[From the early history of roentgen documentation].

By chance, eight X-ray plates dating from 1899 were found in 1986. Each of these glass plates shows the patient's name, the date of the examination were performed in Vienna by Prof. Franz Exner, who was a friend of W.C. Roentgen. The glass plates are 25 x 13 cm in area and 3 mm thick. One side is coated with silver bromide. Probably these are some of the earliest X-rays records in the history of radiology. Their quality is comparable with the quality that can be achieved today, even though the materials and the imaging method used were somewhat unsophisticated.

[Significance of a transthoracic imaging technic in shoulder arthrography].

Even in this era of sonography, arthrography of the shoulder is an important diagnostic investigation, making it possible to exclude unrecognized connective tissue lesions in patients with chronic shoulder pain resistant to therapy. Sometimes standard X-ray techniques do not yield all the information the surgeon needs about ventral or dorsal localization of a rotator-cuff rupture. An additional approach to the joint is needed: a transthoracic lateral-view X-ray provides the surgeon with adequate information to help in selection of the most appropriate operative approach to the shoulder.

[The Black Forest arched clock in the x-ray picture].

The typical painted wooden Black Forest clock of the late eighteenth and early nineteenth century was the original famous Black Forest clock. The clocks changed from a useful article to an object in great demand by collectors; however, among them there are many fakes. Because of the use of colors containing lead and zinc at that time, the paint on these clocks can be evaluated radiologically and restaurations made later or even fakes can be detected easily. Important information can even be obtained about the wood used: its condition, the wooden axles, and abrasion of the gears.

[A new Y-endoprosthesis for drainage of bile duct obstruction of the hepatic bifurcation].

Biliary decompression in cases of central tumorous biliary obstruction requires surgical or internal/external catheter bypass techniques. The development of a 14-F Y-shaped-polyurethane endoprosthesis stent provides the possibility to drain the left and right biliary system simultaneously. The endoprosthesis is placed by a combination of external transhepatic and endoscopic approach. The tip of the singular choledochal stent segment is placed within the choledochus or duodenum.

[Bone biopsy of uncertain bone lesions using a new biopsy cannula measuring 1.4 mm].

Bone biopsy for the clarification of ambiguous lesions has until now been the last resort in diagnostic procedures because obtaining the material for histological evaluation has involved very traumatizing methods (biopsy by surgical intervention, percutaneous biopsy with high-caliber needles). A new bone-biopsy apparatus has now been developed that is provided with a cannula not much larger than a fine needle (1,4 mm); it can be used to obtain histologically evaluable biopsy material from almost all regions of the skeletal system. As the procedure can be carried out under local anesthesia, there is no undue stress to the patient. The complication risk is no higher than when using conventional fine-needle biopsy. Despite the small diameter of this needle it is possible to obtain biopsy specimens not only of osteolytic but also of osteoplastic lesions. An electric motor (20 rotations/min) is the power source for apparatus. The value of bone biopsy in diagnostic procedures has increased considerably.

Administration of a terpene mixture inhibits cholesterol nucleation in bile from patients with cholesterol gallstones.

Patients with cholesterol gallstones referred to elective cholecystectomy were randomly assigned prior to operation to no treatment (n = 14), treatment with one capsule t.d.s. (n = 12) or two capsules t.d.s. (n = 11) of a terpene mixture (Rowachol). Patients with pigment stones (n = 7) or no biliary tract disease (n = 5) were also studied. Lipid composition, presence of cholesterol monohydrate crystals, and nucleation time were determined in gallbladder bile aspirated during surgery. Cholesterol saturation was similar in the different groups. Crystals were present in all cholesterol gallstone patients without treatment and in none of the controls. In one of the patients treated with one capsule and four of the patients treated with two capsules crystals could not be detected. The terpenes prolonged nucleation time from 2.8 to 5.8 days (one capsule; P less than 0.05) and to 9.5 days (two capsules; P less than 0.001), respectively; but nucleation did not occur in seven controls. Although the mechanism by which the terpene mixture inhibits the formation of cholesterol crystals in bile was not determined, the findings suggest that the terpene mixture might be a useful agent for a clinical trial to test whether they will prevent recurrence of gallstones after medical dissolution.

Acute surgical intervention for complications of percutaneous transluminal angioplasty.

