[Hansen's disease-associated disabilities in sanatoria residents. A study of the actual situation in three sanatoria located in the Setouchi area].

PURPOSE: Clarification of details of Hansen's disease in Japan is important for obtaining an adequate understanding of the present circumstances regarding control and for formulation of specific suggestions for future measures. The average age of residents in national Hansen's disease sanatoria reached 74.9 in May 2002 and most of these suffer from serious disabilities caused by the disease. In order to implement measures facilitating their return to society or secure living in sanatoria, a proper understanding of the residents' physical situation is essential. Therefore, the present study was conducted of residents suffering from Hansen's disease in Japan. METHOD: A direct interview was conducted with all 1282 residents of three sanatoria, located in the Setouchi area: Nagashima-aiseien, Oku-komyoen, and Oshima-seishoen. An investigator visited the rooms of the residents, interviewed them using a questionnaire, which consisted of about 100 items including the OPCS scale, and filled out an answer form. The reply rate was 64%. RESULTS: The average age of the residents was found to be 72.8 years, their average age when they entered the sanatoria was 26.0 years, and the average period of residence was 52.4 years. The damage they suffer as a result of the isolation policy is physical, mental, economical and social. That is to say, their entire life is harmed as a direct result of the Leprosy Prevention Law or the Eugenic Protection Law with discrimination in society and negative effects on their families. Their physical condition features serious disabilities in the use of fingers, as well as in walking and seeing caused by peripheral nerve damage. Such disabilities are related to their age and period of residence, and in addition, these in use of their fingers are related to the work which residents were forced to do in sanatoria. The results indicate that patients' physical condition is related to their age, how long they have had the disease and their work in sanatoria. CONCLUSION: It is clear that previous Hansen's disease control in Japan has caused a great deal of harm, from which it is very difficult to recover so that residents have to live with serious problems. In order to implement measures allowing their return to society or more secure life at sanatoria, sufficient care is needed to treat progressive disabilities caused by ageing and prevent deterioration caused by peripheral nerve damage.

[Outbreak of foodborne and waterborne infections and intoxications in hospitals and sanatoria in Poland in 1985-1999].

Fraction of the outbreaks of foodborne intoxications and infections in hospitals and sanatoria is estimated at 1.5% to 6.3% of the total number of outbreaks annually in 1985-1999. Number of people affected in hospitals/sanatoria constituted 2.7%-9.2% of the total number of cases in outbreaks. In the paper analyzes of number of cases in hospitals and sanatoria is shown, according to the food--vehicle of the foodborne intoxications and infections and according to the etiological agent. Eleven great epidemics (above 100 cases) in hospitals and seven in sanatoria were described in 1985-1999.

[Community health and national sanatoria for Hansen's disease in Japan--survey of receipts of National health insurance].

National sanatoria for Hansen's disease in Japan were special institutions that only treated in-patients. But a few sanatoria for Hansen's disease located in islands and rural districts should have other duty, namely service to community health because there was no other clinic office. New patients with Hansen's disease are very small in number and inpatients become older and older. Then the new role should be seeked to National sanatoria for Hansen's disease in Japan. Amami-wakoen, National sanatorium for Hansen's disease in Amami island started skin clinic for out-patients with skin diseases in March, 1983 because there were few skin clinics in Amami district. The number of patients with skin diseases increases year by year (from 12.3 persons every day in 1983 to 36.4 in 1989). We surveyed receipts of National health insurance for the purpose of studying the main diseases in Amami main island and the behavior of the patients with skin diseases. The results of survey are followings; 1. The percentage of patients with skin diseases in Amami main island in August, 1987 was significantly superior. 2. The percentage of patients with mental diseases and pregnant woman in hospital and patients with gastrointestinal disorders and pregnant women in clinic were significantly large in May, 1987 in Amami main island. 3. There was a tendency that the patients with skin diseases visited to the clinic near in place. It was found that we treated only 5.1 percent persons of patients with skin diseases in August, 1987.

