[Intramural blood vessels in the AV segment of the human heart conduction system].

We have investigated the intramural blood vessels of the A-V segment in the conductive system of 20 human hearts using the method of light microscopy. The results of our study have shown that the A-V segment of the conductive heart muscles in human hearts has an abundant vascularisation. We have found the greatest density of the intramural net in the dorsal part of the A-V node; this density of blood vessels decreased toward the Hiss' bundle. After comparing these results with the earlier findings in the study of the intramural net in the S-A system we have found that A-V segment of the conductive heart system has a poorer blood supply in regard to S-A system. We have also found that the intramural net of blood vessels is weaker in the adjacent ventricular parts and in the interventricular septum. It cannot be said that there exists any surface vascular zone subepicardial, middle and deep sub-endocardiac--in the A-V segment of the conductive system of human heart.

[Ultrastructure of the heart conduction system in diphtheria].

Cytoarchitectonics and ultrastructure of the node and conducting myocardiocytes of the conducting system of the heart and surrounding blood capillaries and nervous fibers are described. In diphtheria, metabolic and destructive changes were found in the node myocytes, conducting myocardiocytes and surrounding structures. The most noticeable changes were seen in the left pedicle of the atrioventricular fascicle and later others components of the conducting heart system. These changes are more pronounced in clear cells of conducting myocardiocytes as compared to the node myocytes.

The pathology of the heart conduction system in congenital heart block.

The incidence of congenital complete heart block is estimated in 1 of 2500-20,000 births. Many cases are isolated (found in an otherwise normal heart) and the pathology of the heart conduction system is variable. We report a 51-year-old man with the diagnosis of complete heart block, with a permanent pacemaker. No family history of rhythm disturbances was available. The patient presented and endocarditis after replacement of the pacemaker battery. The prognosis was poor and the patient died three months later. Autopsy examination showed signs of shock, of septic origin. The heart was hypertrophic (450g) and the left ventricle thickened. Histopathological examination of the heart conduction system showed that the sinus and atrioventricular nodes were normal, but the His bundle was interrupted and replaced by fibrous tissue. No inflammatory signs were present. Loss of conducting fibres and their replacement by fibrous tissue is the most common pathological process in complete heart block. In this case His bundle was mostly affected, different to Lev's disease where the process is more distal (branching atrioventricular bundle) and to Lenegre's disease, which shows a diffuse damage in the conducting system.

[Morphological characteristics of heart conduction system in diphtheria].

Heart conductive system (HCS) including the sinoatrial node (SAN), atrioventricular node (AVN), AV bundle (AVB) and its left and right parts was studied histologically. It is shown that different portions of HCS are damaged in diphtheria with different frequency and intensity, the frequency depending on the depth of their location. Left part of AVB is affected most frequently, followed by the right part of AVB, AVB, AVN and the least frequently SAN.

[Effect of ildamen on the heart conduction system in ischemic heart disease patients].

Thirty-four patients with ischemic heart disease (IHD) were studied: 18 IHD patients with latent forms of disturbances of conductivity in the atrioventricular junction and sinus node sickness syndrome revealed at frequency stimulation of the heart; 16 patients with acute myocardial infarction complicated with the atrioventricular junction blockade of II and III degrees. Ildamen solution (4 mg) was slowly intravenously infused to all patients under study. Exerting the beta-stimulating effect, ildamen positively influences restoration of the disordered function of the pacemaker and conduction system of the heart: through immediate action on these systems and an increase of the coronary blood flow contributing to improvement of nutrition of the condition system of the heart.

[Immunohistochemical findings in the heart conduction system].

Immunohistochemical investigation of heart conductive system showed that degenerative changes described by James (9) in some cells of the system had a nature of programmed death. Extinction of certain of number of cells of a reducted part of the system was found in membranous septum. Apoptotic antigen (21) could be proved in some destructed cells by using anti-Bax and anti-bcl-2 antibodies.

[Vascularization of the sinoatrial segment in the heart conduction system in bovine and canine hearts] ].

Vascularisation of the conductive heart musculature was investigated by means of the serial coronorography and injective-corrosive method in bovine and canine hearts. The S-A system of the bovine heart was in 50% of the investigated hearts was supplied by the anterior left by the anterior right sinus artery (Arteria nodus sinuatrialis anterior sinistra), and in 30% of the cases by the anterior right sinus artery (Arteria nodus sinuatrialis anterior dextra). Both anterior sinus arteries vascularize the S-A bovine heart system in 20% of the investigated cases. The S-A canine heart system, in 50% of the investigated cases, receive vascularization from the anterior left sinus artery, from the right anterior artery in 40% of the cases, and only in 10% of the investigated hearts is the S-A system supplied by the right and left posterior sinus arteries (Arteria nodus sinuatrialis posterior dextra et sinistra). The S-A system vascularisation of the conductive heart musculature is not conditional by the type of the arterial vascularisation of the heart.

[General anesthesia tools in endovascular surgery of the conductive heart system].

