Are Langhans giant cells precursors of foreign-body giant cells?

Granulomas were induced in rats by subcutaneous implantation of pieces of Melinex plastic into the dorsum. The pieces of Melinex were removed at intervals varying from 16h to 14 days, and the adherent cells were studied morphologically and quantitatively. Giant cell formation started about 32h after implantation. The first giant cells to appear were of the Langerhans type. Two days after implantation, most of the giant cells are still Langhans-type cells. A few giant cells of the foreign-body type and transition forms between the Langhans and foreign-body types are also present. From the third day on, the foreign-body type gradually becomes predominant. Independent of the duration of implantation, giant cells with 3, 4, or 5 nuclei are virtually without exception of the Langhans type. The higher the number of nuclei between 6 and 30, the more cells are of the foreign-body type. Giant cells with 30 or more nuclei are all foreign-body type. The findings are discussed in the light of current knowledge concerning giant cell formation. It is concluded that under the present experimental conditions, Langhans-type giant cells are the precursors of foreign-body-type giant cells.

Studies on subcutaneous fat necrosis of the newborn.

Biopsy specimens from the skin and subcutaneous fat tissue of four cases with neonatal subcutaneous fat necrosis were made and investigated by light and electron microscopy at 2, 4, and 6 weeks, and 5 months (Case 2) from the onset of the disease. Three stages of ultrastructural change of fat cells were observed. The evolution of crystal formation in the fat cells was seen and phagocytosis of crystals and fat droplets by macrophages and foreign-body giant cells was also noted. In the light microscope accumulation of calcium concretions in the spaces between and inside the fat cells was found. In the electron microscope we detected foci of highly electron-dense granules, which were similar in distribution and structure to calcium salts stained with the von Kossa method. Changes in small and medium size blood vessels were observed.

Reactions of the articular capsule to wear products of artificial joint prostheses.

Examination of a great number of tissue samples taken from the newly formed capsules surrounding artificial joints reveals small particles of prosthetic material. Abraded from the joint by wear and tear, these particles of plastic, metal, and acrylic cement initiate a foreign-body reaction and result in the formation of granulation tissue, including macrophages and foreign-body giant cells. Typical features of tissue reactions exist for each of the materials from which prostheses are made. The consequent formation of scar tissue produces a thickening of the capsule, which, in turn, may cause a reduction in the mobility of the joint. In small amounts, the foreign-body particles are eliminated via the perivascular lymph spaces. Where this transport system is insufficient to handle the volume, however, the foreign-body response may extend to the whole environment surrounding the joint. In such cases, there may be loosening of the cemented prosthetic parts because of deterioration of contiguous bone anchors by the tissue membrane lining the bone cement.

On skeletal muscle pathology in trichinosis.

A study was carried out on the pathology of trichinosis in human muscle biopsy sample and comparatively in experimental trichinosis in the rat, by current microscopic techniques and Troitsky's method of block silver impregnation of the nerve endings. Stress is laid upon the incipient changes in the parasitized muscle fibers, the sarcolemmic origin of the parasite capsule, the myogenic nature of certain periparasitary but intracapsular clear cells of precocious appearance and other associated, non-specific lesions, such as common myositis changes or giant myogenic foreign-body cells. The intimate relationships between parasitized muscle fibres, cellular interstitial reactions, local new-growth vessels and proliferation of the nerve endings are also described. The morpho-functional significance of such morphological synergic manifestations is discussed as an adaptation to the new local conditions brought about by the appearance of the peculiar muscle fibre-parasite unit in trichinosis.

Giant-cell hyalin angiopathy.

Seven examples of an unusual hyalin degenerative change in the walls of blood vessels are discussed. We believe this is a localized phenomenon, not connected to systemic vascular disease. Inflammation is usually present, and in each case a dental procedure had previously been performed. Dental extraction was the most common procedure. Giant cells of the foreign-body type are present, but particulate foreign material was seen in only one case.

An ultrastructural study of nuclear and centriolar configurations in multinucleated giant cells.

