[Meatoplasty with allogenic preserved cartilage. Histomorphological model in animal experiments (author's transl)].

The reconstruction of the posterior wall of the external acoustic meatus destroyed by surgery or a cholesteatoma is necessary for the complete restoration of function. For this purpose, allogen cartilage preserved in thiomersalate has been transplanted on the anterior wall of the air-containing mastoid bone in 160 clinical cases. The histomorphological reaction of the recipient to unilaterally uncovered transplant material has previously been unknown. In 10 guinea pigs, 15 fenestrated bullae (hypotympanum equivalents) were closed with cartilage transplants fixed in place with tissue adhesive. After a period varying between 7 1/2 and 11 months, the walls of the bullae with the transplant were then examined histomorphologically. Despite the variations in the period in situ, the avital cartilage transplant, which remained largely constant in volume and shape, was always to be found. It was tolerated without reaction and was "enveloped"--also on the primarily uncovered surface--by a capsule of bone and connective tissue. In no case was rejection observed. Marked foreign-body reaction was, however, found in the vicinity of the tissue adhesive, which was absorbed only slowly. The clinically well-known tissue-compatible and complication free behaviour of the allogenic cartilage transplant preserved in thiomersalate, is thus explained by the histomorphological findings. In contrast, the fact that marked foreign-body reaction was demonstrable even after almost one year, suggests that tissue adhesive should not be used.

Human tissue reaction to sutures.

Tissue reaction to sutures was studied in 666 specimens obtained from patients from one day to 23 years after operation. Reaction to catgut depends on the stage of absorption and is mainly histocytic in type. Nonabsorbable sutures are encapsulated by a rim of connective tissue, while near the suture histocytes, giant cells and lymphocytes are found. This is most marked with silk and cotton, less so with Dacron, and least with nylon and wire. Tissue infiltration into the suture interstics is variable. The reactions resemble the findings in experimental animals.

Absorption

Chronic vascular reactions to steel coil occlusion devices.

Original Gianturco steel coil vascular embolization devices and two modifications were compared for their occlusive effects and for the degree of adverse reaction in the surrounding vessel wall. Modifications consisted of silk or Ivalon replacing the wool strands of the original device. Single coils were placed into the left renal artery of nine mixed breed swine and restudied 3 months later. The bare coils did not occlude the vessels nor lead to significant foreign body reaction. Wool, silk, and Ivalon coils produced prompt and sustained renal artery occlusion. Upon histologic examination of the vessel wall, foreign body and chronic inflammatory reaction was found to be strongest against wool. This reaction was less with silk and markedly diminished with Ivalon. The results indicate that silk and Ivalon coils, while having the same occlusive effect, are preferable to wool coils if less local reaction is desired.

Morphological studies in tissues surrounding alloarthroplastic joints.

Histological, histochemical and ultrastructural studies were done on soft tissue surrounding alloarthroplastic joints. In 38 cases a prosthesis of the hip joint and in 2 cases of the knee had to be exchanged and replaced. In most of the cases the reoperation became necessary because the anchoring of the prosthetic parts in the bone loosened. Up to 18 months after the first operation infection was responsible for the malfunctioning in some cases. Other complications were luxation and material faults. The morphological changes are determined by the tissue reaction to the different alloplastic materials used and by the time interval they remained in the organism. The large polymerized acrylic cement particles are phagozytosed by multinucleated foreign body giant cells. About 12 months following the implantation of the artificial joints small double refractile particles appear and evoke characteristic morphological changes. The particles are abraded by the continuous friction of the moving alloplastic or metallic surfaces of the prostheses. Usually they are phagozytosed by histiocytes, which form large granulomas and undergo degenerative changes as is indicated by the ultrastructural and histochemical findings. These alterations are more pronounced and occur sooner in prosthesis with parts (rotation ball or cup.) fabricated by polyester than in those made by polyethylene. The abraded particles not only are transported to the inguinal lymphnodes, but also to the tissue between prostheses and bone, where they induce the same morphological changes as in the capsule. Hence the fibrous membrane separating bone and prostheses increases in width, and the spongy bone is partially destroyed by the proliferating histiocytes. It is assumed that by impairing the anchoring this foreign body reaction to the abraded alloplastic particles is the leading cause of the loosening of this kind of artificial joints.

