There is an annual statistical evaluation of documentation, done since 1971, of an early disease diagnosis programme for children. At first examination of neonates in the years 1977-1981, sex-specific difference was low as regards data on pregnancies at risk, but definite and constant differences were recorded as regards psychic and social stress during pregnancy. Pregnancies at risk without any special features at birth were registered in 12.5%. In 58.4% of newborns no special features with regard to pregnancy or birth were recorded. In contrast to all other markers, final pelvic position was more frequent for girls than boys. The proportion of operative obstetric procedures has slowly fallen from 1977 to 1981. Cesarean section has recently been practised in 12.4%. With regard to birth weight of children, the documentation revealed a difference from the Federal Statistical Office, which showed a higher number of low-birth weight new-borns. Comparing the results with those of other German or foreign statistics, there is a strikingly high frequency of cesarean section in the Federal Republic of Germany.
For a number of anatomical and pathological reasons, reconstructive surgery of the oesophagus remains a difficult and complex surgical procedure. The vast experience of performing 1155 reconstructive surgical procedures at Prva hirurska klinika (First surgical clinic), part of Belgrade Medical Faculty, enables the author to reflect upon options and limitations of such operations. There is a considerable discrepancy between the results obtained from reconstructive surgery of the oesophagus and the cardia affected by malignant growths, and form treatment of benign strictures of the oesophagus--the latter producing significantly better results. Digestive tract tissue used in the reconstructive surgery, is applicable depending on the pathological condition, its location, the possibility of isolating a sufficiently long section, and its adequate vascularization. Best results were achieved when a segment of thejejunum was used, but only for distal lesions of the oesophagus. Good results were achieved also when segments of the colon were used, the left colon proving more useful than the right one. Stomach tissue produced the worst results in a series of reconstructions, because it was used primarily in cases where the oesophagus was affected by malignant growths. When comparing the data from this series with the findings from the world literature, we come to almost identical results as in cases of reconstructive surgery of the oesophagus affected with carcinoma. Our experience in reconstructive surgery treatment of benign lesions of the oesophagus produced consistently better results that those shown in reputable foreign statistics.
Analysis of Moscow cancer register demonstrates a steady growth of colon cancer (CC) incidence and mortality in Moscow, also noted in all large industrial cities of economically developed countries. In Moscow between 1996 and 2001, the incidence of CC had grown 1.5 times (from 19.6 up to 30% per 100,000), and the CC-related mortality had grown twice (from 9.1 up to 19.5% per 100,000). The incidence of CC among women is 1.5 to 2 times higher than that among men. The burst of CC incidence is noted after age 55, and the peak incidence is noted at age 65. Not rare (2.9% of cases) are tumors that develop simultaneously in different divisions of the colon, which are diagnosed during the surgery or are omitted even then. Such synchronous large bowel tumors are found mostly in patients over 60. The analysis show that metachronous tumors (3.4% of cases) usually develop in the colon 8 to 10 years after the first surgery for CC, or some other cancer, and are found mostly in patients over 70. Unlike foreign statistics on multicentric CC cancer, according to which primary-multiple (synchronous and metachronous) cancers occur in 1.24 to 14% of cases, Moscow statistics demonstrate a lower likelihood of such a pathology in Muscovites. To sum up, patients who underwent treatment for CC or any other cancer should keep being under an oncologist's observation due to the likelihood of the development of a metachronous colon tumor 8 to 10 years later.
After reviewing the literature on transcultural psychiatry, the admissions in the closed service of the Hospital Stuyvenberg in Antwerpen were analysed for 1967-1975. Immigrants were compared with other nationals, for age, sex and psychiatric diagnosis, with regard to patients born in Belgium. For the foreigners, statistical evaluation was done for age, sex, civil status, land of origin, and jewish religion. Interesting results were obtained for paranoid syndromes in patients originating from Eastern Europe and in menopaused women.
Twenty-six brain-metastases in black patients, appearing for most of them as primitive, were observed during a period of 11 years in the Neurological Clinic of the Fann U.H.C. in Dakar. They represent 13.9% of brain-tumours and 11.81% of intracranial space occupying lesions. Their etiology is very diversified: lung cancer (4, 15.4%); breast (0); liver (3, 11.5%); digestive tract (3, 11.5%); prostate gland (1); ovaries (1); parotid gland (1); thyroid gland (2); lympho-reticulosarcoma (2); melano-sarcoma (1); chorio carcinoma (3); undetermined (5, 11.5%). This series enables us to appreciate differences with African statistics, which are not numerous and with other foreign statistics. The difference of division of cancer in Africa, particularly in Senegal, explains some results such as the relative high frequency of liver-cancer in the etiology of this metastasis. But it is surprising to learn that lung-cancer is frequent and breast-cancer absent. Also to be marked is the non-negligible influence of the choriocarcinoma metastasis. Progress in approaching diagnosis, by developing scintigraphy, might facilitate extraction of solitary metastasis more often.