Multinucleated giant cells were examined with electron microscopy from (1) four peripheral giant cell granulomas of the jaws, (2) a central giant cell tumor of the maxilla, (3) five giant cell tumors of tendon sheath, (4) experimentally induced foreign body granulomas in rats, (5) a virus-induced sarcoma, and (6) osteoclasts from the mandibles of rats and (7) the femurs of hamsters. The purpose of the study was (1) to examine the location and arrangement of the nuclei and the centrioles of the multinuclueated cells and compare them with the osteoclast which has previously been thought to have a unique arrangement and (2) to observe whether the physical location of the centrioles and the nuclei were such that the nuclei could undergo mitosis within the cytoplasm. All of the multinucleated cells were found to have a similar arragnement of nuclear concentration areas and nucleus-free areas. A giant centrosphere containing multiple pairs of centrioles was found in the nucleus-free areas, unassociated with any particular nucleus. In the case of the foreign body giant cell and the osteoclast, this giant centrosphere was located very close to the foreign material or bone. When occasional single pairs of centrioles were found, they were located in the area of nuclear concentration, closely associated with a particular nucleus at the periphery. These findings have shown that a common centrosphere containing multiple centrioles is not an exclusive feature of the osteoclast as was previously thought. The findings suggest that a mononuclear cell containing a centriole pair fuses to the larger cell and maintains its centriole pair in close proximity to its nucleus for a short period of time, during which it may undergo mitotic activity. Eventually, the centrioles proceed to a common centrosphere whereas the nuclei aggregate in another area making further mitotic activity an unlikely possibility.

Foreign-body tumorigenesis in mice: DNA synthesis in surface-attached cells during preneoplasia.

(CBA/H X CBA/H-T6)F1 mice were given sc implants of unplasticized vinyl chloride acetate 15 X 22-mm copolymer films. The animals were pulsed with [3H]thymidine at various times during the 15 months following implantation. DNA synthesis occurred in the film-attached cell population, predominantly macrophages, throughout the preneoplastic phase in both females and males. Giant cells with fewer than 10 nuclei were labeled synchronously and asynchronously. No DNA synthesis was detected in giant cells with more than 10 nuclei. Previous studies have shown that phagocytic inactivity and ultrastructural signs of functional dormancy are characteristic for the foreign-body-reactive macrophage. However, this investigation demonstrated that the macrophage was not dormant with respect to DNA synthesis.

The tissue reaction to implanted foreign bodies in levan-treated mice.

The effect of local administration of levan on the reaction to foreign bodies was studied in the subcutaneous tissues of mice implanted with sterile cotton wool pellets or Melenex plastic coverslips. Treatment with levan reduced leucocytic invasion of the implanted pellets as well as the vasoproliferative response of the surrounding tissue. Reduction in the thickness of the capsule surrounding the implant in levan-treated mice provided further evidence for the latter. Assessment of cells attached onto implanted coverslips showed that levan treatment diminished the number of macrophages attaching to the plastic and significantly reduced the number of nuclei within the multinucleate giant cells formed at the site of implantation. This reduction indicates that levan also exerts an inhibitory action on fusion between macrophages.

Human tissue reaction to sutures.

Tissue reaction to sutures was studied in 666 specimens obtained from patients from one day to 23 years after operation. Reaction to catgut depends on the stage of absorption and is mainly histocytic in type. Nonabsorbable sutures are encapsulated by a rim of connective tissue, while near the suture histocytes, giant cells and lymphocytes are found. This is most marked with silk and cotton, less so with Dacron, and least with nylon and wire. Tissue infiltration into the suture interstics is variable. The reactions resemble the findings in experimental animals.

Asteroid bodies: derivatives of the cytosphere. An electron microscopic contribution to the pathology of the cytocentre.

An electron microscopic study of asteroid bodies in granulomatous diseases of man, in particular in sarcoidosis and foreign body reactions, has led to the following conclusions. 1. Asteroid cytoplasmic inclusions are composed of organic proteineous structures. The body and arms of the stars consist mainly of longitudinally orientated, partially helically wound, collagen free, microfilaments with a diameter of approximately 50 A. Microtubules participate in their composition to a lesser degree. The filaments intertwine in various directions within the corpus of the stars. 2. Deeply osmiophilic compact structures, considered to represent condensation forms of protein, are consistently present within the body of the stars. These structures are partially granular and partially ring-shaped; in the latter case, transverse sections of tubules are noted. Since it is disputed whether they constitute procentrioles or postcentrioles, they are best referred to as paracentrioles. 3. The asteroid bodies are derivatives of the cytosphere, thus relating, in the broad sense, to the microfilamentous and microtubular apparatus of the cell. 4. The asteroid bodies in multinucleated giant cells participate in the spatial compartmentalization of the cytoplasmic content and nuclei. The sphere, aided by the centrioles, induces the formation of the mitotic spindle and, in addition, function as an organizing system of the cell. 5. The light microscopical periastral clear zone does not correspond to a fluid space. Rather, it consists of closely packed accumulations of laminated and whorled myelin figures, which are derived from the degradation of lipoid substances.

