A clinical statistics survey of the operations was performed at the Department of Urology, Nissei Hospital during a 20-year period from 1982 to 2001. The total number of operations was 3,164, and the number of extracorporeal shock wave lithotripsy procedures was 143. Operations for urogenital malignancies urolithiasis and prostatic hyperplasia numbered 893 (28.2%), 328 (10.4%), and 767 (24.2%), respectively. The total number of operations has been decreasing owing to the development of new diagnostic and therapeutic modalities.
A clinical statistic survey was carried out on the operations performed at the Department of Urology, Handa Hospital between 1985 and 1994. The total number of operations was 2,532 and the number of operations other than extracorporeal shock-wave lithotripsy (ESWL) was 1,938. The mode of operation for urolithiasis has changed dramatically from open surgery to the ESWL and endourology.
The analysis of genomic DNA fragment patterns has revealed as a powerful tool for strain discrimination in Staphylococcus aureus; for use as an epidemiological marker, stability during the course of an outbreak is an essential prerequisite. Genomic DNA fragment patterns (SmaI restriction, pulsed-field electrophoresis) of four different epidemic MRSA strains were compared along with intra- and interhospital and country-wide spread over more than 12 months in Germany. Strain I was isolated from infections in 8 hospitals. In one hospital a subclone arised which differed from the original strain by 4 fragments. Strain II was spread among 4 hospitals, isolates from three of these hospitals exhibited a variability of one to three fragments in the 150-200 kb range. Two hospitals in the Hannover-area were affected by strain III; in 17 isolates of this strain a variability up to three fragments was found in the 170-200 kb range. Strain IV was isolated from 19 cases of infections in 3 hospitals in Berlin. The fragment patterns were completely stable. When S. aureus strains are typed by genomic DNA fragment patterns, a variability in a definite range of molecular masses during the course of an epidemic should be taken into consideration.

The smear taking practices of departments of genitourinary medicine were investigated by telephone inquiry. Departments seem to be broadly and equally divided into those where the smear is taken before the cervix is wiped or endocervical specimens are taken for microbiology, and those where the smear is taken afterwards. In the unit at Guy's Hospital we compared cervical smears taken in both of these sequences and concluded that, to diagnose more cases of premalignant cervical disease, smears should be taken before any other cervical samples.

Paris public hospital urology departments conducted a study which had three objectives: - to test the indicators of activity to determine whether they were well adapted to measurement of the surgical activity performed, - to determine the contribution of the PMSI to the measurement of this activity, - to propose actions designed to improve the representation of the surgical activity actually performed by urology departments. The results of this study show that one half of the operations performed do not correspond to the field of PMSI (day-only admissions, outpatients department). The indicators currently used for budget allocation or interdepartment comparisons of activity are inadequate and, most importantly, induce deformities, variable from one department to another, which severely affect the image of the activity produced by these indicators. Two main reasons can explain these deformities: - the use of the NGAP and the score to measure activity: there is a marked variation of the score for the same operation, from one department to another. - the very poor census of activity performed in outpatient surgery. These two reasons are not related to the medical activity. Definition of a unique nomenclature for surgical procedures would constitute a great progress, provided that this nomenclature allows us to accurately describe our activity and can be modified according to our needs. Although the PMSI is a marked improvement of the information system, its application, in its current state, will not radically change the situation. The PMSI must be enlarged in one way or another, to outpatient activity including procedures performed in the outpatients department. For these measuring tools of activity, which determine the calculation of our budgets, to be adapted to our practice, we must be able to analyse their relevance and introduce changes. We need to develop this field of medico-economic research in urology.
We investigated the clinical statistics of the operations and inpatients since the establishment of the department in June 1994 up to May 1999. The total number of inpatients was 1,269 (1,047 males and 222 females), and a total of 1,098 operations were performed. Extracorporeal shock wave lithotriopy (ESWL) was also introduced in 1997, and in addition, it seemed that the number of operations and inpatients would be increasing in future.
OBJECTIVES: To quantitatively evaluate the emergency activity of a teaching hospital urology department. To determine the epidemiology of urological medical and surgical emergencies. MATERIAL AND METHOD: Single-centre prospective study conducted in one of the 9 teaching hospital urology departments of the Paris region equipped with an emergency room. The on-call team consisted of an intern and a hospital student on the spot, a Senior Registrar on call. Evaluation was based on daily computer records of all consultations performed by the intern outside of the urology department, and all emergency surgical operations performed during the out-of-hours period (8:00 p.m. to 8:00 a.m.). RESULTS: A total of 1,715 consultations were performed, 15.6% of which required admission to hospital. Eleven per cent of consultations in the emergency department were transferred from another centre. Emergency surgery was performed in 5% of cases. 37 multi-organ harvests and 54 renal transplantations were also performed during this same period. Mean age: 53.18 years (range: 15-100). 12% of patients were over the age of 80 years. Sex-ratio: 78% of males, 22% of females. The most frequent presenting complaint was low back pain (25%). The diseases most frequently encountered were infectious (19.2%). 30% of emergency surgical operations were performed for drainage of obstructive pyelonephritis. Bladder tumours were the cancers most frequently responsible for emergency consultations (35%) and traumatology represented 8.7% of emergency consultations. CONCLUSION: Urological emergencies in a teaching hospital constitute a distinct clinical activity, which requires specific material and human resources. This exhaustive study could be used as a basis for review of the organization of the management of urological emergencies.
A clinical statistical survey of the operations was performed at our urology department during a 10-year period from 1993 to 2002. The total number of operations was 2,909 including 806 sessions of extracorporeal shock-wave lithotripsy (ESWL) that started in 1999. Since a combination unit of ESWL, fluoroscopic apparatus and digital video-urodynamics system was built next to the outpatient department in 1999, the number of operations and examinations has dramatically increased. Operations for bladders made up the greatest portion of all the surgeries and especially transurethral resection of bladder tumor and hydrodistention have been increasing.
