[Renal microaneurysms in necrotizing vasculitis--incidence in autopsy and clinical value].

Histological renal sections of 24 autopsied patients were evaluated for ectasias with greater than or equal to 2 mm diameter that corresponded to "microaneurysms" of radiologic nomenclature. Such renal "microaneurysms" of smaller and medium sized arteries were seen in 7/9 patients with periarteritis nodosa, 6/10 patients with secondary vasculitides and 1/5 patients with Wegener's Granulomatosis. Lumen ectasias in acute or subacute lesions of smaller and medium sized arteries were caused by fibrinoid necrosis of the arterial walls. Destruction of intimal elastic fibres and scar tissue within the arterial wall resulted in real aneurysmatic ectasias. Generalised, aggressive, necrotizing vasculitides show the highest frequency of microaneurysms. In that cases angiography can frequently establish the definite diagnosis by demonstration of microaneurysms.

[Differential diagnostic considerations in the assessment of habitual abortion].

The nonviability of fertilized eggs or the death of the embryo is a frequent event (34 to 60%). They are called recurrent or habitual in the case of more than 2 consecutive abortions. Maternal etiologic factors are discussed: Endocrine disorders (sexual hormones, diabetes, thyroid function), uterine congenital anomalies (septale uterus), uterine tumors (fibromyoma) and uterine synechia formation, cervical incompetence, infections (i.e. mycoplasma infections, chlamydia infection, cytomegalovirus, herpes simplex virus) and autoimmune-disease with antinuclear antibodies. According to the etiology the diagnostic procedures in cases of recurrent abortions consist of hormone analysis, basal body temperature, oral glucose tolerance test, thyroid function and endometrial biopsy. Sonographic examination of the uterus, hysterosalpingography, search for local and systemic infections and cytologic examination for genetic disorders of both parents are obligatory. At the end of the diagnostic scheme HLA-analysis of the parents must be included.

[Employment of a dental hygienist in Germany. 1. Experience and reminiscence of three decades].

Dental hygienists in most European countries and in America are highly qualified assistants to the dentist's daily professional life. Especially for long lasting achievements in periodontal therapy and maintenance, the hygienist's help is absolutely essential. All attempts to introduce a similar vocational reputation, as well as an adequate career training course throughout the Federal Republic have failed so far.

[Employment of a dental hygienist in Germany. 2. The cooperation between dentist and dental hygienist in day to day practice].

The comprehensive range of functions in a dental practice with special emphasis on periodontal cure requires cooperation amongst highly qualified personnel. The dental hygienist has a special standing and is responsible for as well as in charge of important treatment stages, as much as the dentist himself. The treatment stages are of greater relevance to long-lasting therapeutic achievements.

[Endoluminal angioplasty, in situ thrombolysis and arterial endoprosthesis. Immediate and long-term results (2 to 8 years): report of 5,216 operations].

Over a period of 8 years, 5,216 percutaneous transluminal angioplasties (PTA) and local thrombolysis were performed in the Department of Radiology of the University of Freiburg and the Hochrheinklinik of Bad Säckingen. From 1984 to 1989, 5,216 patients were reviewed clinically, and, in 360 cases, angiographically. The short-term results were compared with the long term results (i.e. 2 to 8 years after PTA or local lysis). The (stage IIb to stage IV) occlusive disease was treated by PTA. A successful PTA result was obtained in stage IIb in 73% of cases, in stage III in 68% of cases and, in stage IV in 36% of cases after 2-8 years including all regions of treated arteries. Complications occurred in 2.51% of cases. Over the last 2 years, dilatations, local thrombolysis and even intraluminal stent implantations can be controlled by ultrathin angioendoscopes.

[Scintigraphy using 111In-labeled antimyosin in Churg-Strauss vasculitis with myocardial involvement].

A case of Churg-Strauss vasculitis in a young woman is reported. Diagnosis was confirmed by muscle biopsy. Affection of lungs, kidneys and skin was evident. In addition, myocarditis was suspected on clinical evidence. A highly positive scintigraphy with 111In-antimyosin enabled diagnosis and assessment of damage to the myocytes. With a heart-to-lung ratio of 3.0 the accumulated activity in the myocardium was higher than usually found in myocarditis. This finding supports the hypothesis of an additional ischemic necrosis.