Study of encapsulation of silicone rubber implants in animals. A foreign-body reaction.

It has long been known that the formation of a fibrous capsule around an implant is a general phenomenon in nature, an inevitable result of the tissue defense mechanism called the foreign body reaction. We have investigated this reaction in animals and find it consists of a series of interrelated processes in which the final result may vary, depending on the susceptibility of the foreign material to phagocytosis, incorporation by giant cells, or isolation by fibrosis. This susceptibility depends, in turn, on the physical and chemical properties of the implant. The process of capsule formation, as well as the structure of the final capsule, is similar in animal models and in humans--so that investigations of human encapsulation may be done in animals. The cause of abnormal induration around human breast implants is still unknown, and the question of whether normal induration in the animal model can be used to elucidate abnormal clinical induration depends on further investigation of both phenomena.

The locomotory behaviour of the multinucleate giant cells of foreign body reactions.

Cinemicrographic observations have shown that multinucleate giant cells are motile but the degree of translocational movement decreases with increasing size. Generally these polykaryocytes are slower than macrophages exhibiting a speed translocation of 0-3-1-0 micron/min for small polykaryocytes (2-4 nuclei) to 0-04 micron/min for large polykaryocytes (greater than 15 nuclei). Such behaviour would tend to maintain these cells near the site of their formation. The direction of translocation of small syncytia, correlates with the direction of the single leading lamellipodium. Slightly larger cells (5-15 nuclei) show loss of polarity with multiple lamellipodia, none of which is dominant, while the very large syncytia possess a single, large, circumferential lamellipodium whose movement leads to little displacement. These latter polykarya also display reduced contact inhibition which may favour fusion phenomena. Glycolysis appears to be the main source of energy for cellular movement which is also dependent on physiological calcium concentration and an intact microtubular-microfilamentous system. Apposition to the substratum is not complete, occurring in some instances by means of flaps and microvilli which may also be involved in cellular locomotion. Loss of glycocalyceal material probably occurs during locomotion.

Amalgam tattoos: light and electron microscopy and electron-probe micro-analysis.

A common tattoo occurring in the mouth is caused by the insertion of amalgam filling material into the soft tissues. Fifteen amalgam tattoos were examined. Amalgam or its derivatives were found within macrophages, fibroblasts and multinuclear giant cells. Fine particles were found associated with: the basement-membranes of mucosal epithelium, of striped muscle fibres, and of muscle cells of blood vessels; collagen; elastic tissue; and the connective tissue of nerves. Amalgam consists mainly of mercury, silver and tin. The analytical results suggest that corrosion occurs and that mercury and some tin are lost from the tattoos, leaving silver and tin in macrophages and giant cells, and silver in fibroblasts and in the above-mentioned extracellular sites.

Alveolitis due to hair-spray. Ultrastructural observations in two patients and the results of experimental investigations.

Observation of two patients with hair-spray induced lung disease have prompted us to study the ultrastructure of the lung lesion. We have compared the results with experimental lesions in animals injected with hair-spray extracts and with human monocyte cell cultures exposed to hair-spray. The lungs show a chronic alveolitis with a striking granulomatous reaction including macrophages and multinucleated giant cells of the foreign body type. The intraalveolar and interstitial macrophages and the giant cells all contain PAS-positive material. Ultrastructurally distinct lamellar inclusions are found in the secondary lysosomes of the macrophages and giant cells. Identical structures can be produced in animals injected with hair-spray extracts and with polyvinyl-pyrrolidone and -acetate (PVP/PVA), which are regular ingredients of hair-sprays. Large, presumeably polymeric particles (PVP/PVA) are ingested by giant cells. This "gigantophagocytosis" is associated with the fusion of mononuclear phagocytes and leads to the genesis of giant cells. In cell cultures of human blood monocytes hair-spray extracts and PVP/PVA induce maturation and aggregation of these cells, with PAS-positive cytoplasmatic inclusions. The development of multinuclear giant cells in these monocyte cell cultures is also seen. These observations suggest that hair-spray induced lung disease is caused by the prolonged and extensive body response of the local mononuclear phagocyte system (MPS). Overstimulation of the MPS leads to a quantitative and qualitative change which is followed by a partial blockade of this system. The alveolitis is a consequence of the foreign body response to inhaled hair-spray substances.

