A prospective study on some immunological changes occurring in the first year of grass pollen desensitization.

Some immunological parameters were followed up during the first year of grass pollen desensitization in 15 patients. The skin reactions with grass pollen extract in a series of tenfold dilutions were diminished in all patients during treatment. In nearly all patients a distinct increase in blocking antibody titre was seen. Serum reagin titres as measured by RAST remained constant or decreased somewhat during desensitization. No significant correlations between all three parameters could be demonstrated.

Research on a new type of antiinsulin antibody in a diabetic female patient: the cytotoxic antibody. antiinsulin cytotoxic antibody.

A study was carried out on a diabetic patient showing allergy to insulin, with urticaria and Quincke's edema, apparently owing to sensitization with IgE antibodies. By means of a hyposensitizing treatment her allergy improved due to the building up of IgG protecting antibodies, but at the same time she showed resistance to insulin, probably owing to IgG antibodies as well. This suggest that caution should be exerted before applying hyposensitization with insulin on diabetic patients allergic to the hormone. We were able to describe on the same patient a type II sensitization caused by a cytotoxic antibody against insulin. It was shown that this antibody agglutinates leukocytes which had been previously incubated with insulin and damage occurred when complement was present. Those effects could be especifically inhibited when the antigen (insulin) was added to the medium. The antibody belongs to the IgG group and it acts both in vivo and in vitro. In the case under study, the cytotoxic antibody disappeared as soon as treatment with insulin was discontinued for a year, and reappeared when renewed. It does not seem to occur very frequently, for it was not found in 20 consecutive diabetic patients under insulin treatment.

[New immunologic perspectives on specific desensitization].

The development of objective techniques for the quantitative evaluation of synthesis and production of IgE in man and experimental animals and the sensitivity of basophils in interaction with allergen has made it possible to initiate studies on desensitization on a scientific and objective basis. Several new mechanisms influencing the regulation of antibody formation, including the production of IgE, have recently been discovered, such as suppressor T lymphocytes and autoanti-idiotypic antibodies. Although present empirical methods used for desensitization therapy provide appreciable results, it is to be expected that the emerging immunological knowledge about regulation of IgE production will markedly influence therapeutic approaches in the foreseeable future.

Oral administration of immunomodulators and the mucosal immune system.

Orally administered antigens reach the lymphoid tissue in Peyer's patches in the gut where they initiate an immune response with clonal expansion of antigen-specific T and B cells. Activated T cells migrate through lymph and blood to intestinal epithelium (intra-epithelial leukocytes) whereas activated B cells migrate to the lamina propria, other mucosae and exocrine glands where they differentiate into plasma cells secreting polymeric IgA1 or IgA2. These antibodies are transported across the epithelial cells after binding to a poly-Ig receptor, then excreted in the lumen as secretory IgA. Reciprocal interactions have been demonstrated between lymphoid and epithelial cells in the mucosae. Oral administration of antigens in different experimental models may induce the production of secretory antibodies and/or systemic unresponsiveness with suppression of delayed-type hypersensitivity or specific IgG and IgE antibody production or both. New strategies are currently being explored for the development of oral vaccines using recombinant antigens or viral vectors (e.g. pox-viruses, vaccinia virus, cholera toxin B subunit etc.). Conversely, immunomodulating compounds or procedures which could enhance specific oral tolerance in association with antigen would have considerable therapeutic applications in auto-immune diseases and allergy.

Ants and bees: whole body extracts vs venom.

Ants, bees, and other stinging hymenoptera can induce serious anaphylactic reactions in susceptible individuals. Certainly, hymenoptera venom contains potent antigens that have been useful in testing and treating hymenoptera-sensitive patients who fail to acquire protection from whole body extract therapy. However, before whole body extracts are forever abandoned, their antigen content and stability should be re-examined by means of modern immunologic methods.

Hyposensitization in allergic rhinitis. Immunotherapy. Indications and contra-indications.

Immunotherapy in allergic rhinitis should be initiated only when other efforts at elimination and treatment of symptoms prove ineffective. The indications become more convincing if symptoms of bronchial hyperreactivity and asthma are also present. General preconditions which must be fulfilled prior to treatment are the establishing of a precise diagnosis, and that the allergen planned for treatment must play a decisive part in the disease. The main contra-indications are other, concurrent immunological diseases, and pregnancy.

[New possibilities of hyposensitization with pollen-l-tyrosine complexes (author's transl)].

The so far most comprehensive individual study into the clinical findings in the hyposensitization therapy with pollen-L-tyrosine complexes in 253 pollinosis patients, who were first subjected to a hyposensitization therapy, permitted a comprehensive evaluation of the indication possibilities of these preparations (Pollagen, Tyrosin-Allergoid; in other countries available as Pollinex, Polvac, Bencard-Polen). The success of this hyposensitization, which may be reduced to three injections, can be immunologically established and is only slightly smaller than in the case of a specific desensitization over a period of 5--6 months preseasonally. The therapy is indicated first and foremost in patients who either have a moderate or medium-degree sensitization to grass pollens or who come to a medical examination only briefly before the pollen season. The addition of rye pollens obviously gives the preparation Tyrosin-Allergoid an advantage over the mixture of pure grass pollens as used in Pollagen. Rye pollens are responsible for the highest pollen concentrations and thus for the most severe complaints of pollinosis patients. A considerable increase in the therapeutic success, approximately 20%, can be achieved on the basis of a therapy modification, presented here for the first time: The patient is treated preseasonally with a specific desensitization vaccine and immediately before the pollen season he is additionally vaccinated with tyrosine-adsorbed pollen allergens as a kind of booster vaccination.

[Penicillin allergy. Immunological bases in the diagnosis and prevention of acute reactions].

Drug alergy is always a risk in Therapeutics. Penicillin allergy although less frequent than usually though, is very important because sometimes is life threatening. Penicillin allergy is an immunological phenomenon, it is possible to use in vitro and in vivo procedures able to demonstrate individual ability to develop allergical reaction, in particular to predict such severe reactions like anaphilaxis. This paper review current concepts regarding immune mechanisms for Penicillin allergy and immunological testing useful in its study.
