[Evolution of the Mycobacteria Laboratory of the Pasteur Institute of Madagascar from 1991 to 1994].

In 1991, the Laboratory of Mycobacteria was a small laboratory, part of the Clinical Biology Centre (CBC) of the Institut Pasteur de Madagascar: 656 pathological samples have been analysed for the account of the CBC and the National Control Programme activities. Within 4 years, the number of samples tested increased by more than threefold and the technical ability has evolved in an important way, specially for the identification and the antibiotic sensitivity testing. The scientific equipment have been modernized and the rooms surface increased by fourfold. In 1995, this laboratory was officially designated as the National Reference Laboratory for the culture, the identification and antibiogramme for the account of the National Control programme and for the private clinicians. It also participates to the tuberculosis research programmes of Institut Pasteur de Madagascar. It is associated to the Laboratory of Mycobacteria in the Institut d'Hygiène Sociale of Antananarivo which is the National Reference Laboratory for the bacilloscopy, the teaching and the supervision of the peripheral laboratories.

[Installation of a local information network: case of Pasteur Institute of Madagascar].

The development of computer networks, especially Internet, leads to a new way of working organization. Internet mail represents a real progress in communication management for institutions far away from northern collaborating centres such as Institut Pasteur de Madagascar. The resources sharing capacities (hard disks, printers, back-up systems) induce important economies. Thus, the installation of an Ethernet network within the establishment is completely justified.

[Cervical cancers diagnosed at the Pasteur Institute of Madagascar from 1992 to 2002].

In Madagascar, the epidemiological data actualized concerning the cancer of the collus of uterus are not available because of the absence of register of cancer. The objective of this study is to achieve a first assessment of the problem, to complete the epidemiological knowledge, to point out the tool of precoce detection of the precancerous lesions, to propose the measures aiming to improve the management of the patients and to contribute to the institution of a register of cancer. This is a retrospective survey on the frequency of the cancer of the cervix observed from 1992 to 2002 about 23,908 withdrawals addressed to the Institut Pasteur de Madagascar for anatomopathological exam and 12,605 cervical smears for cytological exam. In pathological anatomy, 2,621 (63.4%) of 4,136 cases of diagnosed cancer, have been observed in women. 687 cases (26.2%) of them were localized in the collus. The 3/4 of the cancers of the cervix is invasive and the mean age is 48.2 years old at the time of diagnosis. The cytology detects only 74 cases of invasive cancer of which most don't have an histological confirmation. 274 pre-lesions of cervix cancer were diagnosed for this period, the majority lesions are cytological diagnosis. In spite of a non representative recruitment of the general population, and by the number of withdrawals considered, these results may represent indicators of the epidemiological situation and justify the institution of program to detect the precancerous lesions in a national scale.

[Toxoplasmosis at the Pasteur Institute of Bangui, Central African Republic (1996-1998): serological data].

A serological study of toxoplasmosis was conducted between 1996 and 1998 on 1953 patients of the Medical Analysis Laboratory of the Institut Pasteur de Bangui. The mean age of patients was 28 years. Among sera tested by ELISA, seropositivity to IgG antibodies was observed in 50.6%, and 2.6% sera were found positive for IgM antitoxoplasma antibodies (immuno-capture). The seroprevalence did not vary significantly according to sex or age. The results showed 40.8% sera had IgG antibodies titered 400 Ul/ml and more. The proportion of high level (> 400 Ul) IgG was more important in males than in females. High level IgG antibodies were statistically significant more frequently in the sera of females aged 10-29 years. Of the procreative women, 49.1% were at risk of contacting toxoplasmosis. The diagnosis of recently acquired infection, based on the coexistence of IgM antibodies and high level IgG antibodies, was noted in 1.6% among sera of procreative women. In the Central African Republic, serologic survey during pregnancy is not systematic and HIV seroprevalence is high (15%). Risks of acute infections during pregnancy and of opportunistic infections in HIV-infection patients are high. A control of toxoplasmosis is justifiable (screening during pregnancy, sanitary education) in CAR.

[Seroprevalence of rubella in pregnant women at Antananarivo. Study of 853 sera at the Pasteur Institute in Madagascar].

The authors reported the results of a seroepidemiological study of rubella in pregnancy in Antananarivo at the Institut Pasteur de Madagascar. A total of 853 sera were tested using hemagglutination inhibition as reference method. Rubella antibodies were present in 80.5% of the malagasy pregnant women. These results seem quite similar to those observed from other tropical african countries. The rubella prevalences among the pregnant woman's population coming from France, India and Pakistan and living in Antananarivo (respectively, 89.8% and 81.8%) seem the same than in their home countries.

[Management of breast cancers diagnosed at the Pasteur Institute of Madagascar from 1995 to 2001].

Breast cancer is a great problem of public health all over the world. In developed countries, breast cancer represents the most common cancer in females. Its incidence is also increasing in developing country. In Madagascar, no data is available to estimate the real incidence and prevalence rates of breast cancer. However, the data at the Institut Pasteur de Madagascar can confirm the extent of the problem even if it is not at a national scale. The authors report the results of a retrospective study from histological examination at the Laboratory of pathological anatomy of the IPM, during 7 years. Among 2,337 cases of cancer, 16% (373) were breast cancer. Most of them were a female breast cancer (356 cases). The average age is 48 years old. 30% of the tumors were more than 2 cm in size, corresponding at least to the T2 stade from the International Union Against Cancer anatomoclinical classification. The current histological type is the infiltrating ductal carcinoma (80%), about 2/3 belong to the grade 3 of the Scarff-Bloom-Richardson histopronostical classification. Early diagnosis of the cancer is difficult because of the insufficiency of the sanitary infrastructure, particularly for cervical and breast cancers. A national policy for screening must be set up in order to decrease the rate of these invasive carcinomas. In the meantime, informing women and training all the medical staff is a priority. Recording all the data in Madagascar would be desirable.

