The stomachs of 118 donkeys were examined at postmortem during the period from March 1982 to February 1983 for Gasterophilus spp. larvae. G. intestinalis larvae clustered in groups near the boundary of the glandular and non-glandular epithelium of the stomach and infested 98.3% of the donkeys with highest numbers in July and lowest numbers in October. G. nasalis larvae were mainly attached near the pylorus and first part of the duodenum and infested 87.3% of donkeys with highest incidence in December and lowest in October. The ratio of the second and third instars of G. intestinalis to G. nasalis ranged from 71% to 29%. The percentage of donkeys infested with 1-100, 101-200 and 201-300 larvae was 72.0, 18.6 and 4.3% for G. intestinalis and 76.3, 8.5 and 0.8% for G. nasalis.

An obese male patient developed hypoxia, hyercarbia and radiological signs of pulmonary oedema/atelectasis in the dependent lung after surgery in the lateral decubitus position. This appears to have been due to ventilation-perfusion mismatch, although other factors were considered. The patient recovered following 36 hours of intermittent positive pressure ventilation of the lungs.
In 12 of 22 patients with coronary artery disease undergoing elective coronary artery bypass graft (CABG), verapamil (0.075 mg/kg body weight) pretreatment was given on initiation of cardiopulmonary bypass (CPB) before aortic cross-clamping (ACC), whereas no verapamil was used in 10 control patients. The volume of cardioplegia solution required to achieve and maintain cardiac asystole during ACC while on CPB was not significantly different in the two groups. After the release of ACC, the energy of direct current countershock required for defibrillation, the incidence of heart block, and the need for pacemaker were not significantly different. However, defibrillation was more readily achieved by a single countershock in the verapamil group than in the control group. Also, the incidence of ST segment changes after defibrillation was significantly lower in the verapamil group, suggesting that verapamil pretreatment before ACC may potentiate the myocardial preservation achieved by the cardioplegia, and hence may decrease the incidence of ischemic changes during the critical reperfusion period.

A review of 52 consecutive cases of congenital diaphragmatic hernia composed of 36 cases of Bochdalek hernia (B.H.) 13 of diaphragmatic eventration (D.E.), and 3 of diaphragmatic agenesis (D.A.) is reviewed critically. The operative mortality rate in Bochdalek hernia was 8.3% but became 20% with reference to the 15 infants with B.H. operated in the first 24 hrs. of life and 31.5% with reference to all 19 infants requiring repair of the diaphragmatic defect during the first day after birth. The operative mortality in D.A. was 100% and was nil in D.E. The mortality appeared related directly to prematurity, early age at operation, preoperative hypoxemia (PaO2 less than or equal to 60), hypercarbia, (PaCO2 greater than or equal to 60), and acidosis (pH less than or equal to 7.0), association with life-threatening anomalies affecting the lungs and its vessels, the heart, the size of the diaphragmatic defect and of the celomic cavity, the postoperative development of NEC and of obstructing intestinal adhesions. Eight of the 33 survivors (24%) with B.H. developed intestinal obstruction secondary to adhesions 2 months to 14 years after operation, of whom 7 required surgical intervention, and 3 bowel resections. The total mortality rate in B.H. was 14% and the rate in this series of combined defects was 15%.

Six patients were treated for food ileus within a three-year period. In three patients an obstructing phytobezoar (made up of bran, sauerkraut and pumpkin in one case each) was removed at surgery from the small intestine. In three other patients ingestion of a grapefruit, bran and turnip led to an ileus, which lasted for 3-9 days but responded to conservative treatment after 3-9 days. The common finding in all six patients was severe intraperitoneal adhesions of the small intestine (after repeated laparotomies in five, due to Crohn's disease in one). A high content of indigestible roughage in food, antacidity in conjunction with disordered gastric emptying are predisposing factors in phytobezoar formation. In addition to these well documented factors, intraperitoneal adhesions after laparotomy should also make one think of phytobezoar formation. In these circumstances gastrointestinal disorders are not rarely produced by spontaneously expelled phytobezoars. Even a complete ileus can sometimes be successfully treated by conservative measures.

Serum pseudocholinesterase activity in pregnant women was assayed spectrophotometrically using either propionylthiocholine or suxamethonium as substrate. All patients had a normal phenotype as indicated by normal dibucaine and fluoride numbers. However, the mean cholinesterase activity was lower than in the general population. There was a negative correlation between cholinesterase activity and the duration of neuromuscular blockade following suxamethonium, but no correlation was observed between the cholinesterase activity and the duration of block following atracurium.
