Arrangement of anatomic-type artificial teeth into balanced occlusion.

The mechanical factors which are involved in the arrangement of anatomic type artificial teeth into balanced occlusion have been described. Many of the mechanical problems encountered during artificial tooth arrangement have been listed, together with suggestions for their solution. Hanau's Laws of Articulation have been utilized in a practical manner in formulating some of the solutions to tooth arrangement problems when establishing balanced occlusion. Trial dentures which present ideal occlusions in the articulator may not always do so when examined in the patient's mouth. Possible reasons for these differences are listed, and methods for making the necessary corrections are described.

Porous titanium endosseous dental implants in Rhesus monkeys: microradiography and histological evaluation.

Artificial tooth roots with porous surface coatings were fabricated by sintering spherical powder of titanium alloy to solid cylindrical cores. The tooth roots were implanted subgingivally in healed mandibular premolar extraction sites of fifteen Rhesus monkeys. Supracrestal abutments were screwed into pretapped holes in the superior aspect of the primary subgingival stage four to eight weeks after implantation of the root. Clinical evaluations were performed monthly. Ten animals were sacrificed for histological evaluation of the functioning free standing implants. Of twenty-nine implants placed, three were lost and four were rated failures on the basis of histological evaluation. Postmortem evaluations revealed bone growth into the porous surface coating of the primary stage of all the implants. The most characteristic features which could be used to describe differences in the implant histology were the buccal and lingual crestal bone heights measured in relation to the root porosity. Twelve of sixteen implants had crestal bone heights within one millimeter of the superior aspect of the root. Four other implants displayed excessive bone recession, revealing as much as one half of the root porosity supracrestally. The four implant failures could be related to unfavorable features of recipient bone sites. The results demonstrate that the bone growth into the porous surface coatings of artificial tooth roots is an efficacious method of dental implant fixation.

Alloys

The upper anterior sectional denture.

An upper anterior sectional denture is described which consists of (1) a cast cobalt-chromium framework with two passive clasps that contact undercuts on the distal surfaces of the most posterior remaining teeth and (2) a labial flange that carries the replaced tooth or teeth and engages the labiointerproximal gingival undercuts of the anterior teeth adjacent to the edentulous ridge. The cast framework is inserted occlusogingivally, and the labial flange is inserted labiolingually. Both sections are held together by the frictional resistance of the split post of the framework that fits into a tube on the labial flange and hollowed-out artificial tooth. With this type of prosthesis, certain clinical problems can be handled without abutment tooth preparation and with a minimal amount of chair time.

Early formation of dental plaque on platic films. 1. Light microscopic observations.

In human subjects with healthy gingiva thin transparent plastic films were applied to the buccal surfaces of premolars in the upper and lower jaws. The films were left in place for peiods of 15, 30, 60, 120 and 240 minutes. The presence of coccoid bacteria, leukocytes and epithelial cells was investigated in an area adjacent to the gingival margin. The results showed that by 15 minutes coccoid bacteria had become attached to the artificial tooth surface. The number of microorganisms gradually increased during the time of the study. Large numbers of cocci and the formation of micro-colonies were observed after 120 and 240 minutes. Increasing numbers of leukocytes and epithelial cells were also found during the period of observation.

Direct bonding in the management of an ankylosed second deciduous molar.

A simple and efficient bonding procedure has been described for the management of an ankylosed deciduous second molar. This has, in our experience, provided a satisfactory occlusal and periodontal environment in selected patients. Based on a limited number of cases, restorations have been maintained for about two years to date.
