Diagnostic value of computed tomography in pancreatic carcinoma: a comparison with other radiologic methods.

Computed tomography (CT) performed on 54 patients with pancreatic carcinoma showed evidence of pancreatic disease in 44, but a definite diagnosis of an inoperable pancreatic carcinoma could be made in only 16 by revealing liver metastases. Differentiation between neoplastic and inflammatory disease was impossible in most patients, particularly in patients with resectable tumors. CT was found to be inferior to angiography, PTC and ERCP in diagnosing pancreatic carcinoma. CT was also significantly less sensitive than angiography in assessing operability. However, in patients where CT demonstrated liver metastases, angiography and ERCP may be avoided.

Diagnostic value of computer tomography in chronic pancreatitis: a comparison with current diagnostic tests.

During a 3-year period 39 patients with chronic pancreatitis were subjected to roentgen examinations and exocrine pancreatic function tests (EPFT). Computer tomography (CT) was carried out in 36 of these, and revealed pancreatic disease in 29, while characteristic findings of chronic pancreatitis were demonstrated in 24 patients. CT was particularly valuable in diagnosing intra and extrapancreatic pseudocysts and pancreatic abscesses complicating chronic pancreatitis. At endoscopic retrograde pancreatography (ERP) pathologic alterations were demonstrated in 23 of 24 patients, and was diagnostic for chronic pancreatitis in 20 patients. The cannulation failed in 7 patients. The results indicate that ERP cannot be replaced by CT. ERP is still needed for detailed demonstration of the pancreatic duct system, especially preoperatively. Conventional films of the abdomen, EPFT and angiography were the least sensitive tests.

Elective surgery for ulcerative colitis: colectomy in 158 patients.

Chronic ulcerative colitis was treated by elective colectomy in 158 patients. Proctocolectomy and ileostomy was performed in 140 patients and colectomy and ileorectal anastomosis (CIRA) in 18 patients. The operative mortality was 2.5%, and postoperative complications, mostly infections, occurred in 38%. Within a 2-year postoperative period another 1.9% of the patients died, and late complications occurred in 18%. Colorectal carcinoma was present at the time of colectomy in 5.1% and developed some years later in another two patients primarily operated on with CIRA. Half of the cancer patients died of malignancy. Most extracolic complications, present in 25% of patients before colectomy, regressed or disappeared after operation. Half of the patients operated on with CIRA needed to have their rectum removed within a few years because of cancer or proctitis, and few of the rest had lasting relief of symptoms.

Results of hepatic lobectomy for primary epithelial cancer in 31 adults.

Hepatic lobectomy for primary epithelial cancer was performed in 31 adults from 1964 through 1977 in the surgical departments of six Scandinavian hospitals. Twenty-three patients were discharged and had a 2 year survival rate of 62 per cent and a 5 year survival rate of 16 per cent. Alternatives to surgery have not yet emerged. Further progress requires centralization.

Adenoma, Bile Duct↗

Circulatory homeostasis in rats after bile duct ligation.

The bile duct was ligated in rats, and their tolerance against a small blood loss was evaluated 7 days later. A 10% blood loss precipitated a large and sometimes fatal reduction in arterial blood pressure, while no reduction was seen in shamoperated rats. The plasma and erythrocytes were labelled by isotopes and the animals were rapidly frozen in liquid nitrogen. The splanchnic and pulmonary blood volumes were estimated from the isotope content in blood and tissue. These vascular beds will normally reduce their blood volumes during a blood loss and thus serve as vascular depots. In the bile duct occluded animals, the partition of blood is changed. More blood is to be found in the splanchnic vessels, and the depot function of the lung vessels is partly used for compensation. When these rats were bled, their liver blood volumes were not reduced, and only a small further reduction took place in the lung vessels. It is concluded that rats with bile duct occlusion will suffer considerably from small blood losses. This may be due to a lacking depot function of the splanchnic vessels.

Portography of normal and regenerating dog liver.

The angiographic appearance of the portal vein in the dog before and after major hepatic resection is described. An increased diameter and length of the lobar vein and their ramifications were found following resection. The appearance both in the portal and sinusoidal phase indicated a diffuse growth of the liver remnant. One week after resection a portal hypertension was recorded, which had usually receded after six weeks. Major hepatic resection seems to imply a higher risk for fatal toxic effects from the contrast medium.

Hepatic venography of normal and regenerating dog liver.

Total and selective hepatic vein angiographies were performed preoperatively and 6 weeks after 70 per cent liver resection in 6 dogs. The alterations in the hepatic vein system, increased diameter and elongation of the remaining lobar veins and their major tributaries plus increased angle between the radicles, are consistent with the findings earlier described by angiography of the hepatic arterial and portal venous system after major hepatic resection in dogs.

Recurrent and persistent Cushing's syndrome after assumed total bilateral adrenalectomy.

Two cases of recurrent Cushing's syndrome after assumed total bilateral adrenalectomy are presented. Both cases were due to adrenal tissue left at the adrenalectomy. In the first case the lack of preoperative localization of the adrenal remnant caused severe postoperative complications. In the other patient localization of the adrenal tissue was achieved by scanning after administration of 131I-19-iodocholesterol. The reoperation was easy and the postoperative course smooth.

Blood flow in normal and cholestatic dog liver as measured by intraparenchymal injection of Xenon 133.

Parenchymal liver blood flow as reflected by the wash-out of intraparenchymally injected 133Xe was studied at different stages of total cholestasis in the dog. 248 registrations at 13 laparotomies in 6 mongrel dogs showed that extrahepatic cholestasis had no statistically significant influence on the blood flow as compared to normal. A review of the literature shows little harmony between the results of this and various other studies.

Effects of different doses of acetylsalicylic acid on renal oxygen consumption.

The aim of the present study was to examine the acute effect of acetylsalicylic acid (ASA) on renal oxygen consumption (Qo2) and renal sodium excretion (UNaV) in anaesthetized dogs. With plasma salicylic acid (SA) concentrations ranging from 200-400 microgram/ml, Qo2 increased 36% (P less than 0.05) in spite of a 16% decrease in renal blood flow. At plasma SA concentrations of 80-200 microgram/ml Qo2 was significantly increased 45 min after the onset of ASA infusion. UNaV decreased from 97.7 to 21. 5 mumol/min (P less than 0.05). Glomerular filtration rate and absolute tubular reabsorption of sodium (RNa) was unchanged. The ratio RNa/Qo2 decreased from 31.0 to 21.3 (P less than 0.05). Renal lactate uptake increased. The results are most consistent with an uncoupling affect of ASA on oxidative phosphorylation in the kidney.
