Digital subtraction phlebography (DSP) is suitable for x-ray examinations of the shoulder veins and the veins of the upper mediastinum. Acute thromboses, post-thrombotic syndromes, malformations and collateral systems can be demonstrated. The main advantages of DSP over conventional phlebography are the possibility of using contrast medium solutions of low osmolarity enabling pain-free examinations not involving the risk of thromboses, the saving in the amount of contrast medium, and the reduction of examination time.
The lacrimal drainage systems of 92 patients were examined by a digital subtraction technique; 30 patients were examined additionally by a conventional technique, using films obtained during screening for the contrast injection. The digital subtraction dacryocystogram provided excellent demonstration of the lacrymal sac and of the tear ducts, but there is little diagnostic gain when compared with the conventional technique. Both methods are highly reliable. In view of its low radiation dose and rapid and simple course, conventional dacryocystography is regarded as the method of choice for routine examinations. The digital subtraction technique should be reserved for special problems.
Preliminary examinations with digital subtraction myelography revealed the possibility of saving up to 70% of water soluble contrast medium compared with usual myelographic procedures, thereby reducing the risk of neurotoxic adverse reactions.
Experience with 74 patients has shown that digital subtraction angiography is a suitable method for the demonstration of the vena cava. In 28 patients the examination was repeated by the conventional large film angiography procedure, but this added no significant information to help to solve the clinical problem. Electronic contrast enhancement permits lower concentrations of contrast to be used than for conventional techniques. Combined with a more selective use of films, this leads to a reduction in the cost of the examination.

In patients with symptomatic lumbar disc disease, lumbar myelography was performed in combination with the digital subtraction technique. Contrast medium was injected during the digital subtraction procedure, and the following part of the examination was treated conventionally. In all patients the subtracted images were superior for documentation purposes and also for evaluation of the upper lumbar spine. If the contrast medium was injected epidurally, the artifact could be suppressed by the selection of a suitable mask.
