Acupuncture reflexotherapy in the treatment of sensory urgency that persists after transurethral resection of the prostate: a preliminary report.

AIMS: In this study, we wanted to evaluate whether acupuncture reflexotherapy is able to treat the sensory irritative components of LUTS (lower urinary tract symptoms) that persist after transurethral resection of the prostate. METHODS: We have evaluated 42 patients, randomly selected into three groups: 14 patients received placebo, 15 patients received oxybutynin, and 13 patient were treated with electrostimulation by acupuncture reflexotherapy. RESULTS: Before treatment, the mean maximum flow rate (Qmax) was 21.0 +/- 3.2 mL/sec, the mean International Prostate Symptom Score (I-PSS) score was 12.9 +/- 4.2, the mean I-PSS Quality of Life (IPSS QoL) score was 3.6 +/- 1.2. At the first check-up performed after 3 months, we could observe that the I-PSS and QoL scores were 12.6 +/- 4.3 and 3.8 +/- 1.3 in the group who received placebo; the scores decreased to 11.1 +/- 3.2 and to 3.1 +/- 1.0, respectively, in the 15 patients treated with oxybutynin and decreased to 6.1 +/- 2.6 and 1.3 +/- 1.1, respectively, in the 13 patients who underwent acupuncture reflexotherapy. At 1-year follow-up, these parameters were practically similar. The voiding diaries allowed us to deduce that the average number of daytime voidings decreased by 8% in patients who received oxybutynin and decreased by 20% in 13 patients who underwent reflexotherapy; the average number of nocturnal micturitions decreased by approximately 20% and 60%, respectively, in patients who received oxybutynin and reflexotherapy. CONCLUSIONS: This study has pointed out that acupuncture reflexotherapy has a real benefit in patients with sensory urgency that persists after transurethral resection of the prostate.

[Effect of reflexotherapy on the functional state of the sympathoadrenal system in patients with bronchial asthma].

Activity of sympathoadrenal system in patients with infectious-allergic bronchial asthma treated with reflexotherapy by monitoring alterations in spontaneous and L-DOPA stimulated daily excretion of adrenaline, noradrenaline, dopamine and L-DOPA with urine. Before and after the reflexotherapy course spontaneous excretion of catecholamines and L-DOPA with urine was found to be similar to the physiologically normal level. The test with per oral administration of 250 mg of L-DOPA showed that before the reflexotherapy the course of consumption of L-DOPA was impaired and, hence, the functional state of sympathoadrenal system was altered. The reflexotherapy improved the L-DOPA consumption suggesting normalization of catecholamine metabolism. The data obtained suggest that normalization of sympathoadrenal system activity is one of important effects of the acupuncture therapy.

Acupunctural reflexotherapy as anaesthesia in day-surgery cases. Our experience in left internal vein ligature for symptomatic varicocele and in circumcision.

Acupunctural reflexotherapy has been performed in patients during minimally invasive treatments for benign prostatic hyperplasia and in patients with lower urinary tract symptoms. We report two cases of left internal spermatic vein ligature for varicocele and a case of circumcision using acupunctural reflexotherapy, a procedure, to our knowledge, never applied before in open surgery. A 38 and a 24 years old male patients came to our observation for a 2nd degree symptomatic left varicocele and a 40 years old patient came to have a circumcision; they requested alternative anaesthesia for the operation and acupunctural reflexotherapy has been performed. This technique for analgesia can be useful and can be applied in selected cases, i.e. minor surgery in patients who do not wish or cannot have a general anaesthesia; it can be associated to other techniques of anaesthesia as peri- and post-operative analgesia. No contraindications nor collateral effects exist. The limits of this methods are represented by the length of time required for preparation of the patient and the existence of patients non-responder to electrostimulation.

[Current methods of migraine reflexotherapy].

Results of treating 166 patients from migraine by reflexotherapy methods (acupuncture, electropuncture, electroacupuncture) are presented along with the data of clinico-genealogical, electrophysiological (EEG, REG, OPG, ECG) and biochemical (serotonine, histamine, and dopamine blood levels) examinations performed prior to and after the reflexotherapy course. Recommendations for treating migraine with reference to the form, stage, intensity, and localization of the headache are given. A positive effect of the reflexotherapy was obtained in 95.9% of the patients: this was confirmed by the control clinico-electrophysiological and biochemical findings.

[Changes in peripheral hemodynamics in patients with vertebrogenic pain syndromes during reflexotherapy].

The results of an analysis of circulation changes in the major vessels of the upper and lower extremities of 240 patients with vertebrogenic pain syndromes are reported. These changes were analyzed before and after treatment of the patients by the methods of acupuncture reflexotherapy (120 observations) and electron-ion reflexotherapy (the other 120 observations). Use was made of longitudinal rheovasography with contour analysis of the curves by the method of G. I. Enini (1965). It was found that the reflexotherapy produced a favourable effect on the peripheral circulation in patients with reflectory syndromes. In patients with spinal radicle compression no positive rheovasographic changes were noted.

