Histopathologic study of changes occurring in eyes with intraocular lens implantation: autopsy eyes, enucleated eyes and corneal buttons.

The clinicopathologic findings in 17 eyes which were surgically enucleated or removed at autopsy as well as the histopathology of 72 corneal buttons removed from eyes with intraocular lenses are described. In four of five autopsy eyes the intraocular lens was well tolerated; however, in all cases, changes related to the pseudophakos were found. In seven of 12 surgically enucleated eyes, postsurgical bacterial or fungal endophthalmitis had developed. On histopathological examination, one eye revealed clinically unsuspected endophthalmitis phacoanaphylactica after extracapsular cataract extraction and intraocular lens implantation. In two cases, choroidal melanoma became evident after cataract extraction and insertion of a pseudophakos. In both cases preoperative ultrasonography was limited to A-scan axial measurements. The 72 corneal buttons were removed from patients with pseudophakic bullous keratopathy; histologically the most striking findings were guttata-like changes in 50% of the specimens and retrocorneal membranes in 13.9% of the specimens.

Influence of mercurial sulfhydryl agents on aqueous outflow pathways in enucleated eyes.

Enucleated calf and primate eyes were perfused with either p-chloromercuribenzene sulfonate (PCMBS) or p-chloromercuribenzene (PCMB). Both caused a decrease in aqueous outflow that did not result from inhibition of glycolysis in trabecular cells. Morphologic studies of PCMBS-treated primate eyes suggested that the reduction in aqueous outflow occurred as the result of cellular swelling in the trabecular meshwork and, particularly, in the juxtacanalicular connective tissue. These findings suggest that changes in cell volume may influence trabecular outflow resistance and that mercurial-sensitive membrane sulfhydryl groups could participate in the regulation of aqueous outflow.

The expression of cyclooxygenase 2 in retinoblastoma: primary enucleated eyes and enucleation after conservative treatment.

PURPOSE: To provide insight into the relationship between cyclooxygenase-2 (COX-2) expression and histopathologic features of retinoblastoma specimens treated either by primary or secondary enucleation. DESIGN: Laboratory investigation. METHODS: Twenty-five retinoblastoma specimens received between 1994 and 2003 were retrieved for this study from the Ocular Pathology Registry, Federal University of São Paulo, Brazil and the Henry C. Witelson Ocular Pathology Laboratory and Registry, McGill University, Montreal, Canada. The specimens retrieved were divided into two groups: Group I, enucleation was performed as a form of primary treatment (n = 15) and Group II, enucleation after failure of conservative treatment (n = 10). Patient information and formalin-fixed, paraffin-embedded specimens were obtained. New sections of these blocks were used for hematoxylin and eosin stain and for immunoassay using a monoclonal mouse anti-COX-2 antibody. Two independent ophthalmic pathologists reviewed all of the microslides. RESULTS: Twenty-three specimens (92%) presented a high expression of COX-2 (15 in Group I; eight in Group II) and there was no statistical difference in COX-2 expression between the two groups (P = .07). However, all specimens expressed COX-2 to a different degree. The areas of tumor invasion were positive for COX-2 in 87.5% of the two groups. CONCLUSION: Most retinoblastoma specimens revealed a high COX-2 expression. Future studies will be necessary to correlate the high expression of COX-2 in retinoblastoma and a possible applicability of anti-COX-2 medications in the treatment of these tumors.

Evaluation of eyes enucleated for scleritis.

A series of 30 enucleated eyes, all of which had a primary histological diagnosis of scleritis, was analysed. The average age of patients at enucleation was 68 years; many of these patients had had the disease for more than 30 years. In 40 per cent the diagnosis of scleritis was unsuspected and was often masked by multiple complications. Scleritis with uveitis and glaucoma was the most common combination to come to enucleation. In 82 per cent, pain was the reason for enucleation, which suggests that some patients were on inadequate levels of steroid treatment or were unable to tolerate them. In the series, 37 per cent of the eyes perforated. Perforation occurred in those patients who were having steriods but not in those who were not, but the data are inconclusive as to which method of steroid administration was most likely to cause ocular perforation.

Transneuronal changes in the optic tectum following eye enucleation in the newborn rat.