Since 1979, percutaneous transluminal angioplasty (PTA) resulting in an overall improvement of 80.3% has been performed in 4380 patients with occlusive atheroma of the lower extremities. Complications requiring immediate surgical treatment occurred in 123 (2.8%) of the cases, consisting of occlusion (16%), dissection (13%), perforation (10%), embolisation (15%) and haematoma at the puncture track (69%). Of the group treated surgically, 18 (14.6%) patients underwent amputation of a lower limb. Since 1980 348 percutaneous transluminal coronary angioplasties (PTCAs) have been performed for coronary artery disease. In 15 cases (4.3%) emergency coronary bypass surgery was necessary because of complications encountered with PTCA. Two patients died and perioperative myocardial infarction occurred in 7 (subendocardial-3: transmural-4). In spite of a low complication rate and good results from PTA and PCTA acute surgical intervention for complications carries a significant operative morbidity and mortality. We conclude that selection for PTA and PTCA has to involve vascular and cardiac surgeons, their presence being mandatory at institutions in which therapeutic radiological procedures are undertaken.

[Sonographic imaging and assessment of stomach wall changes].

By the use of a special method of investigation, the true value of percutaneous echography was investigated using 28 patients with primary gastric neoplasms and 3 patients with secondary infiltration of the gastric wall, for the proof and the judgement of the tumors, their extension and their metastases. Tumorous gastric wall thickening could be demonstrated in 87% (27/31), tumor invasion in neighbouring structures in 85% (12/14) and lymph nodes metastases in 66% (10/15). In all patients, who had liver metastases (4) or ascites (5) they were demonstrable. The sonographic appearance of the different gastric wall changes are described and discussed.

Ribosomal proteins and DNA-binding protein II from the extreme thermophile Bacillus caldolyticus.

Crystallographic studies, presently on ribosomal and DNA-binding proteins from the moderate thermophile Bacillus stearothermophilus, can be expected to benefit from the use of even more stable proteins from extreme thermophiles. Bacillus caldolyticus, which is able to grow in the temperature range of 70-80 degrees C, appears to be a suitable candidate. We have compared the two bacilli using two criteria: the two-dimensional gel patterns of ribosomal proteins and the properties of DNA-binding protein II. The latter protein is ubiquitous in the eubacterial kingdom and can be purified in large quantities. B. caldolyticus can be grown at 75 degrees C in continuous culture with a generation time of 45-60 min. The yield of ribosomes compares favorably with that of B. stearothermophilus. The gel patterns of the ribosomal proteins are very similar but several differences, in particular among the 50S proteins, are observed. The N-terminal amino-acid sequence of the DNA-binding protein differs in 3 positions (out of 39) from B. stearothermophilus and the protein shows an increased resistance to thermal denaturation. Tetragonal and monoclinic crystals of DNA-binding protein II have been obtained which are suitable for X-ray studies and the diffraction patterns of the two crystal forms are shown.

The N-terminal sequence of ribosomal protein L10 from the archaebacterium Halobacterium marismortui and its relationship to eubacterial protein L6 and other ribosomal proteins.

The amino-terminal sequence of ribosomal protein L10 from Halobacterium marismortui has been determined up to residue 54, using both a liquid- and a gas-phase sequenator. The two sequences are in good agreement. The protein is clearly homologous to protein HcuL10 from the related strain Halobacterium cutirubrum. Furthermore, a weaker but distinct homology to ribosomal protein L6 from Escherichia coli and Bacillus stearothermophilus can be detected. In addition to 7 identical amino acids in the first 36 residues in all four sequences a number of conservative replacements occurs, of mainly hydrophobic amino acids. In this common region the pattern of conserved amino acids suggests the presence of a beta-alpha fold as it occurs in ribosomal proteins L12 and L30. Furthermore, several potential cases of homology to other ribosomal components of the three ur-kingdoms have been found.

[Percutaneous angioscopy. Report of initial experiences with PTA and local lysis under angioscopic conditions].

In dogs and 11 patients a new endoscopic method for arteries has been developed. The approach is transfemoral, and endoscopy is combined with angiography, percutaneous transluminal angioplasty (PTA), and local lysis. An ultrathin endoscope with a diameter of 2.4 mm is used, which also provides a working channel with a diameter of 0.4 mm. Guide wires, contrast media, and drugs for local lysis can be inserted through this channel. Under fluoroscopic control, the endoscope is placed in the region of interest, followed by a special method for decreasing blood flow. Endoscopy is documented by video or by a high-speed camera. No complications have occurred. In all patients, it was possible to demonstrate the results of dilatation, recanalization, or local lysis before and after the interventional procedure.
