["Spontaneous perforation" of Douglas' pouch].

"Spontaneous" perforations of Douglas's pouch are a rare complication following gynecological surgery. Three patients are reported, in which Wertheim-Meigs-operation, abdominal and vaginal hysterectomy had been performed. Laceration of the vaginal vault with opening of Douglas's pouch occurred "spontaneously" at sexual intercourse. In another patient rupture of the pelvic floor with prolapse of the intestinum was observed 7 months after colpocleisis. Small perforations may be closed through the vagina; prolapse of the intestinum requires laparotomy.

[Douglas' pouch swab for detection of anaerobic asporous micro-organisms (author's transl)].

Bacteriological diagnosis was applied to Douglas' pouch swabs taken off 119 patients, most of them with non-inflammatory genital diseases. Purely anaerobic cultures were recorded from 11.8 per cent, mixed anaerobic-aerobic cultures from 8.4 per cent, and purely aerobic cultures from 56.3 per cent. Cultures remained sterile in 23.5 per cent of all cases. Forty-six of those patients were afflicted with uterine myomatosis, a group from which the following results were obtained: purely anaerobic cultures in 21.7 per cent of all cases, mixed anaerobic-aerobic cultures in 6,5 per cent, purely aerobic cultures in 54.4 per cent, and sterile cultures in 17.4 per cent. The importance of such sited flora to both genesis and kinetics of pelvic inflammations is discussed.

Postcoital recovery of sperm in Douglas pouch aspirates of infertile patients.

Tubal dysfunction is certainly involved in some cases of infertility. Clinical diagnostic procedures for tubal patency are occasionally misleading and contradictory. Moreover, they provide no information on tubal function. The recovery of viable sperm in Douglas pouch aspirates (DPA) was used to evaluate tubal function in 94 infertile patients with different tubal findings at hysterosalpingography (HS) and laparoscopy. Sperm recovery in DPA of ten infertile patients with poor or negative postcoital Sims- Huhner test results demonstrated the limitation of this test for evaluation of sperm transport in the female genital tract. Laparoscopic aspiration of DP may be performed in infertile patients undergoing chromosalpingoscopy as a part of their infertility investigation. The procedure should be supplementary to other available diagnostic parameters.

Vaginoplasty from peritoneal tube of Douglas' pouch for congenital vaginal agenesis.

Vaginal reconstruction using skin and sigmoid loop has resulted in complications like dryness, stenosis and hair growth in the graft in the former and mucus discharge in the latter. A case of Douglas' pouch peritoneal tube vaginoplasty in a girl of 17 years with absent vagina is reported with satisfactory functional results. The procedure is much less invasive than the currently described methods.