Following unilateral eye enucleation, degenerated neurons were observed in the affected tectum. However, dead cells also were present in the ipsilateral tectum, i.e., on the side which does not receive input from the enucleated eye. Further, the degeneration pattern observed in the experimental animals was similar to that observed in control unoperated animals. From these results we concluded that within the period studied, eye enucleation does not cause cell death to the neurons onto which the optic tract projects. Rather, the dead cells observed in the tectum are due to normal cell death that occurs in embryological development. The reduction in volume that occurs to the affected tectum following eye enucleation is attributed to an absence of the optic tract rather than to a neuronal loss.

Appearance of substance P-like immunoreactive neurons in the rat superior colliculus after neonatal eye enucleation.

The possibility that eye enucleation within one day after birth has an effect on the postnatal development of substance P (SP)-like-immunoreactive (SP-I) structures in the superior colliculus (SC) was investigated in the rat. Results were compared with those in animals enucleated at postnatal day 15. All the animals were allowed to survive until postnatal day 90, after which changes in SP-I neurons and fibers were identified immunohistochemically. In colchicine-treated rats, the most remarkable changes occurred in SP-I neurons following eye enucleation at birth; large numbers of SP-I neurons appeared in the ventral part of the stratum griseum superficiale (SGS), stratum opticum (SO) and stratum griseum intermediale (SGI) of the deafferentated SC. SP-I neurons did not appear in these layers, when deafferentation of the SC was carried out in rats at postnatal day 15. These findings suggest strongly that eye enucleation at birth affects the production of SP of neurons in the ventral part of the SGS, SO and SGI at the deafferentated SC. The appearance of SP-I neurons in the neonatal eye enucleation may be due, at least partially, to reorganization of another neuronal system in the SC.

A dummy orbit for training in diagnostic procedures and laser surgery with enucleated eyes.

PURPOSE: A dummy orbit for enucleated eyes was developed for training residents in diagnostic and surgical procedures. METHODS: Porcine eyes were used to construct the model. The device is made of black anodized metal. The final prototype was tested for ease of use and optical quality. Principal elements are an adjustable eye support, a cylinder, and a removable ring. RESULTS: The dummy orbit and enucleated eye allow direct and indirect ophthalmoscopy; inspection with contact lenses, including gonioscopy; and retinal laser coagulation using contact lenses. Laser trabeculoplasty, argon laser trabeculotomy, Nd:YAG laser iridectomy, and Nd:YAG laser capsulectomy are possible, as well as training in tonometry and ultrasonography techniques. CONCLUSION: The dummy orbit is a valuable aid for teaching and training in diagnostic and laser surgery procedures with nondissected enucleated eyes.

Justification of the enucleated eye test with eyes of slaughterhouse animals as an alternative to the Draize eye irritation test with rabbits.

The enucleated eye test (EET) with the isolated eye of rabbits has been recognized as a valuable alternative to the Draize test, because it represents a test system nearest to the in vivo test, without the need to use live animals. In this ex vivo bioassay, three parameters are measured to detect possible adverse eye effects, namely corneal swelling, corneal opacity and fluorescein retention. The measurement of corneal swelling in this assay guarantees a highly objective and discriminative parameter. In combination with the detailed observation of corneal opacity and fluorescein retention, a reliable evaluation of the eye irritation potential of test materials is achieved. However, laboratory animals are still necessary as eye donors. The use of slaughter animals, such as the cow, the pig and the chicken, as possible as eye donors for the EET was therefore examined. From these candidates, the chicken appeared to be the most practicable. 21 reference compounds, ranging from non-irritant to severe irritant, which had been tested previously in a validation study on alternative test methods for eye irritation testing, sponsored by the Commission of the European Communities, were examined in the Chicken Enucleated Eye Test (CEET). When compared with the in vivo EC classification, the CEET correctly classified each of the compounds that must be labelled in the EC as irritant (R36) or severely irritant (R41). In addition, since the CEET recognizes three levels of irritancy rather than two (as in the case of the EC classification) a small number of the compounds were recognized as slightly irritant, which according to the EC classification need not be labelled. It was concluded that this ex vivo test system is highly accurate in the assessment of eye irritation potential without the use of laboratory animals.

Serum obstruction of aqueous outflow in enucleated eyes.