Morphological studies in tissues surrounding alloarthroplastic joints.

Histological, histochemical and ultrastructural studies were done on soft tissue surrounding alloarthroplastic joints. In 38 cases a prosthesis of the hip joint and in 2 cases of the knee had to be exchanged and replaced. In most of the cases the reoperation became necessary because the anchoring of the prosthetic parts in the bone loosened. Up to 18 months after the first operation infection was responsible for the malfunctioning in some cases. Other complications were luxation and material faults. The morphological changes are determined by the tissue reaction to the different alloplastic materials used and by the time interval they remained in the organism. The large polymerized acrylic cement particles are phagozytosed by multinucleated foreign body giant cells. About 12 months following the implantation of the artificial joints small double refractile particles appear and evoke characteristic morphological changes. The particles are abraded by the continuous friction of the moving alloplastic or metallic surfaces of the prostheses. Usually they are phagozytosed by histiocytes, which form large granulomas and undergo degenerative changes as is indicated by the ultrastructural and histochemical findings. These alterations are more pronounced and occur sooner in prosthesis with parts (rotation ball or cup.) fabricated by polyester than in those made by polyethylene. The abraded particles not only are transported to the inguinal lymphnodes, but also to the tissue between prostheses and bone, where they induce the same morphological changes as in the capsule. Hence the fibrous membrane separating bone and prostheses increases in width, and the spongy bone is partially destroyed by the proliferating histiocytes. It is assumed that by impairing the anchoring this foreign body reaction to the abraded alloplastic particles is the leading cause of the loosening of this kind of artificial joints.

Critical evaluation of prosthetic materials in repair of abdominal wall hernias: new criteria of tolerance and resistance.

Six different materials (3 mesh and 3 cloth) commonly used for the repair of abdominal wall hernias were evaluated in 180 rats after implantation through musculature and peritoneum. Serial macroscopic and bacteriologic investigations were done, and bursting strength of the wound was determined in all groups at intervals with an original device. Histologic criteria were used to characterize the resistance of the wound and the tolerance of the host to the foreign material. Statistical analysis of the results demonstrated: (1) after postoperative day 15, the resistance of the wound was similar for each material tested; (2) during this early period mesh materials exhibited more resistance to bursting pressures than cloth materials; (3) the incorporation of mesh material was constant, whereas encystment or extrusion was always observed after implantation of cloth material; (4) no infection occurred with mesh material, but significant bacteria were found in 18 per cent of cloth material implantations; (5) the extent of the cellular reaction, the enumeration of giant, inflammatory, and fibroblastic cells showed the superiority of mesh materials; and (6) the ratio of fibroblasts to inflammatory cells reflected closely the mechanical resistance and tolerance of the foreign material.

[Histological findings after application of Kveim antigen in animals (author's transl)].

The frequence of local granulomatous changes after injection of kveim antigen and previous sensibilisation (Freund's adjuvant, BCG-vaccine, atypical mycobacteria, implantation of sarcoidotic lymph nodes, human-gamma-globulin of sarcoidotic patients) was investigated in 69 mice and 38 guinea pigs. Granulomata with epitheloid and giant cells, resembling a "positive Kveim reaction" were found in 26% of the mice and 18% of the guinea pigs. Doubtful positive results were observed in 7% of the mice and 5% of the guinea pigs. A high percentage of non-specific foreign body reactions was observed in guinea pigs (82%), relatively scarcely in mice (16%). The most effective method for sensibilisation of the animals is the administration of Mycobacterium avium to guinea pigs or of Kveim antigen with complete Freund's adjuvant and the intraabdominal implantation of sarcoidotic lymph node material to mice.