Endometrial morphology in women exposed to uterine systems releasing progesterone.

A blind study was done in 402 endometrial biopsies of women who had intrauterine devices releasing different daily amounts of progesterone or a placebo (empty device): 175 were obtained in what presumably was the proliferative phase and 227 in the secretory stage, as all these subjects were normal, healthy women with a history of previous fertility. With all dose levels of the progesterone-releasing devices there was variation of the endometrium general pattern and the over-all picture varied from normal secretory to suppressed endometrium. In addition to these changes of the endometrial pattern, in 231 specimens there was significant inflammatory infiltration and in six cases even plasma cells were seen. Predecidual reaction was frequently seen and in 45 cases it was diffuse and marked. The significance of these data is discussed on the grounds of the frequent similarities of the changes here reported with those in women using combined oral steroids for contraception.

[Glycolic acid/polymer as suture material].

Tissue reactions to polyglycol acid and plain catgut threads were compared. The second part of the paper deals with changes in the mechanical properties of the polymer. Their reduction after insertion in tissues was proportionally equal for threads of different sizes. This finding is explicable histologically, since it can be shown that cell and humoral attack of the single filaments is homogenous throughout the entire cross-section of the thread.

Abdominal Muscles

Giant-cell hyalin angiopathy.

Seven examples of an unusual hyalin degenerative change in the walls of blood vessels are discussed. We believe this is a localized phenomenon, not connected to systemic vascular disease. Inflammation is usually present, and in each case a dental procedure had previously been performed. Dental extraction was the most common procedure. Giant cells of the foreign-body type are present, but particulate foreign material was seen in only one case.

Adolescent

[Light- and electronmicroscopic findings in nine auto-alloplastic vascular grafts (Sparks-Mandril) (author's transl)].

Auto-alloplastic arterial prostheses with a dacron mesh of 1 or 2 layers according to Sparks were investigated by light and electron microscopy after variable times of incorporation and perfusion. At the end of the organization-period the characteristic structure of the wall is composed of three layers. The interior layer is adapted to the form of the mandril, which holds the place of the lumen, the medial layer is formed according to the inforcing dacron-mesh. With increasing duration of incorporation the granulation tissue shows somewhat less inflammatory cells and capillaries and more collagen fibres. There were some characteristic disturbances of healing only partly due to infection, which resulted in focal or total necrosis of the granulation tissue: Not all of these findings were of clinical importance. Already after a perfusion time of 10 months an atherosclerosis of variable degree was found. This atherosclerosis started from mural thrombi and showed in the electron microscope proliferations of modified smooth muscle cells. Advantages and disadvantages of the autoalloplastic arterial prostheses are discussed.

Arterial Occlusive Diseases

Healing of periodontal flaps following use of MBR 4197 (flucrylate) in rhesus monkeys. A clinical and histological evaluation.

Experimental periodontal surgery was performed on six adult Rhesus monkeys, using the modified Widman flap technique. Each monkey provided four or six segments for flap surgery, and three different experimental time intervals were used. Flaps were immobilized after surgery by conventional sutures or by application of MBR 4197 cyanoacrylate spray. Each animal provided its own contralateral comparison, and the mode of flap immobilization was reversed in the corresponding segments in the other animal of each experimental group. One monkey died approximately 12 hours after surgery, and the other animals were sacrificed at 14, 35 and 180 days. Histological sections were prepared from all operated areas, and examined for the pattern of healing. A special staining technique was used to determine the presence of MBR 4197 within the tissues.

Schistosomiasis--cancer: etiological considerations. A review.

Evidence for a causal connection between Schistosoma haematobium-infection and carcinoma of the urinary bladder is discussed. A group relationship of schistosomiasis cancer to cancers as+sociated with asbestosis, foreign body implants, and cicatrization is suggested on the basis of several criteria. Results of experimental foreign body tmuorigenesis in mice are presented and elaborated in relationship to schistosomiasis cancer. Carcinogenic development at the cellular level is discussed with emphasis on the essential role of tissue-environmental conditions, especially fibrotic changes and macrophage quiescence.
