The problem of the spinal osteochondrosis treatment has not been solved yet. Commonly applicable manual therapy is not always effective. In order to liquidate complex degenerative hypoxic processes in soft tissues of spine, cervical, thoracal and lumbar parts of spine of 468 patients aged from 20 to 80 years, in line with the manual therapy, have been exposed to hyperbaric oxygenation with constant magnetic field. In case of persistent pathologic process in spine a single kenalog-novocaine blockade after Shneck was additionally applied. Prolonged good and excellent results have been achieved with all patients during 3 years. Mean term of invalidity equalled to 7 days. Proposed method of spinal osteochondrosis treatment in polyclinic facilitates the economic situation of hospitals and saves patients from hospital bed.
A method of the treatment of the painful syndrome in spinal osteochondrosis is suggested. It lies in the action on functionally significant points specific for each level of injury. To establish blockades, use is made of the preparations on the basis of snake venom, administered intracutaneously or of nitrous oxide injected subcutaneously. Blockades are made once every 4 days. In the majority of cases, the beneficial effect was attained after 1 to 3 sessions. The maximum number of blockades was 6-7 in patients with a severe pattern of the painful syndrome. Out of 240 patients with spinal osteochondrosis, good clinical effect was achieved in 237 patients. The time of the patients' stay at the hospital reduced from 31.5 to 12-18 bed days. The high efficacy of the method suggested, simplicity of performance and minimal expenditures permit recommending it as one of the methods for the treatment of the painful syndrome associated with spinal osteochondrosis.
Spinal osteochondrosis, previously known as Scheuermann's disease and spinal osteochondritis, is common and is probably becoming increasingly prevalent. Its incidence in patients whose primary complaint was of backache was shown to be twice as great as that in the general population. There was also a significantly higher incidence of lower lumbar spondylosis in patients with previous osteochondrosis compared with those without. Osteochondrosis is thereby shown to be an important aetiological factor in spondylosis.
Patients with neurological symptoms of spinal osteochondrosis in exacerbation took reduced intensive courses of physiotherapy which consisted of ultraphonophoresis of 1% oil solution of polar fraction of lipids from sulfide mud (eplir). The course included 12 daily procedures. Positive functional changes and those in vertebroneurological status support high efficacy and good tolerance of such short courses.
The paper contains a comparative analysis of the distribution of the blood groups of the ABO system in 1731 patients with syndromes of spinal osteochondrosis and in 9013 representatives of the population. Besides the frequency indices the authors studied as well the index of a relative morbidity A/O the "racial-biochemical" Hirshfeld index and the relationship of the frequency of group 0 to the sum of frequency in the A, B, AB group. It was demonstrated that there was a significant drop in patients with spinal osteochondrosis of the frequency of the 0 group and an increase of the group A. These deviations demonstrate a correlation with the sex, age by the onset, localization of the process longitudinally to the spine and hereditary loading (cases of repeated cases in the family). The revealed regularities may be considered as evidence of the influence of the genotype on the development of spinal osteochondrosis as well as one of the mechanisms determining the immune response in this disease.
A number of case studies were followed for 10 years. Children suffering from postural defects, structural scoliosis and spinal osteochondrosis were separated from a normal group. Spinal mobility and posture were compared with the mobility of normal, healthy boys and girls aged 5-14 years. In postural defects curves similar to the physiologic one were observed. The range of motion was not physiologic in scoliosis. In this disease, rotation-flexion thoracic hypermobility with decreased lumbar flexion ability was found. A physiologic trend was found in spinal osteochondrosis, although sometimes the rotational mobility of the spine was reduced. An extremely high increase of thoracic kyphosis was found only in children aged 14 years.
AIM: To assess the response to chondroitin sulfate (structum) in low back pain (LBP) due to spinal osteochondrosis. MATERIAL AND METHODS: 30 patients (mean age 51.4 years) with a definite primary LBP took structum in a dose 1 g/day for 24 weeks. The diagnosis was made according to WHO recommendations (2000). The response was assessed with uniform international questionnaires and visual analogue scale. RESULTS: The treatment reduced pain syndrome and improved spinal function in 73.3% patients with LBP studied. CONCLUSION: It would be valid to include long-acting chondroprotective drugs in the program of LBP in spinal osteochondrosis.
