Risk analysis, systems analysis, and Covey's seven habits.

This article relates problem solving to the common approaches of the gestalt-holistic philosophies of systems analysis, risk analysis, and Stephen Covey's Seven Habits of Highly Effective People. Guiding principles developed on the basis of these philosophies provide the foundations for methodological frameworks that build on a plethora of theory, methods, tools, and techniques. Although systems analysis and risk analysis differ in their historical evolution and technical maturity, both study and solve problems using methodological frameworks that share a holistic vision.

Systems analysis of intracranial pressure. Comparison with volume-pressure test and CSF-pulse amplitude analysis.

Systems analysis is explored as a method of evaluating intracranial pressure (ICP). The intracranial cavity is characterized by a transfer function that is evaluated by the blood pressure pulse acting as the system input and the ICP pulse acting as the output. A comparison is made of the ability of systems analysis, volume-pressure test (VPT), and cerebrospinal fluid-pulse amplitude analysis (CSFPAA) to distinguish between an epidural balloon inflation (EBI) and an intraventricular infusion (IVI) at various steady state levels of ICP. The VPT could not distinguish between EBI and IVI at any level of ICP, and above 30 mm Hg the volume-pressure response decreased. Spectral analysis was able to distinguish EBI from IVI above 30 mm Hg, and CSFPAA was demonstrated to be a simplified spectral analysis. Changes in ICP waveform generated during each cardiac cycle appear to be related to changes in vasomotor reactivity and may have value in the clinical monitoring of ICP.

Arginine vasopressin metabolism in dogs. II. Modeling and system analysis.

System modeling and analysis methods were applied to interpret data regarding arginine vasopressin (AVP) metabolism in dogs. Based on this analysis a new nonlinear 3-pool model of AVP distribution and disposal was proposed and quantified; the model pools included the plasma, a receptor pool, and an extravascular nonreceptor pool. The receptor pool mediated a portion of the rapid flux of hormone between the plasma and the extravascular pool. Mathematical analysis indicated that the plasma AVP impulse response (bolus) data would be insufficient to uniquely estimate all the model constants, but additional plasma impulse data using 125I-labeled AVP, which does not bind to physiologic hormone receptors, would allow unique model quantification. Other required measurements were the urinary excretion of intact hormone, and plasma AVP degradation. The model was successfully fitted to the data from 10 dogs. The results suggest that, in the normal dogs studied, plasma contained 25% of the total AVP, 19% was bound to receptors, and the remaining 56% was in the extravascular pool. Eighty percent of the flux of AVP from the vascular compartment was mediated by the receptor pool; 98% of AVP degradation occurred in the extravascular pool; and urine excretion and plasma degradation made up the remainder.

A new system analysis of motor and locomotor activities associated with a microdialysis study of pressure-induced dopamine increase in rats.

In this work, we report a new analysis system to quantify the behavioral disorders observed in the model of the rat submitted to high pressure and monitored by piezoelectric sensor. The major advance consists in a spectral 3D representation of LMA and tremor, which provides a better selectivity than previous systems. This behavioral processing indicated that tremor is characterized by a 7-14 Hz frequency band and LMA by a 20-35 Hz frequency band. The association of this system to the microdialysis technique to simultaneously evaluate the striatal DA level confirms that pressure-induced striatal DA increase is in great part linked to the LMA, and supports the concept of a complex ethiology for this symptom. We conclude that this new behavioral system analysis associated with microdialysis study constitutes a powerful tool to investigate the role of different neurotransmitters in the occurrence of the behavioral components described in the HPNS of rats.

Normalising impacts in an environmental systems analysis of wastewater systems.

In an environmental systems analysis of four wasterwater systems, the environmental aspects were prioritised by normalisation of predicted impacts from the studied systems to the total impacts from society. Priority Group 1 (highest priority) consisted of discharges (flows) of nitrogen, cadmium, lead and mercury to water, recycling of nitrogen and phosphorus to arable land and flows of heavy metals to arable land. A conventional wastewater system (A) was compared to irrigation of energy forest with biologically treated wastewater (B), liquid composting of toilet wastewater (C) and a conventional system supplemented with urine separation (D). Analysing the aspects in priority group one, systems B-D improved the management of plant nutrients and decreased the flow of heavy metals to water, while the flow to arable land increased, especially for system B. The suggested method is useful in municipal environmental planning and when choosing a wastewater system.