We perfused enucleated human eyes via the anterior chamber by the constant pressure technique. Infusion of human serum into the anterior chamber of enucleated human eyes for 30 minutes at 23 mm Hg pressure induced a 42% decrease in facility of outflow, which was not relieved by irrigation of the anterior chamber with balanced salt solution or alpha-chymotrypsin. Diluted serum also reduced the facility of outflow. Measured in a glass viscometer, diluted serum had less viscosity than undiluted, but interfered with outflow from the eye more than anticipated on the basis of viscosity alone. When we used lens depression to induce tension on the iridocorneal angle to simulate the effects of contraction of the ciliary muscle, outflow facility increased in control eyes that had not been exposed to serum and in serum-perfused eyes. However, the partial obstruction to outflow that had been induced by serum persisted. Normal serum components may become adsorbed or entrapped in the aqueous outflow system so as to obstruct outflow, and this may result in secondary glaucoma in eyes with chronic uveitis.

The chicken enucleated eye test (CEET): a practical (pre)screen for the assessment of eye irritation/corrosion potential of test materials.

The enucleated eye test with chicken eyes (CEET) obtained from an abattoir proved to be a valuable and practical alternative for the 'traditional' enucleated eye test with eyes of laboratory rabbits. Since 1992, the CEET has been incorporated in standard contract toxicity testing at the Toxicology Division of the TNO Nutrition and Food Research Institute as a (pre)screen for the Draize eye test with rabbits. The results of the first 44 compounds tested showed excellent correlation with the in vivo results. The CEET identified non-irritating or severely irritating compounds, and predicted (slightly to moderately) irritating compounds. Statistical analysis of the CEET and the rabbit in vivo scores showed high linear correlations between the critical values of both tests and confirmed the relevance of this assay with respect to ocular effects. In general, tiered in vitro/in vivo testing was considered a meaningful approach for further validation of alternative methods and for reducing the use of suffering of laboratory animals to a minimum. Tiered testing of compounds in cases of eye irritation hazard assessment should be incorporated in the legislation of the European Community.

[Morphogenesis of Mauthner neurons of Xenopus laevis tadpoles following early unilateral eye enucleation].

The connection between Mauthner neuron (CMN) and visual analyzer in Xenopus laevis tadpoles was studied using these neurons capacity to change their morphogenesis during the development in case of interruption of their connections with afferent fibres. In phase 31 of development tadpoles were subjected to unilateral enucleation of eye. Morphogenesis of contralateral (experimental) and ipsilateral (control) MN with respect to the operated eye was followed up. Comparison with intact Mauthner neuron of the tadpole of the same age who were not exposed to operation was conducted. Contralateral MN were found to be delayed in development phases 47 and 50. This was associated with 1.5-3.3 fold difference in neuron body central section area and 2-3 fold differences in nuclear section area. Three-dimensional reconstruction revealed lower number of cell processes in contralateral MN as well as decline in their length and diameter. Control and intact MN showed no difference similar to experimental and control ones in phase 41. The data obtained indicate that MN of Xenopus laevis tadpole possess powerful ipsilateral connections originating from the side of visual analyzer and contralateral ones from the side of the eye. This affects MN development and in contrast to octavolateral system (including vestibular apparatus and organs of lateral line) controls the contralateral neuron.

New tumours in non-enucleated eyes of bilateral retinoblastoma patients.

Forty non-enucleated eyes with bilateral retinoblastoma which were treated by external beam radiation therapy (EBRT), transconjunctival cryopexy, and photocoagulation were retrospectively analysed for the age of onset and location of new intraocular tumours. Of these 40 eyes, 9 (22.5%) eyes developed 17 new tumour foci over a mean follow-up of 3 years. The risk of new tumour formation was age-related being 47% in children with age at onset of retinoblastoma less than 1 year compared to 4.4% in older children (P < 0.001). Four eyes (44%) had 2 episodes of tumour formation. All tumour foci developed within 11 months of initial treatment at an average episode interval of 4.0 months. In 89% of cases, new lesions ceased to occur by 18 months of age. Our study clearly shows that EBRT did not prevent development of new lesions. The tumour islands which developed in the peripheral retina in 88% of cases were successfully treated with transconjunctival cryopexy. In 8 cases (89%), the eye could be salvaged. All young bilateral retinoblastoma patients should undergo frequent periodic detailed examination of the retinal periphery with 360 degrees scleral depression to pick up new tumour lesion early and to treat them effectively with simple globe saving methods.

Pathological findings in eyes enucleated because of perforating injury.

