Practitioners occasionally examine patients who have lost teeth as a result of accidents. In this unusual case, a patient's tooth was removed 3 times in 12 hours.
Maxillofacial trauma is the main cause of emergency admittance to dental clinics. Mental retardation and epileptic status are important factors in an increase in the risk of dental injuries. Tooth avulsion, which is the total displacement of a tooth out of its socket, is an infrequently observed entity. Maxillary central incisors are the most commonly affected teeth. The case of a patient with severe dental injury resulting from an epileptic attack is presented. He had several teeth avulsed and displacement of a tooth into the soft tissue of the chin.

Therefore, it is essential that school nurses be well prepared to intervene when such a dental emergency arises. It is also imperative that school nurses and school personnel are fully equipped to manage a tooth avulsion. Time is of the essence when attempting to save an avulsed tooth. The goal is to successfully reinsert the avulsed tooth and to preserve the periodontal ligament. The school nurse is responsible for implementing the appropriate actions needed to save the avulsed tooth.
The traumatic avulsion of a tooth presents unique and challenging difficulties in patient management. The successful replantation of the avulsed tooth is influenced by many factors occurring from the moment of injury to the moment of replantation. Immediate and decisive treatment is essential in this time period to avoid subsequent inflammatory root resorption and ankylosis.
Tooth avulsion is common in children, and emergency management in these cases is critical. This management can be made, not only by a dentist but by people who are present where the accident occurs. Consequently, knowledge of tooth avulsion is fundamental for school professionals working with children. The purpose of the present study was to evaluate the knowledge of tooth avulsion of school professionals from Adamantina, São Paulo, Brazil. For that purpose a questionnaire, including questions regarding emergency procedures for tooth avulsion, was answered by 117 teachers. The results demonstrated that 75.2% of school professionals knew the importance of emergency management and 60.6% would look for a dentist for treatment of the cases; 18.8% would reimplant the tooth and 7.6% would keep it in milk. This study showed the lack of knowledge of teachers on tooth avulsion; educational campaigns are necessary to improve the emergency management of tooth avulsion.
Successful management of the avulsed tooth begins at the accident site. Correct first-aid care followed by proper management in the dental surgery will significantly improve the prognosis for the avulsed tooth. The major aim following avulsion is the prevention of further damage to the periodontal ligament and, if possible, the maintenance of pulp vascularity. Studies have indicated that variables such as the extra-oral period, the storage medium and the stage of root development of the avulsed tooth significantly influence the final outcome. Endodontic therapy and chemical conditioning of the root surface after an extended extra-oral period has the potential to delay the resorptive processes of a replanted tooth.
This report describes the treatment sequence after traumatic loss of a maxillary central incisor in a 15-year-old patient. Following extraoral root canal treatment and initially successful replantation, the case presented 9 years later with complete root resorption. After augmentation with an autologous mandibular corticocancellous graft, a dental implant was placed in a second stage surgery. The case highlights the challenge facing clinicians in providing the appropriate standard of care for today's treatment options.
