59 ng/dL at baseline to 17 +/- 16 ng/dL at week 4 (P < .05 versus baseline) and was 32 +/- 50 ng/dL at week 8. Plasma renin activity decreased from 80 +/- 88 ng/dL at baseline to 11 +/- 12 ng/dL at week 4 and was 16 +/- 38 ng/dL at week 8 (both P < .05 versus baseline). Arginine vasopressin fell from 5.0 +/- 4.8 fmol/mL at baseline to 1.1 +/- 0.7 fmol/mL at week 4 and 1.2+/-0.8 fmol/mL at week 8 (both P < .05 versus baseline). CONCLUSIONS: The reduction of plasma renin activity, plasma aldosterone, and arginine vasopressin occurred earlier than the reduction of plasma volume and atrial natriuretic peptide after HeartMate implantation, possibly because of decreased pulmonary congestion and improved renal perfusion. The reduction of atrial natriuretic peptide cannot be responsible for the lack of adequate decrease of plasma volume; its reduction can be taken as a marker of improved cardiac pump function and decreased atrial stretch.
The authors have described genesis, localization and diagnosis of the first branchial cleft anomaly and the case of the branchial cyst in the deep parotid lobe. Surgical treatment (superficial parotidectomy and cyst extirpation from the superior part of the deep parotid lobe) was effective; one-year follow-up revealed no recurrence of the anomaly.

The authors have presented the rare case of metastasis the cardiac adenocarcinoma to the middle ear. They have described occurrence of metastatic tumors of the temporal bone, mechanisms of their formation, clinical picture, diagnostic difficulties, prognosis and ways of therapy. A radical surgical treatment and chemotherapy have been used in this case. The patient died 17 months after diagnosis of the cardiac adenocarcinoma and 10 months after diagnosis of metastasis to the temporal bone.
In the paper authors have analyzed 70 cases of different neck, throat and larynx tumors connected with thyroid gland (44 female and 26 male, age 14 to 78), treated surgically in ENT Department of Country Hospital No 1 in Rzesów from 1974 to 1994. The material consists of 35 (50%) patients with neck tumors of thyroid origin, 20 (28,6%) patients with the tumor of thyroid gland or with nodular goitre, 8 (11,4%) patients with advanced cancer of the thyroid with shortness of breath and 7 (10%) patients with ectopy or splitted of thyroid. Based on the histopathological examination it was revealed that the highest group consists of patients with thyroid cancer because all of them live in the south-east part of Poland, which is known to be the area of higher risk of thyroid cancer. Neck tumor was the first symptom of thyroid disease in half of patients from the group mentioned above and the first symptom of 57,5% of patients with malignant neoplasms of thyroid gland were metastases to the neck lymph nodes. Among patients with thyroid malignant neoplasm the male:female ratio was 1:1,35, and the mean age-53,5 years was about 10 years higher than in the rest of patients. In 7 patients with thyroid cancer effective radical surgical treatment was applied. In 5 cases surgical treatment exceeded thyroid gland. It required very careful dissection and identification of recurrent laryngeal nerves and parathyroid glands to avoid iatrogenic damage.
From 1974 to 1994 in Otolaryngology Department of County Hospital No 1 in Rezszów, Poland in 35 patients tumor of neck derived from thyroid gland has been revealed. There were 22 females and 13 males, aged 16 to 78 (mean age 48), among the patients. Pathology examination of surgery removed tumors has revealed malignancy in 19/35 (54%) cases, non-neoplastic tumors in 11/35 (32%) cases and non-malignant tumors in 5/35 (14%) cases. Among malignant tumors metastasis of papillare adenocarcinoma was the most frequent pathology (37%). Authors analyze feature of the thyroid gland derived tumors based on their size, consistency, localization, mobility, towards surroundings and the picture during surgery. Because of the presence of thyroid gland malignant tumor metastasis without clinical symptoms, author emphasised the necessity of its consideration in differential diagnosis.
The authors have analyzed 148 cases with submandibular gland tumors treated surgically in ENT Department of County Hospital Nr 1 in Rzeszów from 1974 to 1994. Inflammatory tumors occurred in 123 (83%) patients, in this number non-specific inflammation caused by calculi occurred in 120 (97.5%) patients and specific inflammations (tuberculosis) in 3 (2.5%) ones. Neoplastic tumors occurred in 14 (56.0%) patients and non-malignant in 11 (44.0%) patients. Among malignant tumors adenoid cystic carcinoma occurs most frequently (8 cases, 57%), whereas among non-malignant ones pleomorphic adenoma is most frequent (10 cases, 91%).
