[Psychotherapy with elderly patients. Experiences from a psychiatric psychotherapeutic consultation service].

Experiences with psychoanalytically oriented psychotherapy with the elderly have been made in a psychiatric consultation service at a general hospital. The life-long importance of psychodynamically relevant conflicts is illustrated by two case examples of psychosomatic patients with asthma and functional symptoms of the gastrointestinal tract. The experiences with short-time psychotherapies described here are encouraging and confirm results of other groups demonstrating the efficiency of psychotherapeutic interventions with the elderly. Special problems of psychotherapy with older patients in a psychiatric consultation service and implications for the routine medical treatment of general-hospital patients are discussed.

Social requirements of shortness and efficiency influence the psychotherapeutical Situation in many ways. Based on this background the author first deals with the concept of time which showed mythic features in Sigmund Freud's writings while today we assume linear processes. This has a direct effect in dealing with therapy time where currently short-time therapies dominate. An understanding of therapy time as a setting variable enables a structuring procedure in both out-patient and in-patient therapies where the end of therapy is included early. The article ends with some remarks on how the therapist handles his time.

[An unusual case of non-structural scoliosis (case report)].

The case of a female patient with conversion scoliosis is described. At 24 years, the patient underwent surgery for herniated disks in the L4-L5 and L5-S1 regions and a short-time relief was achieved. At age 28, she was admitted to our department because of severe pain and antalgic posture. Because radiographs of her spine showed no findings corresponding to her symptoms, psychological examination was indicated. This revealed traumatic emotional experience in her family of origin and later also in her foster family, which unfavorably affected her married life. Psychotherapy, including cognitive behavioral treatment, hypnotherapy and audiovisual stimulation, markedly improved the patient's mental and somatic state during two periods of hospitalization. The effects of psychotherapy, physical therapy and adaptation of her physical activity regimen were still evident at 1 year after her therapy was commenced.

[Effects of relaxation therapy as group and individual treatment of chronic tinnitus].

42 patients, suffering from chronic tinnitus, participated in our psychological orientated treatment consisting of relaxation therapy with autogenic training according to J. H. Schultz. The results of individual therapy are compared with group therapy. Using visual analogy scales the therapeutical efficiency can be tested. The individual estimated loudness and annoyance of tinnitus are registered as well as a general emotional status. The results show a positive short-time effect in most cases. A reduction of tinnitus loudness and annoyance after individual and group therapy is seen directly. A positive effect throughout the whole treatment is only found in individual therapy. Concerning the group therapy, many of our patients reported an increase of the pretherapeutical estimation of tinnitus loudness and -annoyance. We believe that the permanent confrontation with the tinnitus problem may advance the psychological conflict in many cases. Therefore psychological management of tinnitus should be concentrated on a temporary limited support aiming to the neglect of tinnitus sensation.

[Sex function disorders after myocardial infarct. Diagnosis, counseling and therapy].

Sexual activities are possible and important even after myocardial infarction. Sexual complaints after a coronary heart disease are frequent, in most cases functional and primarily caused by psychological or psychosocial problems. There are good reasons to provide for psychological counselling and short-time therapy of sexual disturbances in the postcoronary rehabilitation programme. Beside clarifying the organic situation (physical load capacity, length of the interval after myocardial infarction, genitovascular insufficiency, medication etc.), the hormonal and functional situation, it is necessary to pay particular attention to a sufficient counselling and psychic management of the patient's sexual problems (fear of the dangers of overexertion, depression, fear of failure, the undigested change of familial and occupational status etc.). It is shown that these demands may be taken into account in multidimensional therapeutic work which has been performed in a special rehabilitation centre treating patients suffering from heart and circulatory diseases.
