The impact of early palatal obturation on consonant development in babies with unrepaired cleft palate.

OBJECTIVE: The purpose of this investigation was to determine whether palatal obturators enhance consonant development during babbling for babies with unrepaired cleft palate. PARTICIPANTS: Fourteen babies with cleft palate who had worn anterior palatal obturators prior to palatal surgery were matched to 14 unobturated babies according to cleft type, sex, and age at time of presurgical evaluation. MAIN OUTCOME MEASURES: Spontaneous vocalizations of the obturated and unobturated groups were compared to determine whether differences were evident in size of consonant inventory as well as place and manner of consonant production. RESULTS: Paired t tests revealed no significant differences between the groups in size of consonant inventory or place and manner of consonant production. There was a trend for babies in the obturated group to produce more glottal consonants. CONCLUSIONS: In general, the findings of this study suggested that palatal obturators do not appear to facilitate production of anterior palatal consonants during babbling.

Infant palatal obturator.

The occurrence of a congenital cleft palate greatly impedes the habituation of efficient deglutition of an infant, causing aspiration of fluids in the air passage which may lead to complications like bronchopneumonia and infection of the airway and lungs. Inadequate nourishment due to difficulty in feeding affects the health and acts as a stumbling block in the milestones of normal development. Severe nutritional deficiencies further complicate surgical closure at the right time. A palatal obturator given to the infant effectively separates the oral cavity from the nasal cavity and is of great help in feeding till the defect can be surgically repaired.

Indirect retainers in soft palate obturator design.

Definitive framework design for the maxillofacial patient with complete maxillary dentition who has had a soft palate resection requires the use of direct and indirect retainers. It is possible to retain these prostheses, however, with only two posterior clasps and indirect retainers without clasps. This design offers an esthetic, retentive, and functional restoration while preserving existing structures. Four clinical applications are described.

A technique for obturating palatal fistulas.

Use of nontoxic self-adhesive material is described to obturate residual hard palate fistulas. This material may be used during perceptual and aerodynamic speech assessments to evaluate speech distortion and the relative contributions of velopharyngeal port and fistula to nasal air leakage during speech. It may also be used as an interim obturator prior to surgical repair of a fistula.

The role of pressure flow and endoscopic assessment in successful palatal obturator revision.

The purpose of this paper is to illustrate the value of combined aerodynamic and endoscopic examination of velopharyngeal function in the revision of prosthetic speech appliances. Use of these combined measures enables the clinician to identify accurately the site(s) of any under- or overobturation. Furthermore, any needed revision is completed accurately and efficiently.

[The maxillofacial prosthesis: palatal obturators. The principles of treatment and rehabilitation in a case of buccorhinosinusal communication].

Having analysed the causes which lead to the loss of substance from the palatal vault, the different types of communication which then result, their evolution and the consequent functional defects are identified. By describing a complex clinical case, and having taken into consideration both aims and materials, the techniques of prosthetic rehabilitation are illustrated using obturator devices. The results of the various phases are examined.