[Treatment effectiveness of patients with ocular tuberculosis at specialized sanatoria].

The outcomes of treatment of 2908 patients with ocular tuberculosis who were treated at the Vyborg-3, Krasnyi Val, and Plyos sanatoria in 1993 - 1995 are analyzed. These sanatoria used the common guide in evaluating the efficacy of treatment, namely a separate assessment of the outcomes of treatment of the underlying and concurrent diseases. There were no statistical significant differences in therapeutical efficiency at these sanatoria. Therapeutical benefits were obtained in 99.8% of patients in the active phase of the disease.

[Nosocomial infection of scabies in national hospitals and sanatoria in Japan].

The aim of this study was to investigate cases of nosocomial infection of scabies in the national hospitals in Japan. Questionnaires sent to 183 hospitals and sanatoria were returned by 93. Fifty-three cases in 24 institutions of nosocomial infection were reported in the past 5 years. No nosocomial infection was reported from 7 sanatoria for Hansen's diseases where the patients were relatively older and had higher ADL scores. It took 7.5 weeks on the average to eliminate nosocomial infection and more than 8 weeks were required to control them in the hospitals where more than 10 cases had occurred. Benzyl benzoate, gamma benzene hexachloride, and Pyrethroids, which were not approved as drugs for the treatment of scabies in Japan, were used in the all institutions where nosocomial infection occurred except for one institution. Problems to be solved were as follows; (1) delay of diagnosis and treatment, (2) lack of nursing staff, (3) difficulties of complete isolation due to lack of spaces, inability of patients to understand the need for isolation and also the psychological instability of the isolated patients, (4) recurrence due to the use of ineffective drugs, (5) insufficient information about the prevalence of scabies in the previous institutions, (6) misdiagnosis of non-scabies patients with itchy skin rash as the scabies, (7) inconsisitent care due to poor evaluation of skin lesions.

[Improving the activities of tuberculosis sanatoria for children and adolescents].

The paper deals with the examination of the state of health of children treated at tuberculosis sanatoria of republican and local importance. At present the tuberculosis sanatoria for children are not considered any more to be strickly uniprofile. The emphasis of their activities is aimed at conducting controlled chemotherapeutic prevention measures among children with small attenuating forms of tuberculosis as well as among those who belong to high risk groups in relation to tuberculosis including concurrent bronchopulmonary diseases of nonspecific etiology.

Pyroglyphid mites in the High Tatras sanatoria.

1. In our work we studied the problematics of house dust fauna in the sanatoria and institutes of the Tatra region, regarding especially the incidence of mites of the family Pyroglyphidae that are considered for the major source of house dust allergen. 2. In eleven places of different altitude in this region, there were in 1982 chosen 13 localities--sanatoria and institutes. Here, once quarterly, were collected house dust samples from beds and floors. 3. In the Locality 1 Helios, Strbské Pleso, the house dust samples were collected in 1976, once in two months, in 1979 once in three months, and in 1982 once quarterly. Here, the involvement of the new--opened sanatorium by house dust mites was studied. 4. House dust samples in the studied localities were collected using the electric vacuum cleaner into the changeable paper bags. The samples were examined by flotation due to Spieksma (1970) and due to the modification of Spieksma's methodics elaborated by Samsinák et al. (1974). 5. The found house dust mites--pyroglyphids were evaluated due to Bronswijk and Sinha (1971) and due to the modification of their methodics elaborated by Samsinák et al. (1974). The classification due to Wharton (1976) was used as well. 6. In every studied locality, in house dust collected from floors and beds, there were identified mites of the family Pyroglyphidae, considered for the major source of house dust allergen. 7. Mites of the family Pyroglyphidae were represented by the genera Dermatophagoides and Euroglyphus, and by the species Dermatophagoides pteronyssinus, Dermatophagoides farinae, and Euroglyphus maynei. There were found as well the nymphal instars of the genus Dermatophagoides, in this development degree it is very complicated to determine precisely the species--and that is why the term Dermatophagoides sp. nymphal instar is used. 8. In 1982 there were in the chosen localities collected 764 house dust samples from beds and 20 samples from floors, 186 of them (23.7%) were negative--without any mites. In the positive samples there were identified 1794 arthropods, 1776 were mites and 18 were Psocopterae. Of the total mite number 1263 (71.1%) were mites of the family Pyroglyphidae. 9. The other mite families were represented by 513 pieces, i.e. 28.6% of the total arthropod number. 10. In the studied localities there were identified three species of the family Pyroglyphidae and 15 species of other families, the representatives of the families Ereinaetidae, Tetranychidae (not determined preciselier), and Oribatei.(ABSTRACT TRUNCATED AT 400 WORDS)