The anesthetic management course was analyzed in 224 patients who underwent nonvascular surgeries on the conductive heart system. Analgesic and anti-stress techniques, which do not affect the intracardial conductivity and ensure the successful outcome of surgery with spontaneous or auxiliary ALV, were designed on the basis of research. The above schemes were introduced in practice with their efficiency being confirmed. They are based on a balanced use of the new-generation non-steroid anti-inflammatory drugs, like Xephocam, bezodiazepines and fentanyl (when used at subnarcotic doses that do not affect the intracardial conductivity). The main analgesic component of lornoxycam was sufficient when used at a dose of 0.1 mg/kg and at a total dose of equal to or below 16 mg.

[Surgery of the heart conduction system].

Problems concerned with surgery of the heart conduction system are discussed. Two operations performed directly on the heart conduction system are analysed: operation for dividing the bundle of His or Keith's node with subsequent implantation of electrocardiostimulators in ventricular tachycardia unresponse to drugs and operation for dividing the walls of the right atrium and interatrial septum leading to the development of atrioventricular rhythm. The incisions mentioned were used in approach to the mitral and tricuspid valves for their prosthetics. The second part of interventions on the heart conduction system involves surgical techniques aimed at avoiding damage to the conduction system in manipulations mainly on the cardiac septum. The author believes that this branch of cardiac surgery is new and promising.

Atrial natriuretic peptide in the innervation of the bovine heart conduction system: relationship with substance P and autonomic innervation--immunohistochemical studies.

Recently, we observed that atrial natriuretic peptide (ANP) immunoreactivity (IR) was present not only in the Purkinje fibres, but also in nerve fibre varicosities in the conduction system of the bovine heart. These findings and previous observations that ANP is able to influence autonomic neurotransmission in the heart, lead us to elucidate the possible occurrence of ANP in the sympathetic and/or parasympathetic nervous systems and/or in various types of peptidergic innervation in the conduction system. The different parts of the conduction system of bovine hearts were dissected out and processed for immunohistochemistry including double-staining, using antisera against ANP, tyrosine hydroxylase and different neuropeptides. We observed that some of the nerve fibre varicosities exhibiting ANP-IR showed substance P-IR and that ANP was present as scattered immunoreactive granules in intracardial, presumably parasympathetic, ganglionic cells. The study shows that ANP is likely to be present in parasympathetic innervation and in afferent nerve endings in the bovine heart conduction system.

Immunohistochemical study on the development of the rat heart conduction system using anti-Leu-7 antibody.

Morphological aspects of the heart conduction system have been studied by various histochemical markers. However, the actual presence of markers in the conduction system and its developmental mode remain controversial. We have shown the anti-Leu-7 antibody to cross-react with cells of the rat embryonic heart conduction system by an immuno-electron microscopic study. A comparison was thus made between the results of Leu-7 immunohistochemistry and those by other markers previously used for studying the development of the conduction system. As a result, Leu-7 immunoreactivity proved the most reliable marker for studying the conduction system of the developing rat heart. Examination of the developmental mode of a rat conduction system was facilitated with the use of this marker. The immunoreactivity for Leu-7 initially appeared in the anterior wall of the bulboventricular region at 10 days of development. The development of three internodal tracts and two atrioventricular nodes was demonstrated in the following stage. These primordial atrioventricular nodes, one of which connects to a bundle of His, fused to form a single distinctive node followed by completion of the conduction system in the 18 day fetus.

[Functional indices of the heart conduction system in patients with normal atrioventricular conduction].

A total of 16 patients with normal atrioventricular conduction were studied by the method of intracardiac electrophysiologic study. Time intervals of the conduction through the atria and the His-Purkinje system did not change during frequent atrial stimulation. The conduction time through the atrioventricular node increased, the periodical block with a location in the very node developing at a stimulation frequency of 181+/-18 beats/min. The duration of functional and effective refractory periods of the atria and the atrioventricular node depended upon heart rate. A complete block of retrograde conduction through the node was revealed in 5 of 13 patients by stimulating their ventricles. A frequency dependence of the conduction through the atrioventricular node and periodic block in relatively low stimulation frequency were observed in the others. The findings determine normal values of the indices studied and make a basis for the identification of anomalous pathways of atrioventricular conduction.

[Heart conduction system under conditions of general anesthesia. Results of a 15-year study].

Effects of 14 anesthetics, narcotic analgesics, benzodiazepines, and their combinations on the cardiac conduction system (CCS) were studied in 470 surgical patients of a general profile during operation and narcosis by transesophageal electrocardiostimulation. Interactions between components of total anesthesia can improve or suppress the intracardiac conduction. The authors classified the data on the effects of agents used for narcosis on the sinus node function and atrioventricular and anteretrograde conduction. The results will help an anesthesiologist in a differentiated approach to the choice of anesthetics for surgical patients with initially disordered CCS and heart rhythm.

[Function of the heart conduction system in Wolff-Parkinson-White syndrome].

A study of the functional state of the heart conduction system by means of electrophysiologic testing of 46 patients with the Wolff-Parkinson-White syndrome has shown that the peculiarities of atrioventricular conduction and the development of an electrocardiographic picture in this disease are determined by the functions of anomalous accessory atrioventricular pathways--more frequently of the Kent's bundle and more infrequently of the Macheim's bundle. In addition, the degree of manifestations of the ventricular pre-excitation on the ECG depends on the correlation between impulse conduction times along the anomalous and normal pathways. The slow conduction along the Kent bundle form the basis of the latent WPW syndrome not recorded by the ECG.
