[Combined treatment with lithium salts and socioenvironmental therapy in patients with manic depressive psychosis. Considerations on the results obtained in 1984-88 at the Mental Health Service of Local Health Unit No. 32 in Moncalieri].

A study by National Health Unit - U.S.S.L. 32 on 28 patients suffering from manic depressive psychosis has shown the great importance of social and environment factors in the treatment of this disease. In two test groups given the same quantity of lithium salts, the one also receiving a social and environmental assistance revealed a net reduction in the number of hospital admissions for manic symptoms whereas no significant improvement was obtained in the other group.

Use of environmental manipulation and modified informal reality orientation with institutionalized, confused elderly subjects: a replication.

The effectiveness, for confused elderly subjects, of modified informal reality orientation and environmental manipulation without any 'classroom' reality orientation was investigated with 10 experimental subjects in one ward and 10 control subjects in another. All subjects were assessed prior to the start of the intervention and after 6 and 12 weeks of treatment on cognitive and behavioural measures. While the control group showed decline on all measures except one, the experimental group improved in ward orientation and cognitive status and showed no decline on the behavioural measures.

Social dysfunction and treatment failure in schizophrenia.

Recent evaluation of a 20-year-old experimental ward resocialization program for chronic schizophrenics indicates that the general level of activity, much of it social, was disruptive to the psychological functioning of patients, particularly sicker ones. Antipsychotic drugs positively affected psychological functioning and also decreased social behavior. A review of other studies indicates that we are not unique in finding that intensive socioenvironmental therapies have adverse effects on schizophrenics. Furthermore, a wide range of studies indicates that schizophrenics are prone to avoid social interaction and show a decrement in functioning as the intensity of such interactions increases. Unfortunately, although various hypotheses seem feasible, little is known about the reasons for schizophrenics' social dysfunction and little research is presently being done. Given the recent findings that neuroleptic drug treatment by itself is insufficient to keep many patients out of the hospital and that those that stay in the community show an increase in withdrawal, the development of successful treatment programs for schizophrenia may hinge upon our learning more about the nature of and reasons for the schizophrenic's social dysfunction.

Reality orientation, validation and the reality of the disoriented old old.

Approaches of disoriented elderly people can be distinguished in two main perceptions, Reality Orientation and Validation. Reality Orientation appeals to still existing functions and helps people to get oriented toward the reality. Validation goes with people into their inner world, where feelings dominate and facts are no longer important. In this article both approaches are explicated. Further, it is stated that measuring the effects of Reality Orientation or Validation have the same pitfalls, such as lack of adequate scales, unclear definitions of the behaviour and symptoms of patients involved, and the risk of measuring a Hawthorne effect. The author states that Validation meets the demands of disoriented people better than Reality Orientation. She argues to think in-depth about the aims of psychogeriatric care, in order to define what effects should be pursued and measured. Nursing diagnoses should be used instead of medical diagnoses.

[Reality orientation and the reality of the nursing home resident].

In this article the objectives of Reality Orientation Therapy (ROT) and its applications to demented elderly people are analysed. Opinions on the effectiveness of ROT are more optimistic than specialised literature and case studies seem to justify. Until now ROT has, in theory, been considered suitable for all stages of dementia. New developments mentioned in literature and actual experience on a psychogeriatric ward changed ideas on how to approach demented elderly people. Partly ROT arises from aversion of healthy people to very severe decay. Particularly in the advanced stages of dementia the approach advocated by Validation Therapy seems to be more appropriate. Cooperation between professional nurses and psychologists can contribute to a more differentiated treatment of demented patients in geriatric hospitals.

[Rehabilitation and the pathology of dementia].

To recognize the influence of the social and physical environment on demential pathology has methodological and therapeutical implications. Since the beginning of the seventies, many works have demonstrated that senile deterioration is not a continuous nor insurmountable process. There is no ineluctable evolution, independent of environment. The different therapeutical strategies can be analysed in terms of behavior modification through an action upon the physical and social determinants of the environment. The authors present two complementary methods of revalidation: an individualised reeducation of necessary behaviors for a social autonomy (toilet, dressing, continence) and a program of physical and psychic activation, to maintain or reestablish social skills (management of the institutional environment, physical mobilisation, ergotherapy and sociotherapy). These methods are relevant to the functional analysis and behavior modification. They are in concordance with the goals of the contemporary gerontology.

[Reality orientation training: a provisional evaluation].

This article takes further on REality Orientation (RO), a therapeutical approach developed and practiced on behalf of institutionalized psychogeriatric patients. RO is called a milieu- or sociotherapy dependent of the aspect that is stressed most. Goals, methods, potential target groups and studies of its effects are discussed whereupon an evaluation of RO is formulated for the present. The importance of role and function of nursing staff along with their attitude is stressed, for their cooperation strongly adds to the success or failure of RO-programs.

The therapeutic use of ritual and ceremony in the treatment of post-traumatic stress disorder.

The therapeutic purposes and effects of specially designed ceremonies in the treatment of persons with post-traumatic stress disorder are described. Ceremonies compartmentalize the review of the trauma, provide symbolic enactments of transformation of previously shattered relationships, and reestablish connections among family and with society in general. Four ceremonies used with Vietnam combat veterans are described which focus on the themes of separation from and return to the family, forgiveness of the living, and releasing the dead. Ritual and ceremony are highly efficient vehicles for accessing and containing intense emotions evoked by traumatic experience. Evaluation by family and veterans judge these ceremonies to be the most effective components of treatment.

[In the footsteps of Pinel at the Salpêtrière, concerning some unedited documents].

In 1802, the government transferred the treatment of all indigent mentally ill women in the Paris region to Salpêtrière Hospice where Philippe Pinel was physician-in-chief. Four newly identified folio registers in the Paris hospital archives document how Pinel applied what he called "traitement moral" to the women under his care. He elaborated a method of supervised living and encouragement to work. The newly discovered case histories reveal that, in 3 years and 9 months - the timespan of Pinel's report to the Academy of Sciences - 265 out of 1077 patients returned to their families, but 107 women, though cured, remained under police supervision. There appears to be no follow-up study.
