Oral contraceptives and venous thromboembolism.

In 58 patients with phlebographically diagnosed deep vein thrombosis during oral contraception the extension and localization of the thrombotic process was analysed in relation to the coagulation and fibrinolytic system 6 months after thrombosis. Left-sided thrombi dominated and the thrombi were more extensive and proximal on the left side. This left-sided dominance was more apparent with higher oestrogen content in the pills. The right-sided thrombi more often were the source of pulmonary embolism. In 31% of the patients a defective fibrinolytic system was found; this defect was seen more often in patients with right-sided thrombi. Only very few defects were found in the coagulation system. No patients had an antithrombin III deficiency.

The influence of 'Tigason' (Ro 10-9359) on the serum lipoproteins in man.

Eight patients with psoriasis or Darier's disease were studied during 1-3 months of 'Tigason' treatment (0.7-0.8 mg/kg body weight). The levels of triglycerides and cholesterol in whole serum and in different lipoprotein fractions were repeatedly determined. Before treatment all lipid values were within the normal range. During treatment the values changed gradually and five of the eight patients developed pathological VLDL triglyceride concentrations. A significant increase of VLDL cholesterol and a decrease in the HDL cholesterol was also noted. Until more information is available, prolonged treatment periods with 'Tigason' should perhaps be avoided, at least in high risk patients.

Estrogen and progesterone cytosol receptor concentration in endometriotic tissue and intrauterine endometrium.

Cytosol estrogen (ER) and progesterone (PR) receptors were assayed in peritoneal and ovarian endometriotic tissue from 20 patients. In 12 of the cases the intrauterine endometrium was assayed as well. The control material consisted of normal endometrium from 30 women. The receptor content of the endometriotic tissue was less than that of the endometrium of the same patient. In most cases no receptors could be detected (12/20 cases ER-, 7/9 cases PR-) irrespective of the phase of the menstrual cycle. The endometrial receptor concentrations were that same in patients with endometriosis as in the control group. There were no differences in binding characteristics of the receptors in the two tissue types. It is concluded that endometriotic tissue contains lower a concentration of cytoplasmic ER and PR than the normal endometrium.

A study of estrogen and progesterone cytosol receptor concentration in benign and malignant ovarian tumors and a review of malignant ovarian tumors treated with medroxy-progesterone acetate.

ER and PR were assayed in 13 malignant and 20 benign ovarian tumors of different histologic types. ER was detectable in 67% and PR in 40% of the malignant tumors, compared with 35 and 45% in the benign tumors, respectively. The ER concentration was somewhat higher in the malignant lesions but there was no difference in the PR level. A retrospective review of 39 primary ovarian tumors of different pathologic stages and classes, treated postoperatively with medroxyprogesterone acetate (MPA, Depo-Provera, UpJohn) and in most cases Melphalan, showed a high Karnofsky performance index and good complete and partial response rates. The need for a prospective, controlled study of the role of MPA treatment of malignant ovarian tumors in evident.

IUD compared with oral contraception in nulliparae.

172 nulliparae attending a Swedish family planning clinic were studied. Doctors and midwives with various experiences gave the contraceptive method. Continuation rate at 12 months was 58 per cent for IUD and 86 per cent for OC. The expulsion rate in the IUD group was 11 per cent and in 30 per cent of IUD users the device was removed because of adverse effects. 10 per cent of the IUD group and 5 per cent of the OC group discontinued their method without introducing another one, thereby risking an unwanted pregnancy.

Acute deep vein thrombosis (DVT) after cesarean section.

In 169 consecutive women undergoing cesarean section, of which 90 were performed as an emergency and 79 electively, the frequency of deep vein thrombosis was evaluated with a non-invasive diagnostic technique - strain gauge plethysmography. Three patients developed thrombosis (1.8 per cent), all after acute surgery. No specific background factors were associated with the development of thrombosis. In 26 of the 79 patients (33 per cent), who were plethysmographed before operation, a biphasic venous emptying indicated venous outflow obstruction by the pregnant uterus.

Focal nodular hyperplasia of the liver, benign hepatomas, oral contraceptives and other drugs affecting the liver.

Due to the claim of an association between focal nodular hyperplasia of the liver, benign hepatomas and oral contraceptives, the files in the departments of pathology at the university hospitals in Lund and Malmö were examined for these two diagnoses made since 1945 and 1957, respectively. 26 cases of focal nodular hyperplasia of the liver and 7 benign hepatomas were found, 18 and 2, respectively, in women. Since 1963, the year before oral contraceptives were introduced in Sweden, focal nodular hyperplasia has been diagnosed in 8 women in the reproductive period of life; 4 of these had taken oral contraceptives. The 4 diagnoses were established in 1972-1974. At most, 25 per cent of Swedish females between the ages of 15 and 44 years have been on oral contraceptives. The Swedish series of 28 patients with focal nodular hyperplasia comprised 3 epileptics and 3 diabetics. At least two of the epileptics had been treated with barbiturates and/or hydantoins. The prevalence of drug-treated epilepsy in Sweden is 0.4-0.5 per cent, and of diabetes about 2 per cent. The possible aetiological role of drugs provoking an increase of the smooth-surfaced endoplasmic reticulum of the liver and proliferation of vascular fibrous tissue in a part of the liver which preveiously may have been damaged by vascular disturbances or trauma, is considered. No relationship between benign hepatomas and drugs was found. The observations support the notion that oral contraceptives may be of aetiological importance in the development of focal nodular hyperplasia of the liver, although the material is too small for epidemiological and statistical analysis.
