[Assisted ventilation at home for chronic respiratory insufficient patients in the Languedoc-Roussillon area (357 observations) (author's transl)].

Up to May 31, 1978, 357 patients with restrictive (77 cases), obstructive (138 cases) or mixed (142 cases) respiratory insufficiency were given assisted ventilation at home. Only those patients whose PaCO2 could not be stabilized by drug therapy, physical therapy and/or oxygenotherapy, were selected. Seventy-five tracheostomized patients were ventilated endotracheally using a volume generator with fixed frequency in an 8-hour session during sleep. Two hundred and eighty-two subjects were ventilated through a mouth-piece using either a volume generator with synchronizable frequency or a pressure relaxator (self-regulated ventilation) distributed in several day-time session for a total of 4 hours daily. Six months after the onset of home ventilation, a marked decrease of PaCO2 is observed in all the groups regardless of the ventilation method. This improvement is maintained after wards. Furthermore, a decrease in the average length of hospitalization is noted. After 2 years the survival percentage is 77.7%; after 5 years it is 52.3%. The patients who benefit most from assisted ventilation are those suffering from restrictive respiratory insufficiency submitted to endotracheal ventilation (more than 70% of survivals after 9 years). Long-term results are not so good for the obstructive or mixed respiratory insufficiencies, especially when an important bronchospasm and/or a large bronchorrhea is present.

[Non-allergenic treatment of varicose ulcers].

The fundamental element in the treatment of a varicose ulcer is the elimination of venous stasis. However, a local treatment is called for in the acute stage of the ulcer. This leads to the healing over of the ulcer or, at least, to its transformation into a granular and clean wound which can then be given radical treatment--either the surgery or the sclerotherapy of the varices. In most cases the treatment is ambulatory and consists of the application of a dry gauze compress covered with a foam rubber pad and an adhesive elastocompressive bandage. In certain cases the ulcerous crust is removed with chloramine. Less often, in the case of an old ulcer, with allergised and inflamed periulcerous tissues, the patient is hospitalized for a period of about 2 to 8 days before undergoing the ambulatory elastocompressive treatment. The simple treatments ensure the rapid healing of the ulcer, as long as it is really of venous origin.

[Value of isotopic methods in the diagnosis of myocardial infarction of the right ventricle].

In the left ventricle, isotope techniques can be used to study the perfusion of the myocardium or the ventricular function. Because of the thinness of the walls of the right heart, the classical examinations and Thallium 201 perfusion cannot be used to demonstrate a disorder of perfusion in the right ventricle. Nevertheless, it is possible, with the aid of Technetium 99m labeled pyrophosphate, to diagnose and localise necrosis of the inferior wall with an acceptable degree of sensitivity, during the acute phase of the infarction. However, the major value of isotope techniques is to evaluate right ventricular function by determining the value of the global ejection fraction and the kinetic anomalies of the different walls. These methods, together with echocardiography, allow the diagnosis of lesions of the inferior part of the right ventricle in patients with inferior necrosis... Furthermore, because these methods allow quantification and because they are reproducible and repeatable, they can be used to follow the course of the right ventricular function in response to treatment.

[Efficiency of mucociliary clearance and cough in bronchitis].

Progressive alterations in the in vitro transport properties of airway secretions according to the severity and duration of bronchitis have been previously reported, but the potential in vivo dysfunction of bronchial clearance during different phases of bronchitis has not yet been studied. We measured bronchial mucociliary clearance (CMC) and clearance by cough (CTx) using a radioactive aerosol technique (resin particles 7.4 micron in diameter, tagged with technetium-99 m) in patients with recurrent bronchitis (group I, n = 5) and in chronic bronchitic patients, either at an early (group II, n = 13) or advanced (group III, n = 17) stage of their disease. The results were compared with those obtained from a control group (group 0, n = 10) and the respective efficiencies of the mucociliary system and cough were analysed with reference to respiratory function. The initial deposition of the aerosol in central airways is similar in all the patients and healthy subjects. In comparison with group 0, the mucociliary clearance is only significantly reduced in groups II and III (p less than 0.02). In healthy subjects, the clearance by cough is very weak, whereas in bronchitic patients it can be roughly equal to or greater than the CMC. However, the cough is less efficient in group III than in group II (p less than 0.05). A reduced efficiency of the clearance by cough occurs together with abnormalities in the gas exchange.

[Lymphovenous cellulitis].

We will stick to the term "sclerous lymphovenous cellulitis" ("hypodermite lympho-veineuse scléreuse") to avoid any confusion with other types of cellulitis. It appears in predisposed areas; the most common zone for its development if the submalleolar region, on the internal face and behind the long saphenous vein. The asymptomatic nodule at the start spreads progressively, especially in thickness, is painful, and accompanied by oedema and cutaneous lesions. The surgical intervention uncovers a limited nodule but which remains attached at the deep posterior level of the perforants. The histology consists of a cystic fatty necrosis with secondary sclerosis; the vascular alterations are not too considerable. The aetiopathogeny is complex: the predominant factors are venous stasis in incompetent perforators and the state of the panniculus adiposus.

Effects of iron complexes on brain calcium homeostasis.

The effects of two physiological low molecular weight iron complexes, ferric lactate and ferric adenosine triphosphate (ATP) on brain Ca2+ homeostasis modification, have been studied in vitro and in vivo. In vitro ferric ATP complex shows a higher efficiency as modifier of Ca2+ homeostasis. This higher reactivity and the in vivo observed effect of increased brain uptake of iron from ferric lactate provoked by the presence of ATP, corroborate in vitro results showing an iron transfer from ferric lactate to ATP, as well as the mediator role of ATP in the iron-induced cellular Ca2+ homeostasis modification process. The possible role of this process in Parkinson's disease is discussed.