Interpretation of divergent oculomotor imbalance through control system analysis.

Control systems analysis is proposed as a method for understanding the pathophysiology of oculomotor imbalance, thereby permitting better differential diagnosis and more specific treatment. Clinical cases of nonstrabismic divergent deviations, with an emphasis on convergence insufficiency (CI) are presented and explained using conventional optometric diagnostic concepts and fixation disparity (FD) curves with control systems analysis as the central, unifying concept.

A computerized image analysis system for quantitative analysis of cells in histological brain sections.

We propose a reliable method for automatic counting of cells in brain sections labeled with different antibodies (against NeuN, parvalbumin, GABA and c-Fos) and in Nissl-staining. Images of stained sections are converted to binary images by thresholding. Clusters of 'ON pixels' (value of 1) corresponding to cell bodies are selected based on size. The parameters of the algorithm (intensity range and cluster-size) are adjusted for different methods of staining according to expert knowledge. The automatic cell counting method (ACCM) provides correct counting results, as demonstrated by a comparison of computational results with counts gained by human experimenters and with a commercially available image analysis system. On the basis of ACCM counts, small and perhaps physiologically relevant differences in the number of labeled cells can be revealed, as demonstrated here for the GABAergic system following electrical stimulation.

Medical Communication Behavior System. An interactional analysis system for medical interactions.

The study assessed the psychometric properties of the Medical Communication Behavior System. This observation system records time spent by the physicians and patients on specific behaviors in the categories of informational, relational, and negative situation behaviors by using hand-held electronic devices. The study included observations of 101 genetic counseling sessions and also assessed the outcome measures of patient knowledge and satisfaction. In addition, 41 of the sessions were rated using the Roter Interactional Analysis System, and 20 additional control subjects completed the post-counseling information without being observed to examine the effects of recording the session. Results showed good interobserver reliability, and evidence of concurrent, construct, and predictive validity. No differences were found between the observed and unobserved groups of any of the outcome measures.

The motion analysis system for dynamic gait analysis.

The research value of the Motion Analysis System, within its scope of limitations, is large and may allow the podiatrist to lend scientific data to support many areas that have been previously held to be of a theoretic or anecdotal basis. The whole concept of rearfoot control with orthoses is perhaps the most important of these. Podiatrists have been aware of the role of orthoses for years, but now there is the means to demonstrate these findings to other medical professions. The ability to compare a number of clinical variables is another truly useful feature. The Motion Analysis System and other similar equipment are not designed to replace or make obsolete the biomechanical examination and "trained human eye;" rather, they act to compliment these skills and to help bring the realm of podiatric biomechanics and orthotic therapy into the scientific arena.

Objective method of comparing DNA microarray image analysis systems.

Many image analysis systems are available for processing the images produced by laser scanning of DNA microarrays. The image processing system takes pixel-level intensity data and converts it to a set of gene-level expression or copy number summaries that will be used in further analyses. Image analysis systems currently in use differ with regard to the specific algorithms they implement, ease of use, and cost. Thus, it would be desirable to have an objective means of comparing systems. Here we describe a systematic method of comparing image processing results produced by different image analysis systems using a series of replicate microarray experiments. We demonstrate the method with a comparison of cDNA microarray data generated by the UCSF Spot and the GenePix image processing systems.

Scoliosis of the cranial base: radiological and mathematical analysis using finite elements system analysis (FESA) of a case.