[New pregnancy following previous HELLP syndrome. A case report].

The HELLP-Syndrome (H = hemolysis, EL = elevated liver enzymes, LP = low platelets) is a well known and severe form of gestosis. In this case report we analyse the positive outcome of a pregnancy after HELLP-Syndrome has happened. The obstetric management requires regular control of the complete blood count, SMAC and coagulation.

Seat belt use and stress in adolescents.

This study explored the association of adolescent seat belt use with psychosocial risk factors in an urban minority population after the enactment of a mandatory seat belt law. Data on seat belt use, family support, feelings of being down, suicidal ideation, substance abuse, sexual activity, school troubles, and problems with the law were obtained from 541 self-report intake forms administered to an adolescent medicine clinic population from 1986 to 1987. Respondents were almost exclusively black and Hispanic; 315 (59%) were females and 222 (41%) males, with a mean age of 15.4. Seat belt use was reported by 249 (46%) and no or intermittent use by 292 (54%). Chi-square and Wilcoxon rank sums tests were used to examine associations between seat belt use and risk factors. Results showed that the group comprised of those reporting no and intermittent seat belt use was significantly more likely to feel down, have decreased home support, have problems with school and the law, have been on probation, and feel that life in general was not going very well. No association was found between seat belt use and cigarette, drug, or alcohol use or sexual activity without contraceptives. Taking into account the lack of observed behavioral information to validate such self-report questionnaires, these data nevertheless point to the nonuse or intermittent use of seat belts as a possible manifestation of a lack of self-care due to feeling down and/or preoccupation with family, school, or societal problems.

Low fluid intake lowers stool output in healthy male volunteers.

Inadequate fluid intake is though to be one frequent cause of chronic constipation, although this has never been shown. In order to test whether fluid deprivation has an influence on colonic function, we studied eight healthy male volunteers (median age 23 y, range 21-28 y) with respect to stool frequency, stool weight and mean oroanal transit of radiopaque markers in a control week with a fluid intake of more than 2500 ml beverages per day and in a week with a fluid intake of less than 500 ml beverages per day. The two weeks followed each other in randomised order (with a wash out week in-between) and were standardised as to nutrition and physical activity. Stool frequency diminished from 6.9 +/- 0.9 to 4.9 +/- 0.3 (mean +/- SEM) defaecations per week (p = 0.041) and stool weight from 1.29 +/- 0.20 to 0.94 +/- 0.17 kg per week (p = 0.048) during fluid restriction. Mean oroanal transit times were similar in the two weeks. In conclusion, our study shows that a relatively short period of fluid deprivation decreases stool frequency and stool weight in healthy volunteers. Therefore, a low fluid intake may well be an aetiologic factor for chronic constipation in some patients and too low a fluid intake should be corrected. There is no evidence, however, that an increase of fluid intake within feasible limits has a beneficial effect on chronic constipation.

[Follow-up in HELLP syndrome].

The so-called HELLP-syndrome, a special form of severe preeclampsia, is characterized by (H-) hemolysis, (EL-) elevated liver enzymes and (LP-) low platelet count. The data of three HELLP-cases are shown, the development and outcome of the pathologic laboratory parameters are presented. After a several months control it was evident that after delivery the initial improvement of all laboratory findings proceeds and lasts. A special medical treatment is not necessary.

HIV: early virus-cell interactions.

Two entry mechanisms of HIV occur in both lymphocytes and macrophages incubated with purified virus suspensions: (a) direct fusion of the viral envelope with the cell membrane and (b) receptor-mediated endocytosis via clathrin-coated pits and vesicles. Both mechanisms are shown in detail in a time-interval series of electron micrographs. The two lipid bilayers of the viral envelope and of the cellular membrane usually fuse seamlessly within 1-3 min at 37 degrees C, but occasionally membrane ruptures occur, leading to rapid cytopathic effects, i.e., vacuolization and cytolysis only a few minutes later. In the course of virus-cell fusion, gp 120 is integrated into the cell membrane; subsequent syncytia formation was observed after 1 h of incubation. The core disintegrates and releases the viral ribonucleoprotein through the opening at the fusion site into the cytoplasm.

