The impact of work-related trauma on the well-being of perioperative nurses.

Perioperative nurses are confronted daily with the reality of trauma, of experiencing abuse and conflict in the workplace, dealing with practice issues such as gruesome motor vehicle accidents, as well as the heartbreaking tragedy of death (Michael 1999, Michael & Jenkins 2000). This study explored the reported impact of traumatic events upon the well-being of perioperative nurses, using both qualitative and quantitative methods of inquiry. Of the 233 respondents in this study, 161 (69%) reported being exposed to traumatic events. The types of described reactions following trauma were predominantly negative and included negative feelings, negative team member relationship issues, negative behaviour from others, physical concerns and negative working conditions. Intrusion and avoidance behaviour was reported to a significant degree. Performance distress, somatic distress, general feelings of distress and self-related health were significantly poorer in perioperative nurses who had experienced trauma, than their colleagues without such experiences. Conversely, some respondents were able to construe positive outcomes from trauma and grow from the experience. Positive outcomes included gains from the experience, team building, positive feelings and action taken. The current study contributes to understanding of the role of environmental factors, associated work-related trauma and recovery following such events. The outcomes have implications for effective trauma stress management interventions and continuing education of nurses.

International role of the perioperative nurse.

Perioperative nursing extends outside of our operating room environments, not only within our hospitals, but also internationally, across borders. Those perioperative nurses who are interested in international care can easily become involved with a project within their own city and state. The challenge is the commitment and the effort.

Clinical forensics for perioperative nurses.

Perioperative nurses frequently care for victims and suspected perpetrators of violent crimes. Nurses often are the first health care providers to assess the trauma patient and collect crucial evidence for future legal action. Informational evidence includes observations about patients' behavior and appearance and documentation of their comments. Nurses also must protect the chain of evidence so that evidence can be admissible in court. To function in this role, perioperative nurses must understand the concepts of clinical forensics, which is the application of the principles and practices of forensic science to questions of law in the investigation of violent crime.

What's your specialty, Doc? Perioperative nursing.

Perioperative nurses who are contemplating the pursuit of a doctoral degree in nursing need information about the current status and characteristics of doctoral education to make informed decisions. This article addresses questions both philosophic and pragmatic that should be addressed by prospective students. Information sources are identified for assistance with making the selection from the 75 plus programs that offer a nursing doctorate nationwide. Suggested sources include professional organizations as well as current students, faculty, and alumni. Determinations should be based on academic information, such as similarities and differences in degrees awarded; the program of studies leading to the degree; admission and progression standards; research interest of the student relative to identified focus and expertise of program faculty, dissertation process, and university and nursing education resources and services, including financial and technological support. Attainment of the doctorate is presented as an enriching experience that prepares nurses for leadership in both education and health care as clinicians, educators, and administrators. Particular attention should be paid to selecting programs that compare favorably with the published American Association of Colleges of Nursing quality indicators for doctoral programs. There must be strong support in the program from both faculty mentors and peer colleagues, reflective of a community of scholars, pursuing not only individual but also collective wisdom, whereas engaging an individual's mind and soul in the pursuit of new knowledge and skills in an atmosphere of mutual trust and respect.

Applying nursing theory to perioperative nursing practice.

The perioperative nursing role has evolved from that of task-oriented specialists to patient-centered professionals. The concept of caring is significant to perioperative nurses and is manifested by the many caring behaviors perioperative nurses demonstrate toward surgical patients. This article describes how the element of caring is an essential function of perioperative nursing and relates the perioperative nursing role to the work of three nursing theorists (le, Florence Nightingale; Virginia Henderson, RN, AM; Carol L. Montgomery, RN, PhD).

Ethical issues in pediatric perioperative nursing.

Pediatric perioperative nurses often face ethical issues in their daily practice. Some of these issues require the ability to make a quick decision. Resources and mechanisms nurses can use to gain the knowledge required for ethical decision making are to attend ethical, legal, and clinical conferences, read ethical articles in nursing, medical, legal and ethical journals, and discuss issues with colleagues. Consultation can be obtained through ethics advisory committees and ethicists or people knowledgeable about ethics. Nurses must be able to identify ethical issues and how ethical decisions are made in order to analyze arguments critically, reflect on decisions, and examine positions. Nurses must have the ability to acknowledge and identify a conflict that exists between personal and professional values and to attempt to resolve the conflict. Perioperative nurses need to accept responsibility for their actions and take reactions based on ethical reasoning when providing patient care. By being knowledgeable of ethical issues and how to address them, pediatric perioperative nurses are better prepared to provide comprehensive nursing care to all patients and families.

Perioperative nurse practitioners.

Perioperative nursing roles are evolving in response to health care reform, technological developments, and changing opportunities for advanced practice nursing. One response to these changes is the development of the perioperative nurse practitioner role. The perioperative base for this new advanced practice role is described in this article. The advanced practice role is effective in other specialty settings and can be developed readily in a variety of surgical settings. This role requires a master's degree in nursing to provide the academic and practice preparation for clinical decision making and patient management in the perioperative setting. Future opportunities for perioperative advanced practice nursing are discussed.

The value of surgical risk models to the perioperative nurse.

The perioperative nurse has long been involved in wound classification, a variable historically used for analysis of surgical site infections. Now, with the development of more sophisticated surgical risk models, surveillance programs may calculate risk-specific surgical site infection rates. The understanding of these models is pertinent to the practice of the perioperative nurse.

Transient osteoporosis during pregnancy: the perioperative nurse's role.

Perioperative nurses are confronted with ever changing demands in their daily practice. Perhaps the greatest challenge, however, is in the care of patients with multi-faceted health problems. These patients require the coordination of many multi-disciplinary team members in attempting to achieve optimum health. This paper will describe the case study of a 28 year old, 26 week primip, who presented to a tertiary care centre with spontaneous bilateral hip fractures of unknown origin. It will further discuss the disease process known as Transient Osteoporosis during Pregnancy, the surgical approach, and finally, the role of the perioperative nurse in coordinating the patient's perioperative care.

Perioperative nursing in Israel.

Perioperative nursing in Israel's Hadassah Medical Organization hospitals-Ein Kerem and Mt Scopus-reflects quality patient care practices paralleling those in US hospitals. Although these two hospitals, both located in Jerusalem, represent only 5% of the hospital beds in Israel, they receive more than 60% of referrals made to all Israel hospitals. Throughout Israel's tumultuous history, the Hadassah Medical Organization has served the country during both war and peace. In 1991, Mt Scopus was designated as the hospital on duty in case of a chemical attack during the Gulf War. The authors provide an overview of perioperative nursing at the Eln Kerem and Mt Scopus Medical Centers of the Hadassah Medical Organization and describe one American nurse's experience as a volunteer in the Mt Scopus Medical Center OR during the Gulf War.