132 patients suffering from spinal osteochondrosis with pain syndrome were examined for pain perception and psychological status. Pain intensity in such patients showed association with hypochondriac and anxiodepressive disorders combination with rigidity and development of fixations. Deterioration of life quality correlated with severity of pain syndrome and such features as unsatisfaction, tension, anxiety, rigidity, fixation of attention on personal feelings, pessimistic outlook, low stress resistance. Psychological characteristics and quality of life depended on gender and location of vertebrogenic pain syndrome.
The authors have investigated the possibility of objectivization of the treatment of the patients with osteochondrosis of the spine. 86 patients with spinal osteochondrosis and 149 patients with cervical osteochondrosis have been studied, all of whom were treated with variable vibromagnetic field and kinesitherapy. It has been established that the patients' tissues in the area of the biologically active points acquire diode properties (which normally are the electric current conductors). The conduction above the biologically active points after the treatment is restored.
Microwave therapy (millimetric waves) was used for treatment of spinal osteochondrosis. The effect was manifest as early as the first sessions. The pain alleviated, muscular tension decreased, movements recovered, convulsions and numbness of the legs ceased. Possible biophysical mechanisms of EHF waves are discussed.
The paper presents the conception of primary prophylaxis of a prevalent disease--spinal osteochondrosis. The conception rests primarily on the main spinal functions and implies training of nonspecific resistance mechanisms under the conditions of present-day mode of life.
Traction is one of the methods in pathogenetic therapy of spinal osteochondrosis (SO), but not all the traction techniques can be applied in the acute stage: the antalgic posture, impaired motor activity create difficulties for conducting procedures while the acute process endangers paradoxical reaction to the treatment. Adjuvant laser therapy reduces pain syndrome and enhances functional capacities of the neuromotor system thus providing favourable conditions for spinal traction.
AIM: To compare effectiveness of manual and bioresonance therapies for fibromyalgia (FM) in spinal osteochondrosis (SO). MATERIAL AND METHODS: The trial enrolled 60 FM patients with x-ray diagnosis of SO. In addition to routine clinical examination all the patients have undergone kinesthetic study with estimation of the muscular syndrome index, brain echoscopy, neurological examination, electropuncture diagnosis. Group 1 patients received manual therapy (MT) and point massage (PM); group 2 patients received MT, PM and bioresonance therapy (BRT). The treatment took 5-6 weeks. The examinations were made before the treatment and 1-1.5 months after it. RESULTS: The response was observed in both the groups, but in group 2 it occurred more frequently and earlier, was higher and longer. BRT produces no side effects, has no contraindications, acts on the body systemically. It is rather effective against symptoms of neurocirculatory dystonia frequently diagnosed in FM patients.
Altogether 130 patients with associated lumbar spinal osteochondrosis and the radicular syndrome were placed under observation after operation in the form of laminectomy and removal of hernia of intervertebral discs. On days 2-3 after the operation the patients were given different physiotherapy modalities: sinusoidal modulated currents (SMC), electroacupuncture by sinusoidal modulated currents (EAP) and acupuncture (AP). The control group patients were administered conventional drug therapy. According to the data on the disease clinical picture and the rate of impulse conduction in the motor nerve, the treatment efficacy was higher in patients on physiotherapy as compared to the controls. In patients with the predominance of the painful syndrome, EAP and AP were most effective; EAP was most effective in associated pains and motor losses; SMC appeared effective in isolated motor disorders. The drugs slightly reduced the pains but did not recover motor function.
Acupuncture used in cervical-thoracic and lumbar pain arrested the pain completely in 41.7 and partially in 53% patients with spinal osteochondrosis of the neck and chest spine. In lumbosacral osteochondrosis pain eliminated in 33 and 61.2% patients, respectively. If acupuncture is ineffective, vacuum blood letting is recommended as an adjuvant treatment to relieve pain.
Short-term mud therapy (10 daily peat [correction of moor] applications) has been performed in patients with neurological symptoms of spinal osteochondrosis. Such short-term courses are indicated in the absence of exacerbation of the basic and associated diseases under domination of the radicular and reflex syndromes. High efficacy and good tolerance of the treatment are confirmed by vertebroneurological investigations and functional diagnostic tests.
Complex genetic-mathematical analysis (component decomposition of the phenotype variance, segregation analysis, estimation of penetrance parameters) of 324 probands' families with neurologic manifestation of spinal osteochondrosis (3437 relatives of I-III degree) has been carried out is shown that the structure of liability to the disease studied is better approximated by semi-continuous phenotype model within the limits of which contribution of genetic factors reaches 80% (63-66% owing to autosomes and 14-17% owing to gonosomes) under considerable influence (20%) of environmental force (14% accidental and 6% systematic).
