Longitudinal profiles of oxygen uptake during treadmill walking in able-bodied children: the locomotion energy and growth study.

The purpose of this study was to document age-related changes in walking V(O(2)) in able-bodied boys and girls. Beginning at age 6 and ending at age 10, 23 children (14 girls, 9 boys) performed six 5-min bouts of level treadmill walking at 0.67, 0.89, 1.12, 1.34, 1.56, and 1.79 m s(-1) on an annual basis. Prior to data collection, subjects received 60 min of treadmill walking practice. During the last 2 min of each walking bout, a 2-min sample of expired air was collected in a meteorological balloon and analyzed to determine V(O(2)). Averaged across age, interindividual variation in V(O(2)) ranged from 32 to 41%. Repeated-measures analysis of variance demonstrated a speed by age interaction for V(O(2)), such that mean V(O(2)) rose (P< or = 0.05) across the five fastest speeds for 6-, 7-, 8-, and 10-year olds and increased over the entire speed range for 9-year olds. For all speeds, V(O(2)) decreased yearly from the ages of 6 to 8. When averaged across speeds, V(O(2)) was 27% higher for 6-year olds compared with 10-year olds. From a clinical perspective, access to longitudinal measurements of walking V(O(2)) in able-bodied children should be helpful in interpreting gait energy use in children with movement disorders and evaluating treatment strategies designed to reduce the aerobic demand of locomotion in youth with impaired mobility.

Chronic eosinophilic leukemia and hypereosinophilic syndromes. Proposal for classification, literature review, and report of a case with a unique chromosomal abnormality.

The idiopathic hypereosinophilic syndromes (HES) are rare hematologic disorders characterized by persistent eosinophilia with organ involvement that encompass a wide spectrum of clinical and hematological disease states. We propose a classification scheme to further delineate these patients, and present a case of a 45-year-old male with persistent eosinophilia, severe tissue and hematologic involvement, and trisomy 15. Although multiple cytogenetic abnormalities have been associated with hypereosinophilic syndromes, this is the first reported case where trisomy 15 is the sole chromosomal abnormality.

Non-targeted bystander effects induced by ionizing radiation.

Radiation-induced bystander effects refer to those responses occurring in cells that were not subject to energy deposition events following ionizing radiation. These bystander cells may have been neighbors of irradiated cells, or physically separated but subject to soluble secreted signals from irradiated cells. Bystander effects have been observed in vitro and in vivo and for various radiation qualities. In tribute to an old friend and colleague, Anthony V. Carrano, who would have said "well what are the critical questions that should be addressed, and so what?", we review the evidence for non-targeted radiation-induced bystander effects with emphasis on prevailing questions in this rapidly developing research field, and the potential significance of bystander effects in evaluating the detrimental health effects of radiation exposure.

Recurrent necrotizing jejunitis in a child.

We report the case of a 5-year-old boy with segmental necrotizing jejunitis, present a review of the literature including a single previously described North American child, and give evidence to document disease recurrence. This uncommon disease must be differentiated from Crohn disease because the treatment and prognosis are different.

Choroidal melanoma in a dog.

A 7-year-old male Great Dane dog was presented with blindness and a hemorrhagic retinal detachment in the right eye. Ocular ultrasonography revealed a conical-shaped mass in the posterior segment that protruded into the vitreous cavity. Histologic examination of the enucleated eye revealed a heavily-pigmented melanocytoma arising from the choroid lateral to the optic disc.

Recurrent chondrosarcoma of the cranial base: a durable response to ifosfamide-doxorubicin chemotherapy.

The case of a 46-year old woman with recurrent chondrosarcoma of the cranial base, refractory to neurosurgical intervention and external radiotherapy is reported. She received five cycles of systemic chemotherapy utilizing ifosfamide and doxorubicin which resulted in a durable clinical and radiographic response lasting 52+ months. A review of the management options for recurrent chondrosarcoma of the cranial base is also presented.

Dental caries amongst Royal Australian Navy recruits, 1988.

A sample of 1100 Royal Australian recruits were examined for dental caries in 1988. Socio-demographic data were also collected. The mean DMFT scores were 4.33, 6.85, and 8.87 teeth for the 15-19 year-olds, 20-24 year-olds, and 25-29 year-olds, respectively. Caries experience varied between the state of origin of recruitment with Victorian and Queensland subjects having the highest DMFT scores, and Tasmanian subjects, the lowest. Comparison with previous military studies indicated not only a decreased dental caries experience, but also that restoration of teeth rather than extraction is a more likely treatment outcome of dental caries in recent recruits.