Forty-eight eyes enucleated after a perforating eye injury (PEI) in 1980-86 (15% of all PEIs) were examined. The time between injury and enucleation varied from 0 to 1145 days. The lens had been damaged or lost in all eyes and 94% had an uveal tract injury. In 83% the retina was detached, in 15% it was totally prolapsed in injury. The inflammatory signs were most marked in eyes with a corneoscleral or double perforation. Proliferation of the retinal pigment epithelial cells or fibrous proliferation from the wound or ciliary body was found 9-10 days after trauma and epiretinal membranes from the optic nerve head or from the surface of the retina after 1 month. Massive fibrous proliferation was seen in 94% of eyes enucleated 1 month or later after injury. One case of post-traumatic purulent endophthalmitis was detected, but signs of sympathetic ophthalmia were not observed.

Contrast letter thresholds in the non-affected eye of strabismic and unilateral eye enucleated subjects.

To investigate the effects of visual disruption on contrast letter thresholds of the non-affected eye, subjects with one eye enucleated, strabismic subjects using the non-deviating eye and normal control subjects were asked to identify letters on eye charts and single letter cards which varied in contrast (between 4 and 96%) and size. At all contrast, contrast letter acuity of eye enucleated subjects was superior to both normal control subjects and strabismic subjects. Early onset strabismic subjects (onset < 24 months) showed inferior performance to normal control subjects at all contrasts of 25% and above. Late onset strabismic subjects showed normal performance at all contrasts, except for high contrast single letters, where performance was inferior to normal control subjects. Further, for all subjects groups, performance on letter charts was similar to performance on single letter cards. We conclude that disruption to the visual system caused by eye enucleation or strabismus is not equivalent. These differences may be due to intrinsic differences between the visual systems of eye enucleated subjects and strabismic subjects and/or to the profound differences in deprivation caused by the two conditions.

Effects of the eye enucleation on the activity of monoamine oxidase and acetylcholinesterase in the superior colliculus of the rat.

Changes in the activity of monoamine oxidase (MAO) and acetylcholinesterase (AChE) following unilateral eye enucleation were measured for the superior colliculus (SC), lateral geniculate body (LGB), and nucleus suprachiasmaticus (NSC) in the rat by means of microspectrophotometry. The most conspicuous change of high significance was the decrease of MAO activity in the superficial strata of the SC contralateral to the enucleated eye. The other layers of SC, such as the stratum opticum, the intermediate strata, the deep strata and the central gray did not show significant postoperative changes in MAO acitivity. MAO activity of the LGB and the NSC was also not affected to a significant degree. The site of decrease of MAO activity in the superficial strata of the SC sould be degenerating optic nerve terminals. The strong AChE activity in the superficial strata may be derived from the horizontal cells. AChE activity in the SC showed only slight postoperative changes. The possible roles of the monoaminergic and cholinergic systems in the process of visual information are discussed.

[Clinical and histopathological examination of enucleated eyes with contusion ruptures of cornea after radial keratotomy].

Histopathological (histological, histochemical, and morphometric) examinations of 8 enucleated eyes with contusion ruptures of the cornea 2-13 years after radial keratotomy showed two patterns of changes caused by contusion and peculiar reparative processes in the keratotomic incisions. A characteristic feature of contusion injury after radial keratotomy is combination of changes typical of contusion and an extensive perforating corneal wound after its rupture along the incisions. Reparative processes in the keratotomic incisions were regarded as incomplete cicatrization resultant from inhibition of keratoblastic activity after a low-traumatic operation. The structure of cicatrice after keratotomic incisions differed from that of corneal cicatrices after cut wounds. The cicatrice consisted of an epithelial plug and stromal par with picrinophilic amorphous filamentous material and individual keratocytes but no proteoglycanes or filamentous matrix. The length of cicatrice was 80-95% of intact cornea thickness. The structure of keratotomic cicatrice implies its insecurity and sensitivity to blunt injury, and therefore patients should be informed of the necessity to avoid injuries.

A comparison of contrast letter thresholds in unilateral eye enucleated subjects and binocular and monocular control subjects.

We previously reported that unilaterally eye enucleated subjects show superior contrast letter acuity to normally sighted monocular viewing control subjects. We suggested that reorganization of the visual system in the enucleated subjects may compensate for their loss of binocularity. Here we measured contrast letter acuity in normally sighted binocular control subjects and compared these results to previously published results of eye enucleated subjects and monocular viewing control subjects. We found equivalent performance between enucleated subjects and binocular control subjects, suggesting that performance of enucleated subjects might be due to some form of neural summation.
