[Colo-anal anastomosis with reservoir in the treatment of rectal cancer].

Preservation of the anal sphincter system is now often considered in the treatment of adenocarcinomas of the mid-rectum (inferior limit between 11 and 6 cm of the anal margin). Direct anastomosis of the rectum to the anus or the last two centimeters of the rectum after resection of the rectal pouch is a source of discomfort. The construction of a colon pouch, placed directly above the anus, has been devised in order to improve these results. In the last three years, a J-shaped colon pouch, measuring 8 centimeters X 2, was constructed in 76 patients and anastomosed in its portion to the anal canal, after complete rectal resection. The functional results of this procedure, with uneventful post-operative course, were evaluated in 66 patients with a mean follow-up of 17 months. Continence was satisfactory in 96% of patients, with an average number of stools of 1.4 in 24 hours. Thirty one per cent of the patients require a small enema or a suppository to induce defecation.

Anterior fontanelle size in healthy Israeli newborn infants.

The anterior fontanelle size of 303 Israeli neonates was measured. The purpose was to establish normal values of fontanelle size in different Israeli ethnic groups. The size of the fontanelle was calculated as length plus width, the mean size for the whole population being 2.06 +/- 0.6 cm. These results correlate with those of Western, White, healthy neonates. No difference in anterior fontanelle size for the different ethnic groups or between sexes was found. Knowledge of the normal variation in anterior fontanelle size may be of help in diagnosing skeletal developmental disorders. The method used is simple and accurate and can be used in the routine neonatal examination.

[Transposition plasty].

Severe soft tissue and bone injuries are at the lower leg area a special problem for reconstruction. Only a limited number of local tissue transfer methods is available especially in the distal lower leg. Fasciocutaneous flaps, muscle transposition and myocutaneous flaps are available to solve some of the problems. The indication, validity and local topographical use is discussed, early and aggressive debridement and early covering of soft tissue defects with the appropriate flaps is the best way to avoid infection and more damage.

Monitoring microvascular anastomoses by spectral analyzing multifrequency bidirectional CW ultrasound Doppler flowmeter.

A new technique is presented for monitoring microvascular free tissue transfer. When performing free myocutaneous latissimus dorsi flap transfer to the lower limb, the flap artery and vein often lie on top of each other. Using conventional CW or pulsed ultrasound Doppler flowmeters, it is impossible to examine the patency of arterial and venous anastomoses simultaneously. However, a bidirectional CW ultrasound Doppler system and its spectral analyzing display can simultaneously examine both anastomoses, by separating arterial inflow from venous backflow multifrequencies.

Reconstruction of the thenar muscles by microsurgery.

During the last four years 16 toe transfers have been performed at our Plastic and Reconstructive Surgical Clinic in Hannover. In three cases of complete loss of the thenar muscles and the thumb, additional muscle reconstruction for opposition and adduction was considered. The method used in these cases will be described.

[Special technics for improving the results in replantation surgery].

By integrating of special techniques of hand surgery, microsurgery, and plastic surgery into the case of replantations and severe crush amputations, a result can be achieved which improves the function and thus shortens the time of treatment. According to the extent of the injury this can be achieved by immediate reconstruction of bony defects, by bridging long vascular defects and by late-primary soft tissue coverage.

[Fusion correction of polydactyly].

Polydactyly is the most common aesthetic hand anomaly. Its surgical correction is indicated for aesthetic, but especially for functional reasons. Independent of the degree and location (pre- or postaxial) of the anomaly the creation of one single unit, which corresponds to the functional as well as to the cosmetical demands is the main purpose of the operative correction. The principle of this operative procedure is the fusion of the two fingers by using all existent structures. Therefore and due to the individuality of each case of polydactyly only after clear exposure identification of soft tissue structures is possible. Resecting procedures of bony and soft tissue structures which are performed before the fusion shall therefore be performed after the exposure and by preservation of all structures, which are necessary for growth, function, sensibility and blood supply. Using these principles clinical experiences which were made with this method are demonstrated while also postoperative results are shown.