[A new balloon-expandable plastic endoprosthesis. Initial report of experience with the malleable thermostent].

A new system of balloon-expandable stents for different purposes is presented. A special plastic material that can be shaped by a hot balloon technique or other internal or external heating modalities has been developed. The plastic material - a distant derivative of polyurethane - is caprolactone, which is soft from 52 degrees C to 70 degrees C. Using balloon techniques, the "thermo-stent" can be modeled to suit the form of the vessels, the bile ducts or the bronchial tree, as required. The balloon can be heated simply by means of a warm NaCl solution, electric matter in the balloon itself or microwaves. The plastic material can even be heated directly, which is especially beneficial if the stent needs to be thicker, e.g. in the bronchial tree, by an electric network within the plastic material, which heats the material to the necessary temperature by electric current. When the balloon is cooled after the dilatation the new form, the plastic has been modeled to is maintained exactly. The advantages of this thermo-stent will be the perfect adaptation to every individual situation in the intraluminal vessels, the bile ducts, and even the bronchi. The problems encountered hitherto with conventional metallic stents, e.g. high thrombogenicity, risk of metal intoxication or metallic rupture of filaments, have not be seen so far in animal experiments.

[High CA-125 values in miliary tuberculosis and ascites].

High levels of CA-125, the specific tumour marker for epithelial ovarian cancers are also found in inflammatory diseases of the pelvis. This case report of a 28 year-old woman with pathological CA-125 levels demonstrates the association between this finding and ascites caused by miliary tuberculosis. Tuberculostatic chemotherapy led to normalisation of the CA-125 levels within 8 weeks.

The long-term effects of percutaneous transluminal angioplasty, local catheter-lysis and stent-implantation.

During a period of 8 years 4750 percutaneous transluminal angioplasties and local lyses have been performed in the Department of Radiology of the University of Freiburg and the Hochrheinklinik Bad Säckingen. From 1984 to 1988 all patients have been controlled clinically and in 320 cases angiographically. The main localization of the lesions was the pelvic and the femoro-popliteal region. The short-term results have been compared with long-term results i.e. 2-8 years after PTA therapy. Long term success of PTA (2-8 years after the intervention) reached 85% in stage IIa, 73% in stage IIb, 68% in stage III and 36% in stage IV including all regions of treated arteries. 8 patients with stent implantation could be controlled and showed remarkably good results: 5 patients had patent stents after 16 months.

Highly potent and small neuropeptide Y agonist obtained by linking NPY 1-4 via spacer to alpha-helical NPY 25-36.

Analogues of neuropeptide Y (NPY) containing small N- and C-terminal segments linked via flexible spacer arms were found to exhibit receptor binding affinity constants almost as high as NPY as well as post- and presynaptic NPY-agonistic activities. One of the most active analogues contains N-terminal NPY segment 1-4 linked via epsilon-aminocaproic acid (Aca) to the C-terminal partially alpha-helical peptide amide segment 25-36. NPY 1-4-Aca-25-36 is the first highly potent NPY agonist, which is of considerably reduced size in comparison to the native hormone. The analogues are accessible by solid-phase synthesis using Fmoc strategy.

[Treatment of tubal pregnancy with prostaglandins: a multicenter study].

In this prospective multi-centre-study, the new treatment of tubal pregnancies by means of local prostaglandin (PG) F2 alpha and systemic PG E2 application was compared to the usual surgical technique of eliminating the conceptus. In 71 patients treated with PG, the method proved to be successful in 81%; 21 patients (19%) required surgical intervention later. With an initial beta-hCG level of 2500 mIE/ml, the success rate increased to 88%. The duration of hospitalisation was significantly reduced in the PG group compared to the patients treated by primary operation (3 +/- 1 versus 6 +/- 2 days). In the PG-group, only 2 of 24 hysterosalpingograms showed tubal occlusions after treatment, whereas occlusion was present in 3 of 8 patients of the surgically treated group. Four subsequent intrauterine pregnancies in the PG-group occurred compared to none in the surgical group. PG treatment of tubal pregnancy in patients with a low initial beta-hCG value (less than 2500 mIE/ml) revealed promising results with regard to reduced postoperative morbidity and future fertility.
