In vivo inhibitory effects of stimulation at the central end of the pelvic nerve severed from urinary bladder on urinary bladder contraction in rats.

Two- or five-Hz electrical stimulation of the central end of the left pelvic nerve severed from the urinary bladder in rats inhibited bladder contraction induced by intravesical infusion of Tyrode's solution. Inhibition of bladder motility by 2-Hz nerve stimulation appeared after pretreatment with strychnine (0.3 mg/kg, i.v.), naloxone (1 mg/kg, i.v.) and picrotoxin (1 mg/kg, i.v.). Hypogastric nerve stimulation, however, did not affect bladder contraction. These results suggest the presence of an inhibitory mechanism on the pelvic motoneuron activated by contralateral pelvic nerve stimulation in rats.

Metatarsal metastasis from transitional cell cancer of the urinary bladder.

Urinary bladder cancers can be grouped into three general categories: superficial, invasive and metastatic. Approximately 90% of malignant tumors of the urinary bladder are of epithelial origin and the majority of them are transitional cell carcinomas (TCC). Metastatic spread of urinary bladder cancers usually includes regional lymph nodes, the lung, the liver and the bones. The presence of metastasis tends to correlate with muscular wall invasion as often demonstrated at the initial diagnosis; consequently clinical bladder cancer represents a late phase of the disease. Although skeletal metastases of bladder cancers are rather common, they have been rarely described to occur in distal bones. For that reason, we report metatarsal metastasis from transitional cell cancer of the urinary bladder in a 59-year-old woman.

Primary and secondary prostatic adenocarcinoma of the urinary bladder.

Urinary bladder involvement by prostatic adenocarcinoma (PAC) is not well characterized in the literature. Fifteen consecutive cases of PAC diagnosed in the urinary bladder over a period of 10 years were reviewed. All bladder and prostate slides from each patient were evaluated. Eleven patients (group A) had synchronous PAC in the prostate. In these patients, bladder PAC occurred 2 to 11 years after the initial diagnosis of PAC in the prostate and tended to have a higher Gleason score than the original prostatic PAC. Four cases of bladder PAC in group A had areas with features of urothelial carcinoma, with focal positive immunoreactivity for thrombomodulin in 2 cases. Two patients (group B) had undergone radical prostatectomy for PAC 15 years earlier. The lesions in the urinary bladder in both cases showed histopathologic features similar to those seen in the previous prostatic malignancies. Two patients (group C) had histories of previously resected urothelial carcinoma. Bladder PAC was diagnosed at routine follow-up, and repeated prostate biopsy up to 2 years after the diagnosis of bladder PAC showed no evidence of prostatic PAC. PAC in the urinary bladder may be either primary or secondary. Secondary PAC is usually associated with high-grade and high-stage carcinoma in the prostate and may mimic transitional cell carcinoma. Primary bladder lesions may or may not be associated with a history of PAC in the prostate. The prognosis of patients with the primary carcinoma is favorable. HUM PATHOL 32:434-440.

A comparative study on the contraction induced by high K/Na deficient solution in the trachea, gall bladder and urinary bladder in guinea-pig.

In the present paper, a comparative study of high K solution was carried out on the mechanical response, wet and dry weights of tissue, and extracellular space of 14C-sorbitol in the trachea, gall bladder, and urinary bladder of guinea-pig. Hyperosmotic addition of KCl to the physiological solution produced a sustained contraction, while isosmotic substitution of K for Na in the physiological solution induced a contraction followed by a small sustained one in the trachea, gall bladder or urinary bladder. At 120 min after application of the substituted KCl, Na deficient solution, it increased cell water content in the smooth muscle cells of the trachea or gall bladder, but did not change that in the urinary bladder. Both the inhibition of contraction and the swelling in the trachea or gall bladder were prevented by the hyperosmotic application of sucrose or NaCl. The substitution for NaCl with the K salts with more permeable anions (I- greater than NO3- greater than Cl-) produced a greater inhibition of contraction in the trachea and gall bladder. Although the impermeable anion C2H5CO2- did not inhibit the contraction in the trachea, this anion partially inhibited it in the gall bladder. On the other hand, the inhibition of contraction in the urinary bladder was prevented by addition of pyruvate, oxalacetate or NaCl.(ABSTRACT TRUNCATED AT 250 WORDS)

Inhibition of EGFR signaling abrogates smooth muscle proliferation resulting from sustained distension of the urinary bladder.

