[Health resorts--dissatisfaction with health resorts].

An efficient medical rehabilitation is an important part of a modern health care system. There should be no doubts about its necessity. The medical rehabilitative system in Germany however, as it is organized by the sick-insurances and the annuity-insurances, does not fulfil all the claims urgently required. Much criticism seems really correct: the missing scientific basis of rehabilitation, the poor flexibility around the administrative procedures of application and performance, which is inadequate to medical needs, the nearly total absence of any negative selective criteria on the one hand and the huge problems in finding out those persons badly needing rehabilitative measures on the other, and last not least the uncertainty regarding the real economic benefits of the total enterprise of rehabilitation as it is actually practised.

[Evaluation of air hygiene in health resorts].

Concerning the maintenance and restoration of health in patients being subjected to health resort treatment, the spas play an important role. Therefore, legal provisions in this field establish the elimination of unfavourable environmental conditions, as air pollution, for instance. The geographical situation of a health resort exercises influences on its air quality considered in terms of hygiene. This is linked with different atmospheric and bioclimatic conditions in various regions. The major source of air pollution in a health resort is represented by housing and the communal area, by traffic on the roads and partly by industry. Instructions for measuring and evaluating of pollutants are given. Furthermore, various possibilities for improving the air quality in health resorts are mentioned.

Air Pollutants

[The principles of health resort treatment].

"Cures" prescribed in spas and health resorts are useful especially for patients who are chronically ill. The differences between pharmacotherapy and physiotherapy are discussed. The treatment in health resorts produces adaptations to repeated irritations, which need time, mostly four weeks. However, the changes of the function of circulatory system, lungs, metabolism etc. do not disappear after the end of the cure. To this extent, the principles of treatment in health resorts are comparable to sporting training. The days of incapacity to working are reduced to 50-60% in the following two years after a successful cure.

Ballistocardiography in sanatorial and health resort rehabilitation of patients after myocardial infarction.

A sanatorial and health resort rehabilitation was carried out in patients after myocardial infarction. The rehabilitation program included exercise therapy and carbonic acid baths. The ballistocardiographic studies conducted with a diagnostic purpose indicate that carbonic acid balneotherapy may be used in patients after the third month following the onset of myocardial infarction. Clinico-physiological investigations indicate that ballistocardiography may serve to control the effectiveness of sanatorial and health resort rehabilitation and to determine the mechanism of action of the physical factors involved.

[Results of treating the residual manifestations of uveitis of rheumatic and rheumatoid etiology at a health resort (the town of Yevpatoriya)].

The effect of a complex health resort treatment including eye applications of medical mud from the Sax lake has been studied in 100 patients (172 eyes) with residual effects after uveitis of rheumatic and rheumatoid etiology. Besides improvement of the general patient's state, the treatment provided a rise of visual functions, reduction of recurrences in remote terms. This effect can be, to a great extent, explained by antiinflammatory, resolving and desensitizing action of medical mud. The positive results obtained allow to recommend this method of treatment in conditions of a health resort and outside it (imported medical mud).

[Health resort treatment of children with progressive muscular dystrophy].

The results of a complex treatment at health resorts of 126 children with Duchenne myopathy and 84 children with spinal amyotrophies at the age of 5-15 years are given. The complexes included carbon-dioxide sulfurated hydrogen baths, mud application (with different localization depending on a form of the disease), fresh baths, oxygenotherapy and therapeutic physical training. The authors revealed good tolerance to procedures and the positive dynamics of the main clinical manifestations of the disease, confirmed by the electrophysiological and biochemical studies. As a result of the treatment it was possible to achieve progressiveness of the disease in all patients at different periods (from 1 month to 1 year). The authors give recommendations for the use of treatment at health resorts as a necessary link in the general chain of rehabilitative measures for children with various forms of progressive muscular dystrophies.

[Physical training and sports exercises within the framework of an early rehabilitation programme at a health resort (author's transl)].

The article reports on early rehabilitation measures carried out at a health resort on nearly 4000 patients with signs of "wear and tear", degenerative and other changes of the musculo-skeletal system as well as insufficiencies of cardiovascular system. Provided the preliminary diagnostic examinations are carried out and medical supervision ensured physical training and sports become increasingly important as integral parts of an active treatment plan including natural healing methods. Sport-physiological experiences have, however, shown that a real and measurable positive training effect can only be achieved if these components are administered in the correct dosages. This holds particularly true if a wide spectrum of gymnastic and sports exercises is offered - primarily in groups and under professional supervision - which are not only intended to increase physical capacity, but also respond to individual inclinations. Thus, in addition to its therapeutic value, early rehabilitation at a health resort can provide the feeling of achievement appropriate to give the rehabilitees new impulses at home. As a consequence, these programmes make a positive contribution to the well-being of the general public.