[Conservative therapy of upper gastrointestinal hemorrhage (prospective, randomized, controlled)].

In this prospective study for pharmacotherapy of non varicial upper gastrointestinal haemorrhage eight therapy groups were examined. The efficacy of the most important drugs for ulcus disease was considered. The drugs were used alone and in combination. The results indicated no favourable effect of one preparation (Cimetidine, Pirenzepine, Ranitidine). Significant benefit for the long time healing rate was noted among the combination therapies, especially with Sucralfate, which was not used alone.

[10 years of endoscopy at the University Department of Surgery in Graz].

The results of the endoscopies carried out from 1973 to 1983 at our hospital are reported (6947 gastroscopies, 2189 colonoscopies). Gastroscopic aftercare following gastric resection for benign gastric disorders and after partial gastrectomy for carcinoma is discussed. The results of the use of laser in emergency endoscopies are presented (731 patients with 827 bleedings lesions). By implementation of laser there was a remarkable reduction in the rate of immediate and early operation and in early mortality in cases of gastrointestinal bleeding. The results of 244 endoscopic polypectomies are reported.

Establishment of stable human T-T hybridomas.

Human T-T hybridomas potentially provide an invaluable resource for a variety of immunoregulatory molecules that modulate the immune response. To date, success in this technology, using human cell populations, has been hampered by several problems associated with proliferative and functional instability of the hybrid cells. These forms of instability are the result of a multifactorial process, with 1 parameter of importance being the chromosome number of the malignant parent cell line used for fusion. The present studies describe the production of a stable human T-T hybridoma generated by fusing a near diploid (modal chromosome number of 48) aminopterin-sensitive T cell line, CEM TG E11, and lectin-stimulated human peripheral blood lymphocytes. The rapidly growing hybrid cells have been clonally selected for the production of a B cell growth factor. Hybridization was documented by the presence of HLA phenotypes reflecting the combined antigens of the fusion partners. Fusions with 4 other partners besides CEM TG E11, where the majority of the cells had modal chromosome numbers ranging from 78 to 94, were proliferatively unstable. To date, hybrid cells derived from the CEM TG E11 fusion have been doubling approximately every 48 h for greater than 12 months, and selected clones constitutively produce B cell growth factor.

[Reconstruction of the esophagus and hypopharynx by free jejunum interposition].

Beside conventional procedures the hypopharyngeal and esophageal reconstruction using a free jejunum-transfer represents an additional method for primary reconstruction of the hypopharynx and esophagus. Out of 48 free jejunum-transfers 16 were used for bridging an hypopharyngeal or esophageal defect, 32 as a patch for reconstruction of the oral cavity. In 7 patients a hypopharynx carcinoma was resected, in 6 cases the cervical esophagus was infiltrated by carcinoma. 3 patients suffered from an esophageal stenosis caused by corrosion after acid ingestion. 11 free grafts healed without any complications. In 2 cases the jejunum was removed because of necrosis. In 3 cases fistulation from the lower anastomosis was observed. Free, microvascular jejunum-transfer using physiological graft material not only seems to be advantageous for tolerance to radiation but is also a one step surgical procedure.

Attitudes of high school girls in Israel and the United States toward breast feeding.

Newborns are breast fed more frequently in Israel than in the United States (92% vs. 57%). Differences in the attitudes of the two countries might suggest interventions to increase breast feeding. We studied the breast-feeding attitudes of high school girls in Israel and the United States using a 21-item questionnaire. Israeli girls were significantly more positive about breast feeding than U.S. girls. Israeli girls answered 13 items positively more frequently than U.S. girls, while American girls responded positively more frequently than Israeli girls for only two items. Israeli girls were more likely to plan on breast feeding (68.2% vs. 42.4%). We speculate that interventions designed to improve the breast-feeding attitudes of young people may increase the prevalence of breast feeding.