Investigations on the fauna of beds in flats, children's sanatoria and old-age homes.

An analysis was carried of the fauna found in 338 beds from two old-age homes and from flats inhabited by asthmatic children. During hospitalization of these children the fauna in the flats was compared with that found in beds used by them in sanatoria. The most abundant species proved to be Dermatophagoides pteronyssinus that is also widely spread in other countries of Europe. The bed fauna in the sanatoria is very poor, corresponding to the improved state of health of patients during hospitalization. The authors assume that the low population density of fauna is influenced by constant airing of rooms and by strict separation of bedrooms from dining halls and other premises. Also the number of mites in different padding material used in mattresses was evaluated and an analysis of the general fauna per bed was carried out.

Comprehensive care and the sanatoria: Cuba's response to HIV/AIDS.

The authors present an overview of Cuba's response to HIV/AIDS including: (a) the development of comprehensive health care in Cuba; (b) the epidemiology of AIDS in Cuba; (c) the sanatoria approach to treatment and recent changes; (d) the idea of comprehensive HIV/AIDS care as a basic human right; (e) the effects of the special period on the HIV/AIDS program and containment of the epidemic; and (f) a critique of Cuba's HIV/AIDS program.

[The role of specialized sanatoria in the prevention of tuberculosis in children].

The paper presents the results of comparison of the rates of tuberculosis morbidity in children as a whole, in the foci of tuberculosis, in follow-up Group VI and the frequency of using sanatory care for preventive purposes in Russia in 1999. A role of available sanatoria in preventing tuberculosis is found to be in sufficient. By using the best areas as an example, the volume of required work with children at risk for tuberculosis and the rational use of sanatory care are substantiated.

[Evaluation of patient diet in sanatoria and health resorts].

By the example of nutrition of patients from sanatorium "Kristall" (the Black Sea coast) an evaluation of patients nutrition in sanatoria and health resorts is made. It is determined, that the standard patients diets (Table N 15, 5, 10) do not differ from the reference ones regarding their calorie content, main food components and energy value. Though insufficient content of vitamins, unsaturated fatty acids and linoleic/linolein acids imbalance is marked.

[Large intestine dysbacteriosis and therapy efficacy in patients with respiratory tract tuberculosis in sanatoria].

Long-term chemotherapy of tuberculosis leads to dysbacteriosis of the large intestine, that significantly decreases tolerance of tuberculosis drugs, provokes persistence of tuberculosis intoxication and retards involution of tuberculosis process in the lungs. Recovery of the bifidoflora and lactoflora due to the use of an original sour-milk drink developed by the authors was stated in 65 and 55% of the patients respectively. Moreover, it promoted higher tolerance of the chemotherapeutics since its composition includes amino acids, vitamins B, C, D and E, enzymes and microelements. It should be indicated that the drink was used simultaneously with the chemotherapy and did not require its discontinuation. Therefore, it is recommended that the scheme of the treatment of patients with pulmonary tuberculosis in sanatoria should include corrigating agents and in particular probiotics containing live bifido- and lactobacteria having no contraindications and side effects, and providing elimination of intestinal dysbacteriosis.
