One-lung ventilation was used in 13 children, undergoing surgical excision of pulmonary hydatid cysts. Five had right-sided pulmonary cysts and eight had left cysts. An ordinary cuffed tracheal tube was introduced to the main bronchus of the healthy side. One-lung ventilation using 1-2% halothane in oxygen produced PaO2 values of 12.6-33.3 KPa and PaCO2 values of 4-6kPa. After operation there was a high frequency of right upper lobe collapse in cases of selective right bronchial intubation, but this cleared completely within 3-4 days. The technique proved to be a simple and effective method of isolating and ventilating the healthy lung.

The rationale of "crash induction" in patients with full stomach is reviewed. The technique does not precipitate regurgitation in normal patients having competent cardia, provided respiratory obstruction and IPPV are avoided during induction. On the other hand, in patients with incompetent cardia such as intestinal obstruction or hiatus hernia, excessive material may accumulate in the lower esophagus. The accumulation will be suddenly released with the cricopharyngeal relaxation subsequent to "crash induction". The stomach and esophagus should be adequately decompressed pre-operatively, and precautionary measures such as backward cricoid pressure must be taken during induction.

In 13 full-term primipara in active labor an initial single dose of preservative-free meperidine (100 mg) diluted in 10 ml of saline was injected epidurally (L2-3). In another 13 full-term parturients in active labor, 10 ml of bupivacaine 0.25% was used. Pain was scored by the linear analog scale. Onset of analgesia was 5.3 +/- 2.8 minutes following administration of meperidine, and 12.9 +/- 6.9 minutes following bupivacaine (p less than 0.01). Pain score decreased to 0 in 12 of 13 patients following meperidine administration and in six of 13 patients following bupivacaine (p less than 0.01). Satisfactory analgesia lasted 160.8 +/- 90.3 minutes following meperidine, and 103.5 +/- 42 minutes following bupivacaine administration (p less than 0.01). Subsequent supplementation by intermittent doses of 10 ml of bupivacaine 0.25% was more effective and less frequent following meperidine than following bupivacaine administration. Maternal sedation, nausea, and itching occurred frequently following administration of epidural meperidine, whereas hypotension, numbness, and motor dysfunction followed bupivacaine. In neither group was significant respiratory depression observed. All parturients delivered vaginally, 288 +/- 212.6 minutes following meperidine and 348 +/- 195.8 minutes following bupivacaine administration (p greater than 0.05); the neonates showed normal Apgar scores and neurobehavioral responses. Epidural meperidine, supplemented by subsequent bupivacaine as indicated, provides maternal sedation and satisfactory analgesia, and it diminishes the requirements of bupivacaine supplementation. The technique is advantageous in the parturient primipara.
The effect of neostigmine 0.05 mg kg-1 or pyridostigmine 0.25 mg kg-1 on serum cholinesterase activity was investigated in 20 adult patients undergoing elective surgery. Both drugs produced marked depression of enzymatic activity. The maximal depression was observed in samples taken 5 min after injection. The maximal percentage depression of enzymatic activity was not significantly different in the two drug groups. However, at 30-60 min after injection, the degree of depression was less in the neostigmine group. This may be attributed to the different plasma clearances of neostigmine and pyridostigmine.
