[A comparison of the results of endocardial and transesophageal electrostimulation of the heart in assessing sinus node function and atrioventricular conduction].

Forty five patients with cardiac arrhythmias were included in this study (20 had paroxysmal atrial fibrillation, 19, supraventricular tachycardias; 11 had sick sinus syndrome). In all patients endocardial electrophysiological study and transesophageal atrial pacing were performed in order to study sinus node function and atrioventricular conduction. The results of two methods of investigation have a high correlation. Quantitative criteria were developed for defining normal parameters of sinus node function and atrioventricular conduction during transesophageal atrial pacing.

[The clinical importance of atrial fibrillation-flutter induced by electric transesophageal stimulation of the heart].

A study was made of the clinical importance of atrial fibrillation-flutter (AFF) induced by using different modes of left atrium stimulation via the esophagus. Ninety-eight patients were entered into the study including 40 patients with a history of AFF paroxysms, 24 with risk factors of AFF development, and 27 practically normal persons. The stimulation modes applied permitted reproducing stable paroxysms of AFF in 85 percent of the patients with a history of arrhythmias and in none of the normal persons. As for the patients with risk factors, stable paroxysms of AFF could be induced in 33 percent of the cases. The specificity and sensitivity of transesophageal electrophysiologic stimulation (TEES) with the modes applied were 100 and 82 percent, respectively. The investigations have demonstrated that TEES appeared most effective when applied in the mode of an even increase of the frequency of the set pace up to 300 imp/min.

[Sinoatrial reciprocal paroxysmal tachycardia].

Detection rates and features of the clinical course of paroxysmal reciprocal sinoatrial tachycardia were examined in 34 patients. The features of the tachycardia should include its short-term and less marked clinical manifestations. In tachycardia, mean heart rate was 118 +/- 13 per minute. Long-term ECG monitoring was found to be the most adequate tool for the diagnosis of paroxysmal sinoatrial tachycardia. The potentials of transesophageal electrostimulation of the heart in the diagnosis and assessment of the clinical significance of that tachycardia were demonstrated to be limited due to instability of induced paroxysms.

[Sensitivity to the psycho-emotional test and the characteristics of the psychological status of ischemic heart disease patients].

Sixty-three coronary patients were subjected to mental testing, using the SMALL test and Kettell's 16 PF test. Positive test results were significantly more frequent in patients of the second and third functional classes with neurotic anxiety-depressive disturbances. In patients of the fourth functional class, the results of the tests were not dependent on their psychological status. Patients with the most marked neurotic disturbances may develop an anginal attack accompanied with ischemic ECG changes during the mental test.

[Arrhythmogenic effect of anti-arrhythmia agents in patients with paroxysmal supraventricular reciprocal tachycardia].

A retrospective study of the results of consecutive testing of antiarrhythmic agents was carried out in 82 patients with paroxysmal reciprocal supraventricular tachycardia. Arrhythmogenic effects of anti-arrhythmic drugs were detected in 22 (6.1%) of 359 pharmacologic tests. The electrophysiologic basis of arrhythmogenic effect is delayed impulse conduction at some point of the blood flow pathway, conducive to the emergence or maintenance of the re-entry phenomenon.

[Potentials and limits of transesophageal electrical stimulation of the heart in diagnosing supraventricular tachyarrhythmias].

The results of intracardiac electrophysiological examination were compared to those of transesophageal electrostimulation of the left atrium in combination with the recording of its electrical activity in 48 patients with paroxysms of reciprocal supraventricular tachycardia and in 28 patients with paroxysms of ciliary arrhythmia. In order to diagnose a form of tachyarrhythmia using the latter method some criteria were revealed. Transesophageal electrostimulation of the left atrium was shown to be quite an informative method to be widely used in clinical practice for the diagnosis of paroxysmal supraventricular tachyarrhythmia.
