Suppression of hemoglobin H in disorders of iron metabolism.

Disorders of iron metabolism affect the expression of hemoglobin H in hemoglobin H disease. Two cases of iron deficiency with reduced synthesis of hemoglobin H are described in the literature. We report two more cases, one with anemia of chronic disease and another with alcoholic sideroblastic anemia where the hemoglobin H was not detected at presentation and appeared after treatment of the underlying disorder. The pathogenesis of suppression of hemoglobin H is discussed.

Aceruloplasminemia, an iron metabolic disorder.

Aceruloplasminemia is an inherited disorder of iron metabolism caused by the complete lack of ceruloplasmin ferroxidase activity caused by mutations in the ceruloplasmin gene. It is characterized by iron accumulation in the brain as well as visceral organs. Clinically, the disease consists of the triad of adult-onset neurologic disease, retinal degeneration and diabetes mellitus. The neurological symptoms, which include involuntary movements, ataxia, and dementia, reflect the sites of iron deposition. Severe iron overload and extensive neuronal loss were observed in the basal ganglia, while iron deposition and neuronal cell loss were trivial in the frontal cortices. The cerebellar cortex showed marked loss of Purkinje cells. Iron deposition was more prominent in the astrocytes than in the neurons. Excess iron functions as a potent catalyst of biologic oxidation. Astrocytic deformity and globular structures are characteristic features in aceruloplasminemia brains. The globular structures in the astrocytes were seen in proportion to the degree of iron deposition and reacted positively to anti-4-hydroxynonenal, one of the indicators of lipid peroxidation, and anti-ubiquitin antibodies, but not to anti-alpha-synuclein antibody. The lack of ceruloplasmin may primarily damage astrocytes in the aceruloplasminemia brains through lipid peroxidation. Ceruloplasmin may play an essential role in neuronal survival in the central nervous system.

[Anemias resulting from iron metabolism disorders].

For the most part iron deficiency, but also disturbances of distribution, transport and utilization of iron, are able to cause anemias. To understand the origin of such anemias, firstly iron metabolism, then the clinical symptomatology as well as the laboratory parameters necessary for diagnosis and finally the usual therapies are described.

[Serum ferritin and its diagnostic significance in iron metabolism disorders].

Aim of the study was the evaluation of the diagnostic value of the parameters of iron metabolism in normal adults and also in patients suffering from uncomplicated iron deficiency, iron overload due to repeated blood transfusions, malignant lymphoma and Crohn's disease. In these patients, the determination of serum ferritin increased the diagnostic efficiency only in poly-transfused patients with iron overload.

[Compartment analysis in the diagnosis of iron metabolism disorders].

The principle of the ferrokinetics according to the catenary model and to the multilocular system of Pollycove and Mortimer is described in short. The most important results of the investigation of the iron metabolism of 15 patients with chronic myelosis, 31 with osteomyelofibrosis and 68 with chronic liver diseases according to the catenary model are reported. According to the multicompartment model the ferrokinetic investigations of 18 patients with chronic liver disease as well as of one patient each with chronic myelosis and panmyelopathy were evaluated. An increased depot iron metabolism and a decreased effective erythropoiesis could particularly clearly stated in the latter patient. The statements to various parameters according to the two model demonstrations are compared.

[Perioperative correction of hypochromic anemia and iron metabolic disorders in patients with liver cirrhosis and portal hypertension].

Forty eight patients with liver cirrhosis and portal hypertension have been examined, most of them had hypochromic anemia. Serum iron and ferritin levels, total and latent iron binding capacity have been radioimmunoassayed. All the patients developed hyposiderosis, the study of liver and spleen bioptates showed tissue iron deficiency. It has been established that measurement of ferritin, total and latent iron binding capacity were not informative. It is iron concentration that should be determined. Intravenous administration of high doses of the drug Ferrum-lek seems most effective. It recovers red blood count, iron metabolism and iron tissue stores and reduces the incidence of pyogenic and cardiovascular complications.