Urinary bladder outlet obstruction results in sustained stretch of the detrusor muscle and can lead to pathological smooth muscle hyperplasia and hypertrophy. The epidermal growth factor receptor (EGFR) is a cognate receptor for mitogens implicated in bladder hyperplasia/hypertrophy. Here, we investigated the potential for modulation of this pathway by pharmacologic targeting with a clinically available EGFR antagonist using an organ culture model of bladder stretch injury as a test system. Urinary bladders from adult female rats were distended in vivo with medium containing the EGFR inhibitor ZD1839 (gefitinib, Iressa). The bladders were excised and incubated in ex vivo organ culture for 4-24 h. EGFR phosphorylation, DNA proliferation, and the extent of apoptosis in the cultured tissues were assessed. To verify that the smooth muscle cells (SMC) are a target of the EGFR inhibitor, primary culture human and rat bladder SMC were subjected to cyclic mechanical stretch in vitro in the presence of ZD1839. Levels of phosphorylated EGFR were significantly increased in the detrusor muscle with 12 h of stretch in the organ cultures. This activation coincided with a subsequent 23-fold increase in DNA synthesis and a 30-fold decrease in apoptosis in the muscle compartment at 24 h. In the presence of ZD1839, DNA synthesis was reduced to basal levels without an increase in the rate of apoptosis under ex vivo conditions. Mechanical stretch of bladder SMC in vitro resulted in a significant increase in DNA synthesis, which was completely abrogated by treatment with ZD1839 but not by AG825, an inhibitor of the related receptor, ErbB2. Our results indicate that the EGFR pathway is a physiologically relevant signaling mechanism in hypertrophic bladder disease resulting from mechanical distension and may be amenable to pharmacologic intervention.

[Imipenem and cefepime in empirical therapy of infectious complications after radical surgeries in patients with invasive cancer of the urinary bladder].

Urinary tract injection (UTI) is one of the main causes of severe complications and death in oncologic patients and particularly in patients with invasive cancer of the urinary bladder. In our Clinic an up-to-date system for infection control recording all cases of UTI in oncologic patients is being exploited since 2001. The paper presents the data on our observations during the period from 2001 to 2003 concerning typical UTI in patients with invasive cancer of the urinary bladder, etiological agents of UTI and their susceptibility to a wide range of antibacterials, including imipenem and cefepime, a 4th generation cephalosporin, used for adequate therapy of UTI in oncourologic patients.

H+/ATP stoichiometry of proton pump of turtle urinary bladder.

Urinary acidification in the turtle urinary bladder is due to a reversible proton-translocating ATPase. To estimate the H+/ATP stoichiometry of this pump, we measured the delta G'ATP in the epithelial cells and the maximum e.m.f. generated by the pump. The latter is the maximal transepithelial electrochemical gradient for protons placed across the epithelium that is needed to nullify the rate of transport and averaged 179 +/- 7 mV. The delta G'ATP averaged 50.1 kJ/mol. The H+/ATP stoichiometry of these bladders was 2.92 +/- 0.1. In other experiments, the bladders were poisoned by iodoacetate and cyanide and a variable transepithelial electrochemical gradient for protons was placed across them. It was noted that ATP synthesis occurred at a transepithelial electrochemical gradient for protons greater than 120 mV. The delta G'ATP in other bladders treated identically averaged 40.0 kJ/mol, giving a H+/ATP stoichiometry of 3.4 +/- 0.1. We conclude that the H+/ATP stoichiometry of the proton pump of turtle urinary bladder is approximately 3.

Scanning electron microscopic examination of reversible hyperplasia of the rat urinary bladder.

Urinary bladder damage caused by surgical incision, freeze-ulceration, or formalin instillation in male Fischer 344 rats was studied by light and scanning electron microscopy. The first two methods resulted in focal ulceration of the urinary bladder; the last induced diffuse mucosal damage. With each method, the damage was followed by regenerative hyperplasia and repair, the bladder mucosa returning to normal in 3-4 weeks. Epithelial cells in the hyperplastic areas had ropy microridges and uniform short microvilli on their luminal surfaces as observed by scanning electron microscopy. When the hyperplasia was marked, with nodular and papillary formation, occasional epithelial cells had pleomorphic microvilli on their surfaces. Rats treated either by surgical incision or freeze-ulceration had normal bladders after a 2-year observation period. Combined with results from previous experiments, pleomorphic microvilli are not a marker of neoplasia or irreversibility but appear with marked or prolonged mucosal proliferation even if reversible.

Scanning electron microscopy of cyclophosphamide-induced hyperplasia of the rat urinary bladder.