Hypogammaglobulinaemia secondary to reflux oesophagitis.

A 7 year old girl referred for investigation of hypogammaglobulinaemia had hypoalbuminaemia and severe necrotising oesophagitis on oesophagogastroduodenoscopy. Nissen fundoplication resolved all clinical and laboratory abnormalities, but she remains under surveillance because of histological findings of Barrett's oesophagus. Erosive reflux oesophagitis can present with minimal localising symptoms, and hypogammaglobulinaemia and hypoalbuminaemia, presumably from protein loss.

Amount and dietary sources of caffeine and saccharin intake by individuals ages 5 to 18 years.

A nationwide, 7-day food consumption survey was utilized to assess average daily consumption of saccharin and caffeine by individuals 5 to 18 years old. The total sample's average daily saccharin and caffeine intakes were 4.1 and 37.4 mg, respectively. Only 14% of the individuals consumed saccharin while 98% consumed caffeine. On days when these dietary components were consumed, average saccharin intake was 87.4 mg and average caffeine intake was 47.9 mg. In general, intake levels of both dietary components increased with increasing age. However, on a body weight basis (mg/kg) caffeine intakes did not increase with increasing age. When expressed as milligrams of caffeine intake per kilogram body weight per day, children 5 to 6 years old had significantly higher intakes (1.1 mg/kg/day) than 7 to 8 years olds. No other age differences were noted. Artificially sweetened carbonated beverages contributed the greatest number of milligrams of saccharin to total intake while tea, followed by carbonated beverages, made the most significant impact on caffeine consumption. Considerable variation was found for both saccharin and caffeine consumption levels among the sample members as well as for each individual during the 7 days surveyed.

Old age and environmental docility: the roles of health, support and personality.

This study assessed the generalizability of Lawton and Simon's environmental docility hyopthesis across three domains of personal competence. The hypothesis was supported, in that a survey of 102 people (aged 60 to 92) found environmental press to be related significantly to adjustment only among less competent individuals in terms of (a) health (i.e., those with poor ratings of health, vision, and hearing, the old-old, and men); access to caregiving networks, (i.e., those who were unmarried, had infrequent contact with children and family, and lived in smaller communities); and (c) personality, (i.e., those with low self-esteem, and external locus of control, low assertiveness, and an inability to deal with the threat of dependency).

Mechanisms of differential growth of heart ventricles in newborn pigs.

The left ventricular free wall (LVFW) grew approximately three times faster than the right ventricular free wall (RVFW) during the first 10 days of life in neonatal pigs. Faster growth was associated with proportional increases in total RNA and messenger RNA. These findings indicated that greater capacity for protein synthesis was a major factor in accelerated growth. Despite faster growth, heart content of ribosomal subunits was higher in piglets than in 60-day-old pigs or adult rats, suggesting a relatively slower rate of peptide chain initiation than elongation. When hearts from 5-day-old pigs were perfused in vitro, protein synthesis was more rapid in the LVFW than in the RVFW. In the absence of added insulin, the higher rate was due to both greater efficiency and greater capacity for protein synthesis. In the presence of the hormone, greater capacity was responsible for the increased rate of protein synthesis in the LVFW as compared with the RVFW.

Metastatic disease first presenting as eyelid tumors: a report of two cases and review of the literature.

We present two unusual cases in which an eyelid tumor was the first sign of metastatic disease. The first involved a 53-year-old man with pulmonary carcinoma and the second a 71-year-old man with malignant lymphoma. Fifteen similar cases from the literature are reviewed. The most frequent primary lesion is breast carcinoma in women, which appears as a diffuse lesion of one or two eyelids. Metastatic lung carcinoma in men appears as solitary nodules, representing the second-most common type of lesion. The questions of left- or right-side predominance, age and sex of patients, types of tumors, and prognosis are discussed.

Single unit activity in frontal cortex and caudate nucleus of young and old rats.

