[Midterm achievement and problems of all municipal health and welfare plans for the elderly in Yamagata Prefecture].

PURPOSE: This study was made for providing information in the preparation of Social Welfare Systems in municipalities. METHODS: Based on seven official reports by Yamagata Prefecture, quantitative data on health and welfare services were analyzed. We investigated the midterm achievement and problems of a Municipal Health and Welfare Plan for the Elderly in all 44 municipalities by mail survey (response rate 100%). The questionnaire included the progress situation of the plan, the existence of an assessment organization, and the characteristics of the plan. RESULTS: Both health service and welfare service achieved targets level to a degree. Concerning the progress situation, "The midterm achievement is not satisfactory" for 65.9% (29 municipalities). The main reasons were "budgets" (93.1%, 27 municipalities) and "lack of manpower" (75.9%, 22 municipalities) all of which indicates "lack of home helpers". 58.6% of all 44 municipalities were reconsidering the plan. In assessment organization existed in 31.8% (14 municipalities) of all municipalities. CONCLUSION: It is necessary that analysis should consider not only use of quantitative data but also qualitative one. The major problems found are that community characteristics were not reflected and the opinion of inhabitants were not considered.

[Awareness of health and welfare planning for elderly and methods to increase awareness: based on social marketing].

To clarify the socio-psychological factors affecting awareness of health and welfare planning for the elderly, a survey of community residents was performed in the Tokyo metropolitan area. The results obtained were as follows: 1. There were three factors which affected awareness concern about health and welfare services for the elderly as a social issue: concern about the local community; and concern about the local government. 2. Two other factors which did not affect the level of awareness were: anxiety about the health and welfare services for the elderly as a personal issue; and having personal experience of nursing care. From these results, a method to increase awareness was studied based on social marketing methods. Two primary target groups for increasing awareness were identified. One target was people who have the socio-psychological factors described above. Another target was people who have the potential need for health and welfare services but who were not aware of it. That is, they have relatively high anxiety about health and welfare services for the elderly as a personal issue and have personally experienced nursing care. The method of approach for these targets were studied. For the first group, the amount of the information available seemed to be important because these persons are ready to recognize the need for planning. Therefore, efficient information channels should be selected. For the second group, approaches that generate greater consumer participation by presenting this as an efficient method for solution of the problem should be adopted.

A deprivation index for health and welfare planning in Quebec.

Given that one of the goals of public health policy in Quebec and Canada is to reduce social inequalities in health and welfare, it is surprising, to say the least, that most information systems in this field make no mention of people's socio-economic characteristics. The present article proposes an index to reflect the material and social dimensions of deprivation as this concept has been developed by Peter Townsend and other authors. The article describes the method used to create the index, which uses census data and tools developed by Statistics Canada to match postal codes with enumeration areas. Examples are provided of the use of the index in information systems covering three aspects of health and welfare in Quebec: deaths, hospitalizations and births. The value of the information provided by this index in planning health and social services is demonstrated.

Managing health and welfare plans instead of letting them manage you.

The explosive growth and change in the health care provider industry is presenting a considerable challenge to employers that manage these benefits for their employees. Corporate mergers, supportive federal and state legislation expanding benefit availability and access to new consumer markets are a few of the forces changing the shape of the industry. Furthermore, participants are more knowledgeable about their benefit plans and are more vocal about their needs. The authors discuss these challenges and possible solutions for the employer that is attempting to determine how plan delivery and management needs can be served in a way that supports business environment and strategy.