INTRODUCTION AND PROPOSED STUDY: Numerous deforming syndromes of the craniofacial complex involve also the symmetry of the cranial base. This study considers a particular alteration, that of 'scoliosis', in which the line Nasion-Sella-Basion-Inion is not rectilinear but curved, due to a torsion of the cranial base in the horizontal plane. MATERIALS: Plagiocephaly was studied in one patient, which was probably caused by altered timing or mechanism of closure of the cranial sutures. METHODS: This study was carried out using CT images of the patient's craniofacial complex, using standard neuroradiological points. In order to study the forces that operated in the various complexes, a mathematical analysis was applied namely, a finite element system analysis (FESA). RESULTS: From this mathematical study, which has enabled us to evaluate the forces that operate in determining the deformity, it has been possible to locate two sites of force concentration. They were located at different levels and on opposite sides. CONCLUSION: This finding could explain why the middle and lower thirds of the face were also involved.

[A finite element analysis system for two-demension morphological analysis of the soft tissue in orthognathic surgery].

OBJECTIVE: To evaluate a finite element method (FEM) for analysis of the cranial-facial morphology. METHODS: The two-dimensional finite element analysis system was established and used to analysis the lateral side morphology of the soft tissue by the change of each finite unit of the soft tissue in a X-ray cranial-facial lateral cepholometrics film. RESULTS: The finite element analysis system was showing very well in the figures and data made by the system. CONCLUSION: Finite element analysis system may be a good supplement of the traditional X-ray cephalometrics to the soft tissue of orthognatics.

A system approach to pharmacodynamics. Input-effect control system analysis of central nervous system effect of alfentanil.

Virtually all biological variables, including those affected by drugs, are subject to adaptive self regulation. In the description of the pharmacodynamics (PD) of drugs, it may be necessary to consider the endogenous control system (ECS) as an integral part of the PD. A PDECS model based on system analysis principles is presented and tested on PD data for alfentanil considering the central nervous system activity quantified by a power spectrum analysis of the electroencephalogram. The model was tested in terms of a proposed relative prediction performance criterion that measures the accuracy of future predictions relative to how well the model describes (fits) the past effect data. A mean value of 80% (standard deviation, 28) for relative prediction performance indicates that the model performs well when challenged by the complex multiple infusion scheme used in the test. The overshoot phenomenon observed in the data is considered by the PDECS model as a ECS-based tolerance phenomenon. The proposed development of tolerance is modeled as a variable gain in the ECS processing that influences the effect. Although the development and loss of tolerance is determined by a single rate constant in the tolerance model, the rates of increase and decrease of tolerance may be substantially different. Contrary to other PD tolerance models, the proposed PDECS approach models the tolerance in terms of an effect deviation from an ECS set point. The intrinsic (no tolerance) effect of the drug is isolated in terms of an open loop (no feedback) effect.

A Medical Text Analysis System for German--syntax analysis.

Much information about patients is stored in free text. Hence, the computerized processing of medical language data has been a well-known goal of medical informatics resulting in different paradigms. In Göttingen, a Medical Text Analysis System for German (abbr. MediTAS) has been under development for some time, trying to combine and to extend these paradigms. This article concentrates on the automated syntax analysis of German medical utterances. The investigated text material consists of 8,790 distinct utterances extracted from the summary sections of about 18,400 cytopathological findings reports. The parsing is based upon a new approach called Left-Associative Grammar (LAG) developed by Hausser. By extending considerably the LAG approach, most of the grammatical constructions occurring in the text material could be covered.

Development of the large scale digital cell analysis system.

Quantitative analysis of large numbers of cells exposed to various stresses would be facilitated by an automated microscope system designed to monitor up to 1000 microscope fields over time intervals of up to one month. The Large Scale Digital Cell Analysis System (LSDCAS) was designed by the authors to provide such analysis capabilities. This report presents a description of the development of the LSDCAS and its application to the study of cells undergoing radiation-induced mitotic catastrophe. A detailed description of the data acquisition algorithms used and the hardware configuration underlying the system is presented. Quantitative analysis of colony formation of cells exposed to 5 Gy X irradiation is presented. Upon analysis, it was found that the plating efficiency in irradiated populations agreed with that obtained using conventional colony-formation assays, indicating the accuracy of LSDCAS in the determination of cell clonogenicity. An unexpected finding was that fully 50% of the surviving colonies exhibited mitotic catastrophe that persisted throughout colony formation. Ongoing extension of the capabilities of the LSDCAS using image segmentation techniques applied to a variety of cellular endpoints is then briefly described.