Spontaneous neuronal activity was recorded extracellularly from isolated single units in frontal neocortex and caudate nucleus of young and aged F344 rats anesthetized with urethane. Average firing rates, mean interspike intervals (ISI) +/- standard deviations, and ISI frequency histograms were computed and analyzed by microprocessor. For frontal cortex cells (N = 226), there was a nonsignificant trend toward slower average discharge rates in the old group. However, a significantly longer mean ISI and proportionally more very slow firing cells (less than 1 Hz) were observed in old rats. A laminar analysis of frontal cortex unit activity in young animals showed average discharge rates to be distributed somewhat evenly throughout the cortical mantle with the exception of the zone 1200-1400 mu beneath brain surface. This depth corresponds approximately to layer V where a 50% increase in mean firing rate in young animals was observed. In aged animals, this increased cell firing in layer V was absent, while mean discharge rates in other laminae remained essentially the same in the young and old rat groups. Caudate nucleus cells (n = 70) showed a significant shift towards fewer fast discharging cells in old rats, with the average firing rate diminished by one-third. Although more brain regions need to be examined in a similar fashion, the consistency of the present results with those previously reported for the brainstem and cerebellum suggests that slower firing rates and longer ISIs are likely to be wide-spread throughout the brains of aged rats.

Life history of Spalangia gemina Boucek (Hymenoptera: Pteromalidae), a fast-breeding microhymenopteran pupal parasitoid of muscoid flies.

Laboratory studies at 25.5 degrees C and 70-75% r.h. demonstrated that the average daily survival rate for females of the parasitoid Spalangia gemina Boucek (Hymenoptera: Pteromalidae) was 0.907 over a 20-day period, with 50% mortality in 17.3 days. Mean immature developmental time of S.gemina was 22 days and the population growth rate was c. 40-fold per generation. The females successfully parasitized 1-, 2- and 3-day-old pupae of Musca domestica L. and parasitism of 98-100% was obtained at parasitoid to host ratios of 1:0.5-2.5. Parasitism of 81-87% was obtained at parasitoid to host ratios of 1:10. The male to female sex ratio was 1:1.59. S.gemina appears to have advantages over other Spalangia spp., which have immature development times of 28-32 days under comparable conditions, for the biological control of Muscidae.

Study of the pedigree of a patient with type 3 hyperlipoproteinaemia and sinking prebeta lipoprotein.

A 49-year-old woman, suffering from peripheral vascular disease, was found to have two lipoprotein abnormalities, namely, type III hyperlipoproteinaemia and sinking prebeta lipoprotein. Twenty-one members of the kindred were investigated in an attempt to determine the pattern of inheritance of both of these abnormalities. In a 21-year-old son of the proband plasma electrophoresis was in keeping with a type V hyperlipoproteinaemia. It is suggested that this may be a stage in the development of the characteristic type III pattern. If this is so it is consistent with previous suggestions of an incompletely penetrant single autosomal allele. Sinking prebeta lipoprotein was found in the plasma of two other members of the family. If this abnormality is also inherited as a single autosomal allele it must have a very low penetrance and the possibility that the abnormality is acquired cannot be excluded.

Perceptual strategies in prelingual speech segmentation.

Previous work has suggested that infants may segment continuous speech by a BRACKETING STRATEGY that segregates portions of the speech stream based on prosodic cues to their endpoints. The two present studies were designed to assess whether infants also can deploy a CLUSTERING STRATEGY that exploits asymmetries in transitional probabilities between successive elements, aggregating elements with high transitional probabilities and identifying points of low transitional probabilities as boundaries between units. These studies examined effects of the structure and redundancy of speech context on infants' discrimination of two target syllables using an operant head-turning procedure. After discrimination training on the target syllables in isolation, discrimination maintenance was tested when the target syllables were embedded in one of three contexts. Invariant Order contexts were structured to promote clustering, whereas the Redundant and Variable Order contexts were not. Thirty-six seven-month-olds were tested in Experiment I, in which stimuli were produced with varying intonation contours; 36 eight-month-olds were tested in Experiment 2, in which stimuli were produced with comparable flat pitch contours. In both experiments, performance of the three groups was equivalent in an initial 20-trial test. However, in a second 20-trial test, significant improvements in performance were shown by infants in the Invariant Order condition. No such gains were shown by infants in the other two conditions. These studies suggest that clustering may complement bracketing in infants' discovery of units of language.
