Comparative multicentre trial of three IUDs inserted immediately following delivery of the placenta.

A multinational comparative trial of three IUDs (Copper 7, Lippes loop size D and the Postpartum T) randomly inserted immediately following delivery of the placenta was conducted in six centres. A total of 841 women entered the study. As the predetermined termination indices for expulsions were exceeded at six months the trial was prematurely closed. An excess of expulsions during the first 48 hours following insertion was observed for the Lippes loop compared to the other devices. At six months the expulsion rate for the Lippes loop was significantly higher than that for the Copper 7. In addition, the discontinuation rate for the Lippes loop at 12 months was significantly higher than that for the Copper 7. There were no significant differences in either the expulsion rates or the discontinuation rate at six or twelve months between the postpartum T and the other devices. At 12 months the pregnancy rates with all three devices was high; there were no ectopic pregnancies. Considerable between-centre differences were observed, particularly for expulsion rates. Possible reasons for this are discussed and future research lines are suggested.

A survey of contraceptive use and unplanned pregnancy in Perth, Western Australia.

Contraceptive use and unplanned pregnancy were studied in a stratified cluster sample of 1,511 couples with women aged 16 to 44 years resident in metropolitan Perth in 1988. Twenty-one couples were excluded from analysis owing to missing data. The proportion of couples using contraception was 76.8 per cent (1,144 of 1,490), and all but three of the remaining couples gave a reason for nonuse. Among users, surgical sterilisation made up 42.3 per cent (484 of 1,144) of all methods, with a slight predominance of tubal ligation over vasectomy. Oral contraceptives accounted for just over half of nonsurgical methods. Comparisons with a Victorian survey performed in 1978 suggested that surgical methods and condom use may have increased, whereas use of oral contraceptives and intrauterine devices may have fallen. Unplanned pregnancies conceived during the 12 months before the interview affected 3.4 per cent (51 of 1,490) of respondents. The incidence of unplanned pregnancy was four times higher in couples with inconsistent usage patterns of contraception.

Clinical experience with Progestasert beyond one year of use.

A total of 254 women were enrolled in the study of the intrauterine progesterone system, Progestasert, and some of the subjects have been followed into the 8th year of use of the device. Clinical experience on 192 women in their 1st year of use of the device was reported in 1977. This follow-up study reports on 110 patients who had at least 2 insertions of the Progestasert for continuous use of 13 months, to a maximum of 8 insertions for continuous use of up to 86 months, for a total of 3808 months. 60% of subjects were nulliparous women who tolerated the device as well as the multiparous. The expulsion rate was low (3.5/100 women-years) noting that 82% of expulsions occurred within 2 months postinsertion or exchange of the device. A total of 7 pregnancies were observed; the pregnancy rate was 2.2/100 women-years. 2 of these 7 pregnancies were ectopic. No uterine or cervical perforations and no serious infections were observed.