Urinary bladder damage caused by cyclophosphamide in male F344 rats was studied by light microscopy, scanning electron microscopy, and autoradiography. Cyclophosphamide was injected intraperitoneally at doses of 50, 75, 100, 150, and 200 mg. per kg. of body weight, and rats were killed at several intervals following treatment. Extensive necrosis of the bladder was followed by regenerative hyperplasia of the epithelium. A dose response was evident in the number and size of lesions induced and the time of regeneration and repair. Scanning electron microscopy disclosed pleomorphic microvilli on the luminal surface of cells during the early phases of hyperplasia. The greatest number of cells with pleomorphic microvilli occurred 2, 5 and 7 to 8 days following injection of 75, 100, and 200 mg. per kg. of cyclophosphamide, respectively. The hyperplastic lesions also had cells with short, uniform microvilli and ropy or leafy microridges on their surfaces. Autoradiographs of the urinary bladders showed marked labeling of epithelial cells in the early phase of hyperplasia. Of the few rats surviving beyond 1 year after a single injection, none had bladder lesions at any of the dose levels. The changes in the epithelium observed by scanning electron microscopy following cyclophosphamide injection were compared to those observed during bladder carcinogenesis.

Effect of prolonged experimental distension on the function and ultrastructure of the canine urinary bladder.

Urinary bladder distension was induced for 10 hours in dogs and micturition tests were made before over-stretching, immediately afterwards and 5 days later. Compliance and residual urine had increased and maximum pressure during voiding had decreased immediately after retention, but returned to normal after 5 days. Biopses for light microscopic and electronmicroscopic investigation taken from the anterior wall of the bladder on the fifth day showed submucosal haemorrhages and some necrosis. The consistent finding on electron microscopy was Schwann cell oedema, which was seen within the cytoplasm and between the cytoplasmic processes of the Schwann cells. Axonal degeneration was also a common finding. These anatomical changes, although found at the moment of functional recovery, may be linked to impaired conductivity of the nerves in the bladder wall, at least partly, causing its impaired function. Recovery of function occurs quite rapidly in healthy bladders.

Sex differences in o-phenylphenol and sodium o-phenylphenate rat urinary bladder carcinogenesis: urinary metabolites and electrolytes under conditions of aciduria and alkalinuria.

F344 male and female rats were administered 1.25% o-phenylphenol (OPP) or 2% sodium o-phenylphenate (Na-OPP) in combination with 3% NaHCO3 or 1% NH4Cl for 8 weeks and changes in the urinary bladder histopathology and the urinary components were examined. Administration of OPP with NaHCO3 resulted in marked urothelial hyperplasia in the urinary bladder of male rats, the response being less pronounced in females. OPP alone exerted no proliferative effect and NaHCO3 induced only slight hyperplasia in males. Na-OPP alone induced mild hyperplastic lesions only in males, this being completely prevented by concomitant administration of NH4Cl. The findings thus demonstrated a clear correlation between hyperplastic response and reported carcinogenic potential of these treatments. Of the urinary factors examined, increases in levels of pH and sodium ion concentration were positively associated with proliferative lesions especially in males, although the findings failed to explain the sex difference. Urinary concentrations of non-conjugated forms of OPP metabolites were also not directly correlated with the development of hyperplasias. Thus, changes in individual urinary factors presumably affect urothelial proliferation in combination rather than separately. The presence of OPP metabolites, including 2-phenyl-1,4-benzoquinone, in the urine may be unimportant in the OPP urinary carcinogenesis even under conditions of alkalinuria and high sodium ion concentration.

Office evaluation of the patient with an overactive urinary bladder.

Urinary incontinence affects between 13 and 17 million men and women in the United States, with an annual cost exceeding $26 billion. Overactive urinary bladder can be both neurologic (hyperreflexia) and nonneurologic (detrusor instability). The spontaneous involuntary detrusor contractions that occur with the overactive bladder often lead to urinary incontinence. Symptoms vary from patient to patient, with urgency, increased frequency, and urinary urge incontinence being the most bothersome complaints. Multiple components and interactions of the nervous system are required for appropriate storage and evacuation function of the bladder to occur normally. Thorough history taking and physical examination along with appropriate urodynamic testing are necessary for obtaining an accurate diagnosis and treatment planning. The quality of life for many patients with overactive bladder and the resulting incontinence can be dismal. Fortunately, most of these patients can be treated successfully.

Length-tension relations of smooth muscle from normal and denervated rat urinary bladders.

Urinary bladders of rats were denervated by bilateral excision of the pelvic ganglion and removed 10 days after the operation. They were filled with 0.75 ml saline and a longitudinal muscle strip was marked out, measured and dissected out. Strips from normal bladders filled with the same volume were used as controls. Denervated bladders were 4-5 times heavier than control bladders. Muscle strips from denervated bladders showed, in contrast to controls, marked phasic spontaneous contractions which were unaffected by tetrodotoxin, indicating a myogenic origin. Active tension in response to AC stimulation was measured at different lengths. In relation to the in situ length (Lin situ) at 0.75 ml the denervated strips had to be stretched to much greater extent than controls in order to reach optimum length (L0) for force development. Furthermore, the denervated strips shortened less in relation to Lin situ than the controls. If active length-tension relations were expressed in relation to L0, the difference between denervated and control strips was abolished. Maximal active force was the same for denervated and control strips. Water content increased significantly in denervated bladders. The results suggest a remodelling of the smooth muscle structure in denervated bladders; the characteristics of the contractile machinery seem, however, to be unaltered.