[Reflexotherapy in the treatment of alcoholics with depressive syndrome].

A total of 33 patients with associated alcoholism and depressive syndrome were examined. Of these, 23 persons were diagnosed to suffer from depressive neurosis, 9 had reactive and 1 endogenous depression. After the reflexotherapy the depressive symptomatology was removed in all the cases excluding endogenous depression. The alcoholism symptomatology disappeared simultaneously. The authors regard reflexotherapy as an adequate modality to be applied to the combined treatment of patients suffering from associated alcoholism and depression.

[Treating type II diabetes mellitus with foot reflexotherapy].

32 cases of type II diabetes mellitus were randomly divided into two groups. One group was treated with conventional Western medicine hypoglycemic agent) plus foot reflexotherapy (FR), the other group with the same medicine only. After 30 days' treatment, fasting blood glucose levels, platelet aggregation, length and wet weight of the thrombus, senility symptom scores and serum lipid peroxide (LPO) were greatly reduced in FR group (P < 0.05-0.01), while no significant change was observed in the WM group. The study suggested that foot reflexotherapy was an effective treatment for type II diabetes mellitus.

[Cerebro-autonomic correlations in patients with early stages of hypertension during reflexotherapy].

Seventy patients with Stages I and II hypertensive disease underwent electrocardiographic studies of the emotional syndrome and central hemodynamics prior to and following reflexotherapy. Characteristics of the psychovegetative syndrome were identified in the patients. A significant antihypertensive effect and normalized psychological and vegetative parameters observed after reflexotherapy suggest that acupuncture is beneficial in the treatment at early stages of essential hypertension.

[Role of reflexotherapy in the complex treatment of neuroendocrine obesity].

The paper is concerned with comparative estimation of the effect of different therapeutic methods (hypocaloric diet, physiotherapeutic, drug and exercise therapy, reflexotherapy) on loss of body mass. The experience of reflexotherapy of 86 patients showed its positive effect on the psychoemotional activities of patients with obesity, a rise of adaptation capabilities of the body under physical exercise, improved external respiration function, an increase in oxygen saturation of tissues, the stimulation of metabolism (by the basal metabolism findings) by way of increasing the secretion of hypophyseal tropic hormones, triiodothyronine and thyroxin, and potentiation of the time course of loss of body mass.

[Proposed method for double-blind study of reflexotherapy phenomena].

The Authors report their experience with the placebo, an essential step in the clinical study of reflexotherapy. They assert that also in this branch of medicine it is possible to carry out a "double blind" clinical experimentation by means of placebo, that is, an essential condition in order to develop a scientific assessment of both the limits and the potentialities of reflexotherapy.

[Reflexotherapy in patients with ischemic angiopathies of the lower extremities].

Comparative assessment of different methods of reflexotherapy - RT (acupuncture, electroacupuncture, pharmacopuncture and the termopuncture) in treatment of ischemic angiopathies of the lower extremities is presented. One hundred and twenty-four patients with ischemic angiopathies were treated, 86 of them had I - II stage of the disease, 38 - III stage. Diagnosis of the disease and assessment of treatment efficacy were based on clinical, laboratory and special instrumental examinations. Functional examinations revealed equal reaction of peripheral blood flow to different RT methods. Moderate hypocoagulation compared with the baseline was demonstrated. Clinical symptoms (improvement of the general condition, decrease of pain syndrome) and improvement of biochemical and physiological parameters were criteria of successful treatment. The results of the study helped to develop methods of treatment with different variants of RT and to recommend them in clinical practice for combined treatment of this disease.

[The Italian Society of Reflexotherapy, Acupuncture and Auricular Therapy (SIRAA) : from the origins to the third millennium].

The diffusion of acupuncture in Europe begun in the 50ies, following 2 main trends: traditional (or energetic) acupuncture and reflexotherapeutic acupuncture. In 1973, in Turin, Italy, the Italian Society of Reflexotherapy, Acupuncture and Auricolotherapy (SIRAA) was founded. This Society over the years tried to integrate the 2 different trends, whose main characteristics are analysed in this paper. The first aim of SIRAA, since its foundation, has been to study acupuncture and auricolotherapy by means of research and by scientific trials, in order to promote the understanding of its mechanisms of action and to clearly show all its therapeutic applications. The development of acupuncture in Italy, the activities of SIRAA from the origins up to now and the future aims of this Association are described. Among the aims, particularly important is to propose scientific research models compatible with evidence based medicine, but also able to warrant the adequacy of the acupunctural method. In conclusion, it is pointed out the necessity to address the future research on acupuncture not only towards the study of the mechanisms of action and the evaluation of therapeutic results, but also towards some new branches now considered with increasing attention, such as preventive medicine, rehabilitation and palliative cares.
