[An analysis of X-ray diagnostic equipment at Moscow prevention and patient-care facilities].

The paper contains the results of an analytical study held, 2001-2003, at prevention and patient-care facilities of Moscow and dedicated to classification of their X-ray diagnostic equipment (XDE) by models. On the basis of an analysis of typical XDE failures the methodological recommendations for the XDE technical expertise are defined that enable the possibility to evaluate objectively the serviceability of equipment after appropriate repair and to extend its exploitation time over several years without any noticeable worsening in X-ray diagnostics or radiation safety.

Evaluating cost-effectiveness of diagnostic equipment: the brain scanner case.

An approach to evaluating the cost-effectiveness of high-technology diagnostic equipment has been devised, using the introduction of computerised axial tomography (CAT) as a model. With the advent of CAT scanning, angiography and air encephalography have a reduced, though important, role in investigating intracranial disease, and the efficient use of conventional equipment requires the centralisation of neuroradiological services, which would result in major cash savings. In contrast, the pattern of demand for CAT scanning, in addition to the acknowledged clinical efficiency of the scanner and its unique role in the head-injured patient, ephasies the need for improved access to scanners. In the interest of the patients the pattern of service must change.

On the measurement and specification of acoustic output generated by pulsed ultrasonic diagnostic equipment.

The method of describing acoustic output generated by pulsed equipment has not been fully formalized and is often inadequately specified. This paper discusses the terms that we have found useful to specify the acoustic output generated by pulsed diagnostic equipment and proposes additional terms that allow a more complete description of irradiation dosage. The application of the proposed specification is illustrated by measurements made on a typical contact echoscope.

[Quality assurance in ultrasound diagnostic equipment in West Germany. Experiences with current regulations].

Since 1980, the Health Service (KBV) of the FRG has introduced regulations for quality assurance of diagnostic ultrasound equipment. This step was taken to limit overshooting expansion of echography and Health Service costs. The author describes experiences and problems arising from application of the KBV regulations. Preventing diagnostic ultrasound from developing institutional structures and being included in existing health and safety acts, resulted in a lack of legitimate and qualified structures for quality assurance of the methods. This deficiency of structures also compromises the quality assurance by KV regulations and thus produces an administrative torso. Therefore, in West Germany, the first step to improve quality assurance ought to be to look for appropriate institutions for qualified equipment testing.

[Efficacy of an antibacterial filter for preventing contamination of respiratory function diagnostic equipment].

OBJECTIVES: Devices to assess lung function are a potential source of nosocomial infection. Our aims in this study were: 1) to determine the efficacy of an antimicrobial filter to prevent contamination of a multifunctional device; 2) to assess the ability of the filter to prevent cross contamination of individuals being tested; and 3) to evaluate the efficacy of the recommendations of the Spanish Society of Respiratory Diseases and Thoracic Surgery for disinfecting lung function equipment. DESIGN: In this prospective, randomized study in two phases we used filters in phase 1 but not in phase 2. A pharyngeal swab culture was started within 7 days of a patient's lung function test. Swab samples for culturing were taken from three different places in the equipment at the beginning and end of each working day. PATIENTS: Sixty-five patients (31 in phase 1 and 34 in phase 2) were studied. Thirty-two (49.2%) were men and the mean age was 49.4 15.7 years. RESULTS: Significantly less equipment contamination was found in phase 1 (4.2%) than in phase 2 (21%). We detected no cases of cross contamination using the criteria in this study. No cultures from any of the samples taken before exploration were positive. CONCLUSIONS: a) The antimicrobial filter used is effective for preventing the contamination of lung function testing equipment, b) throughout both phases of the study, we observed no cross contamination of patients tested, such that we cannot conclude that the antimicrobial filter is effective for preventing possible nosocomial infections, c) the recommendations of SEPAR for disinfecting lung function equipment are effective.

[An experience of using the DRK-1 dosimeter with conduit ionizing chamber for monitoring the parameters of X-ray diagnostic equipment].

An experience of using, at the State Sanitary and Epidemiologic Supervision (SSES), the Control X-ray Dosimeter (DRK-1) to calculate the dose-square product in the radiation-and-hygienic evaluation of medical X-ray units of different types is described in the paper. The qualitative evaluation monitoring applicable to X-ray equipment is based on the finding and subsequent analysis of graphic dose-square relationships, i.e. anode voltage(measured manually or by X-ray exposure meter), exposition time, anode current, and current-time product. Real graphs for both normally functioning and for malfunctioning units are depicted. The unit radiation output is described.

A comparison of vibrotactile thresholds obtained using different diagnostic equipment: the effect of contact conditions.

OBJECTIVES: Vibrotactile thresholds on the fingers were compared using two alternative methods of controlling contact with a vibrating probe: control of the force of contact with the probe (force control) and control of skin indentation produced by the probe (indentation control). Both systems had the same control of push force on a static surround around the vibrating probe. METHOD: A group of 14 male subjects (aged 20-27 years) were tested at four frequencies (31.5, 63, 125, 250 Hz) in three separate sessions so as to quantify the repeatability of thresholds. Skin stiffness was also measured. RESULTS: Control of skin indentation gave more repeatable thresholds than control of probe force. There was a practice effect whereby thresholds became more consistent over sessions. There were no systematic correlations between thresholds and skin stiffness. CONCLUSIONS: Repeatable and similar vibrotactile thresholds can be obtained with two alternative methods having different contact conditions. Either method may assist the diagnosis of disorders associated with hand-transmitted vibration, but control of skin indentation has the advantage of greater simplicity and, in this study, greater repeatability.

[Experiences in the testing of standard data on roentgen diagnostic equipment].

The TUV (German Standards Control Body) are performing inspections of standard characteristics of different devices for medical diagnostic radiology used in medical consulting as well as in hospitals. The inspections are carried out according to the recommendations of the German rule "Richtlinien über die Aufzeichnungen nach # 29 der Röntgenverordnung-RöV" (i.e. guide line on recording according to # 29 RöV). In addition the surface dose is measured, which is a characteristic of an x-ray equipment and the knowledge of which is helpful when estimating the patient's radiation exposure. The latter inspections are performed for different modes of operation. In this paper the inspection procedure as well as the results of the inspections of radioscopy and of radiography equipment are presented and discussed. These inspections may be regarded as a part of a general performance test aimed at quality assurance in medical diagnostic radiology. The results show a wide statistical variation of several inspected parameters (e.g. film exposure, exposure rate of the image intensifier). The possible causes of this variation are presented. A further result of the inspections is the fact that in many cases some components of the complete imaging system are unrecognisedly defective. For this reason the inspection records contain-if necessary-recommendations for the improvement of the quality of the image and radiation protection.

[Computer-assisted combined management of physical radiation protection and quality control of X-ray diagnostic equipment].

A software for the computer-assisted management of radiation protection and quality control of radiodiagnostic equipment is described. The code was written in DB3-plus language. In addition to some independent archives on X-ray diagnostic systems identification, it includes technical and radiation protection checks, environmental dosimetry in the controlled areas and on workers. The data can be searched through a key field and periodical radiation protection reports and action reports can be printed. The software programs can also perform some "intelligent" operations such as the calculation of controlled areas range and the classification of X-ray diagnostic systems and workers. This is done by retrieving the information from the archives. The software can be updated, revised and modified very easily, due to the non-connection of the archives. It is also very simple to connect DB3-plus archives with many other software packages to process and elaborate data. This software is the core of an integrated system we are developing to store and process data from diagnostic X-ray systems control meters and from our TLD personal dosimetry equipment.