[Envenomations of the Kindia prefecture treated in the Institut Pasteur of Guinea for one year (2003-2004)].

After a broadcast information on the availability of antivenom serum in Guinea Pasteur Institute (GPI), 76 snakebites victims have been admitted to the health centre of GPI within 11 months (October 2003-September 2004), 48 of whom were in a serious condition. In addition, 13 deaths occurred during the transport of the victims, i.e. a total incidence of 89 cases. Among these cases, 84% of the bites occurred during farming works (clearing, weeding, harvesting). Among the 76 victims who came alive at GPI, 9 of them did not present any envenomation. The 67 other victims have systematically received antivenom serum with a 100% recovery rate.

[Evaluation of the work conducted by the Madagascar Pasteur Institute in the study of human immunodeficiency virus (HIV) seropositivity in that country].

Since late 1985, the Institut Pasteur de Madagascar has studied the problem of the distribution of HIV in the island. With the approval and cooperation of the Malagasy Ministry of Health, a number of different surveys have been undertaken, particularly of those sections of the population diagnosed as being of "high-risk". In 1987, a Malagasy prostitute from the country's second largest port was identified as seropositive. One case of clinical AIDS was diagnosed in 1988 in a Franco-Malagasy subject living in NOSY BE. Systematic tests for seropositivity among women with multiple partners in nine "high-risk" areas of the country are applied. The aim of this survey during 1988 is to make such groups aware of the vital necessity of overcoming the problem by preventing its growth by taking appropriate measures. The IPM has obtained government approval and has established the facilities to confirm findings of seropositivity among the population of Madagascar.

[Controlled results of the oral anticholera vaccine from the Institut Pasteur during the Shaba-Zaire epidemic in 1983].

Pasteur Institute oral cholera vaccine has been evaluated in the district of Malemba NKulu (Zaire). A cholera epidemic began 7 months after the vaccination. 216 cholera cases occurred among 18,377 control non-vaccinated subjects and 57 cases occurred among 6,249 subjects vaccinated by the conventional parenterally administered vaccine. In contrast, only 6 cholera cases occurred among 12,014 subjects vaccinated by the oral cholera vaccine.

[Prevalence of toxoplasmosis in Tananarive. Study conducted at the Pasteur Institute of Madagascar (PIM) on a sample of 2354 subjects].

The prevalence of Toxoplasma antibodies was studied in 2,354 individuals in Tananarive, at the Institut Pasteur of Madagascar. The seroprevalence among the malagasy population amounts to 52.1%. At the age of 10 years old, 36.3% of our sample have already acquired toxoplasma antibodies, proving an early infection. No differences have been observed between sexes. The toxoplasmosis prevalence among the female population coming from France, India and Pakistan and living in Tananarive seemed the same than in their home countries, whereas female population coming from China had shown a different seroprevalence compared to their home country.

Institut Pasteur v. United States: the AIDS patent dispute, the Contract Disputes Act and the international exchange of scientific data.

In the case of Institut Pasteur v. United States, the Institut Pasteur (Pasteur) claimed that the National Cancer Institute (NCI) had breached express and implied contracts to share research on AIDS virus samples provided to NCI by Pasteur. NCI scientists allegedly used the samples to acquire information which allowed NCI to file patent applications for an AIDS blood test kit. The United States Claims Court dismissed the complaint by holding that the Institut Pasteur had not complied with certain administrative procedures required by the Contract Disputes Act before bringing its suit. The United States Court of Appeals for the Federal Circuit reversed the decision of the Claims Court by holding that the disputed contracts did not fit within the scope of the Contract Disputes Act. Soon after the Court of Appeals decision, President Reagan and Prime Minister Chirac announced a settlement agreement whereby the lawsuit was to be dropped, American and French scientists were to share credit for having discovered the AIDS virus, and both parties to the suit were to share the patent rights for the AIDS blood test kit. This settlement suggest that international legal disputes involving urgent scientific and medical matters may require dispute resolution techniques that serve as alternatives to national courts.

[Edmond Sergent (1876-1969) and the Pasteur Institute of Algeria].

Edmond Sergent, supported by a distinguished team of colleagues, directed the Pasteur Institute of Algeria for over 60 years, from 1900 to 1963. As a student of Emile Roux, Sergent had received a Pasteurian training. His institute devoted extensive study to malaria. Sergent defined the concept of prevention and extended it to other pathologies. For many years, the Institute persevered in carrying out successful antimalarial campaigns such that Algeria was freed of the disease. In 1916-1917, Sergent and his brother were called upon to organise anti-malarial efforts for the Armée d'Orient. By way of systematic and energetic curative and prophylactic measures, they were able to eradicate the disease. In 1908, the Sergent brothers were the first to discover the role of the louse in the transmission of another disease, relapsing fever. The Pasteur Institute team also discovered the sand fly vector which transmits the parasite causing leishmaniasis. The Sergents found a new form of oculonasal myiasis, called "Thimni". In addition, they led effective campaigns against tuberculosis based on BCG vaccination administered throughout Algeria. The Pasteur Institute of Algeria conducted important research in plant and animal diseases. For example, they detected a trypanosome agent causing dromedary "debab", as well as its vector, the horsefly. They also studied in depth bovine piroplasmosis, which causes widespread and destructive disease, and demonstrated the role of the tick in promoting transmission generally. Their work in botany included the discovery that a Fusarium-type fungus was the causal agent for "baïoudh", the main disease of date palms. They also demonstrated the basic role of the fruit fly in alcoholic grape fermentation.