Balneology

[Phenologic observations of the Rabka health resort in 1990].

Important factors decisive of the health resort's climatic characteristics are the results of aero-phenologic studies. These results take in account the different pollinating seasons of various plants, grasses and weeds. Quantification and qualification of air pollination using the volume method with a Burkhard analytical device. Basing on results the authors have found increasing pollination string from March. At his time the main pollens come from birch, hazel, alder, weeping willow. The concentration of these pollens decrease in April, completely diminishing in May. Grass pollens increase in mid-May, peaking in June to early August. Pine pollens are found in May and June. A characteristic feature of the phenologic calendar is pollination of grass and pines in the same period (late May and June). The results of this are seen in children treated at the Mother and Child Institute Branch in Rabka, who demonstrate increased signs of pollinosis. The results of this study should modify treatment and change prophylaxis in children treated in Rabka.

[Methodologic approaches to assessing the noise level of health resorts and places of mass recreation of the population].

Methodological approaches to the assessment of noise regime at health resorts and places of rest of the population were set forth when carrying out preventive and current sanitary surveillance. The sources governing the noise regime of health resorts and places of rest were identified, and the problems of zone arrangement of places of rest according to its type (long-term or short-term) analysed. Permissible noise levels were given for various functional zones and the measures for normalization of noise regime proposed.

[Effect of Archman health resort factors on the bile chemistry of chronic cholecystitis patients].

During remission, patients (n-83) with chronic cholecystitis showed the presence of lithogenic bile, a disorder which was eliminated under the action of Archman (the Turkmenian SSR) health resort factors. The efficacy of health resort treatment manifested itself in the improvement of the patients' clinical status, normalization of the kinetics of the gallbladder and bile chemistry. The choleretic and cholelytic properties of Archman mineral water permit recommending its use both for the purpose of correcting bile lithogenicity and prevention of the lithogenesis.

[10-year experience with the health resort treatment of eye diseases].

Presented are the kinds of ocular diseases which can be qualified for a health-resort therapy and the kinds of measures the most beneficial in particular cases. After the cure an improvement in the condition of the eyes was found in 68 p.c. of cases. On the basis of results one may conclude that the rehabilitating health-resort treatment is worth to be suggested.

Balneology

[The efficacy of health resort treatment of psoriatic arthritis at Yevpatoriya].

The authors analyze the efficacy of treatment of 162 patients suffering from psoriatic arthritis in Evpatoria health resort. The treatment modalities included brine baths, mud applications, climatotherapy, etc. Clinical cure or considerable improvement has been achieved in 21.8% of patients, improvement in 74.3%, no therapeutic effect in 3.9%. Along with alleviation of the clinical manifestations of psoriatic arthritis, a number of immunological and biochemical parameters (albumin, lysozyme, IgM levels) have shown a tendency to normalization. The combined health resort therapy is an effective means of the after-care of psoriatic arthritis.

[The treatment of diabetics at the health resort of Truskavets].

Results are reported of the treatment of 265 patients with different complications of diabetes mellitus at the endocrinological department of the Truskavets health resort. In patients with fatty hepatosis treatment resulted in normalization of the lipid complex of the blood and intrahepatic blood flow while patients with diabetic angionephropathy showed an increase of glomerular filtration and tubular reabsorption. The efficacy of different methods of health resort treatment in these patients is described.

Combined Modality Therapy

[Indicators of the blood kinin system in patients with bronchial asthma during health resort treatment].

Indicators of the blood kinin system were studied in 57 persons including 42 patients with asthma and 15 healthy persons (control group) in the Kislovodsk health resort area. Activation of the blood kinin system (a decrease in the content of prekallikrein, fast- and slowly reacting kallikrein inhibitors) was observed in patients with infectious-allergic and mixed forms of asthma. Positive shifts of the kinin system indicators pointing to a decrease in the "production" of free kinins were observed in the patients under the influence of multimodality therapy at the health resort.
