[Changes in magnesium and calcium levels in blood and stomach tissue of patients with stomach cancer].

This study was performed to evaluate changes in Magnesium (Mg) and Calcium (Ca) levels in blood and cancerous stomach tissue of patients with stomach cancer, who were classified into four stages of malignancy and three groups of metastatic category. Mg and Ca were determined with atomic absorption spectrophotometer after blood and stomach tissue were digested with nitric and perchloric acid. Significantly high Mg levels in the early stages were found both in plasma and in whole blood. On the other hand, significantly low Ca levels were found in blood plasma in most stages and in all metastatic groups. By contrast, significantly high Ca levels were observed in whole blood in later malignant stages and in two metastatic groups. Significantly high Mg and low Ca levels were found in cancerous stomach tissue in patients with stomach cancer, as compared to their normal stomach tissue. The same tendencies were observed in each malignant stages and metastatic groups. These changes in Mg and Ca levels in blood and cancerous stomach tissue may reflect inherent peculiarities of stomach cancer.

[Animal experiments in electrophysiology of the canine stomach in reference to esophageal substitution with a tubal stomach in man].

In an effort to demonstrate and explain the peristalsis and intact pylorus-function of an oesophagus substituted by a transformed stomach-tube and despite total vagotomy, acute tests were undertaken on 14 dogs of mixed breed in the form of electromyographic examinations on the normal stomach, the stomach after vagotomy and with transformed stomach-tubes. Apart form slight frequency-decreases it was found that extracellular phasic bipolar contractions, traced an independent, individual current curve both after vagotomy and after transformation into a stomach-tube. The clinically observed intact function of the isoperistaltic stomach is to be explained with synergism after biomechanical changes and by the autonomy of the smooth stomach-musculature.

Behavior of microflora in the rat stomach: adhesion of lactobacilli to the keratinized epithelial cells of the rat stomach in vitro.

Certain indigenous bacteria isolated from rats and chickens were found to adhere to keratinized cells obtained from host stomachs and maintained in vitro. Only lactobacilli and staphylococci isolated from rats attached to the keratinized epithelial cells of the rat stomach, suggesting that they are dominant bacteria in the microflora of the stomachs of those animals. Indigenous lactobacilli heated at 60 C for 1 h or treated with the detergents sodium dodecyl sulfate, Tween 80, and Triton X-100 lose the ability to adhere to the keratinized cells of the rat stomach. Indigenous lactobacilli treated with formalin, however, retain the capacity to adhere to the cells. These observations suggest that surface structures of the bacteria are involved in adhesion of indigenous lactobacilli to the keratinized cells of rat stomach.

4 colostomated laying hens received 15N labelled casein as sole protein source for a period of 6 days. Two birds each were slaughtered on the 8th and 12th day of experiment. 23.5% of the total N in the glandular stomach and the gizzard were TCE soluble. The amino acid composition of the gastric proteins was broader than that of the proteins of flesh, with the exception of lysine and histidine. The atom % 15N' of the N of the stomach was roughly the same as that of the protein N; the level of labelling in the TCE soluble N fraction was only slightly higher. 2 and 6 days after the final administration of tracer N an appreciable proportion of 15N was found in the N fractions and in the basic amino acids contained in the crop and the stomach. From this is may be concluded that a considerable portion of the N-containing substances previously absorbed were secreted into the lumen of the crop and the stomach.

[Bacterial flora and nitrite production in the stomach after gastroenterostomy. Experimental aspects of the pathogenesis of carcinoma in the operated stomach (author's transl)].

A gastroenterostomy without enteroanastomosis leads to a change in the bacterial flora of the stomach, where-by in particular the proportion of nitrite-decomposing bacteria, is enhanced. This results in an increase of nitrite concentration in the gastric fluid, which may possibly be accompanied by an augmented production of carcinogenic nitrosoamine. This latter aspect is considered with respect to the origin of carcinoma in the operated stomach. Since the reported changes will be largely prevented by a Roux-Y-gastroenterostomy, this should be taken into consideration for reconstruction of the alimentary tract after gastric surgery.

[Stomach ulcer and stomach cancer].

About 5--10% of the gastric carcinomas develop from a gastric ulcer (ulcerocancer). This process, however, is, referred to gastric ulcers, too rare to indicate a prophylactic resection of the stomach on account of the danger of the development of cancer. Careful diagnostics and following control with X-ray examination and gastroscopy are, however, necessary in every gastric ulcer. The retrogression of the complaints neither proves healing of the ulcer nor benignity of the ulcer. Compared with the ulcerocarcinoma the differential diagnosis of the gastric ulcer is easy when locally advanced, patelliform carcinomas are in question. It is difficult in small carcinomas and especially in the carcinoma of the mucous membrane of type III. Individual radiologic and endoscopic signs and especially their combination are useful for the differentiation, but they may be misleading. A carcinomatous ulcer may radiologically and endoscopically look like a peptic ulcer and may apparently also fully heal. Therefore, a certain differential diagnosis is only possible with the help of an aimed biopsy of the stomach or the cytodiagnostics. The two methods serve further propagation. They should be used in every case, when an apparantly benign ulcer does not reveal a clear retrogression under effective treatment with carbenoxolone within four weeks. The examination of the gastric juice may be used for the differential diagnosis.

[Spontaneous rosette test in peptic ulcer of the stomach and stomach cancer].

The authors present the results from the study of the cellular immunoreactivity of immunocompetent lymphoid cells in 23 patients with gastric carcinoma and 30 patients with ulcer disease of the stomach, by applying the test for the confirmation of rosette-forming T-lymphocytes, according to the method of Bach and Lay. A significantly decreased number of T-lymphocytes was found both in case of ulcer disease of the stomach and gastric carcinoma. Best manifested is the depression of the cellular immunoreactivity in gastric carcinoma and it corresponds to the degree of the generalization of the process.

Humans

Proximal stomach and antrum in stomach emptying.

In a series of measurements of liquid-phase gastric emptying using a radionuclide marker on 35 subjects, five were identified in whom the proximal stomach and antrum were clearly distinguishable. Three of these subjects were normal controls and two suffered from systemic sclerosis. In the three normal subjects, analysis of the movement of the liquid showed the expected movement from fundus to antrum and thence through the pylorus. In the two patients with systemic sclerosis, there was, in contrast, evidence of mass retropulsion of the contents of the antrum into the fundus. In such cases, the measurement of stomach emptying based on the assumption of a single-compartment system in likely to be misleading.

Gastric Emptying

[Fiber endoscopy and radiology in stomach ulcer, stomach neoplasms and hiatal hernia: questions, timing and value].

In 306 patients, fiberpanendoscopy and radiology were performed within a time interval of 7 days or less. In 196 cases, radiology was performed before endoscopy and in 110 cases after endoscopy. In cases with histologically unproven divergent findings, follow-up studies were performed. In the diagnosis of gastric ulcer and gastric cancer, endoscopy with biopsy was more accurate than radiology. In the diagnosis of hiatal hernia, radiology was more accurate. Important additional findings difficult or impossible to observe with endoscopy were present in 36% of the radiological examinations. Thus, the first examination should be endoscopy when ulcer or carcinoma is suspected. In patients with uncharacteristic epigastric symptoms, radiology should be performed first.