Nerve-mast cell interaction in normal guinea pig urinary bladder.

Urinary bladders of normal adult female guinea pigs were analyzed for anatomical evidence of nerve-mast cell interaction using light microscopy and electron microscopy. Nerves, ganglia, and individual nerve fibers were visualized on paraffin sections using immunohistochemistry with antisera against the neural antigens neurofilament protein and protein gene product 9.5, and sections were also immunoreacted with antisera against the neuropeptides substance P and calcitonin gene-related peptide. Separate mast cell populations were identified by counterstaining with toluidine blue and alcian blue. Mast cells of both types were found within nerves and intramural ganglia and were in close contact with individual nerve fibers displaying substance P- and calcitonin gene-related peptide-like immunoreactivity. Moreover, serotonin-immunoreactive mast cells were innervated with nerve fibers that reacted with antiserum against vasoactive intestinal polypeptide. At the ultrastructural level, these fibers were almost exclusively identified as unmyelinated primary sensory afferents. Mast cells contacted these fibers with lamellipodia that wrapped around and enclosed the fibers deeply within the cell. Close association between mast cells, nerves, and vessels was common. Ultrastructural evidence suggests that bidirectional communication occurs between nerve fibers and mast cells. These structures may participate in axon reflexes that regulate normal vascular and detrusor smooth muscle function and cause vasodilation, edema, inflammation, and bladder hyperreactivity. In summary, a close relationship exists between mast cells and peptidergic nerve fibers, including primary sensory afferents. Results suggest that bidirectional interaction could occur between nerves and mast cells.

Ontogeny of neuropeptides in the rat urinary bladder.

Urinary bladders of male rats at ages ranging from 1 day to 18 months were analysed for their neuropeptide content. NPY was detected at birth, VIP and CGRP at 1 week, and SP at 2 weeks. There was then a sharp increase in both the total amounts and concentrations of VIP, CGRP and SP until the rats had reached the age of 3-4 weeks. The total amount of NPY increased in two surges, between 2-3 weeks and between 4-5 weeks. The concentration of NPY showed a biphasic course, a decrease during the first two weeks and then an increase. In adults the total amounts of substance P and CGRP were higher than in younger animals, whereas that of VIP was lower. Following peaks at 3-8 weeks, the concentrations of VIP and substance P decreased with age, whereas that of CGRP remained unchanged. NPY was not followed beyond 8 weeks. The present findings offer a framework for future studies aimed at elucidating the functional role of neuropeptides in the regulation of micturition.

Induction of hyperplasia and its suppression by hydrocortisone in organ-cultured rat urinary bladder.

Urinary bladders from Fischer rats organ cultured in a chemically defined medium, Ham's F12, underwent transitional cell hyperplasia which persisted for the duration of the culture period (10 days). The hyperplastic response was initiated at 2 days of culture in basal epithelial cells as evidenced by [3H]thymidine autoradiography. After 2 days, cells classified as intermediate cells were observed replicating DNA in increasing numbers, whereas the frequency of basal cells replicating DNA decreased. The peak periods of basal and intermediate cell DNA replication were at 2 and 5 days, respectively. The total increase in the number of cells in the epithelium during a 10-day culture period was approximately 2.6-fold. The appearance of DNA-replicating cells before the appearance of mitotic figures indicated that the cells of the transitional epithelium are primarily G1 cells, The hyperplastic response in the transitional epithelium was significantly inhibited by hydrocortisone. Epithelia cultured in the presence of hydrocortisone also displayed less atypia than those epithelia cultured in its absence. Hydrocortisone concentrations of 2.1 and 21 micronM inhibited hyperplasia by 75 and 84%, respectively, Cells replicating DNA at 2 days of culture were considerably less sensitive to the hydrocortisone inhibition of [3H]thymidine incorporation into DNA than cells replicating DNA at 4 days of culture. The possibility is discussed that basal and intermediate cells may have different sensitivities to hydrocortisone.

[The systematic histologic elaboration of urinary bladders with different pathologic findings (urinary bladder carcinoma, tumor-free aging urinary bladders)].

We report our systematic histological examinations of 16 urinary bladders (11 bladders with cancer, 5 bladders was tumour-free). All residual tumour-free portions in bladder with multiple or solitary cancers demonstrated various precancerous variations of urothelium, not only near the tumours. In 1 urinary bladder among 5 from elder patients, we diagnosed a clinically undetected cancer. In a few cases, we correlated histological examinations with graphical demonstration (through a computer) of the exophytic tumour portions. Our results agree with individual therapy of urinary bladder cancer.
