Cerebrovascular CO2 reactivity was studied in 40 healthy subjects (HSs) and 40 patients suffering from cerebrovascular disorders (CVD). Transcranial Doppler (TCD) sonography was used to investigate MCA flow velocity modifications during PaCO2 changes induced by ventilatory rate modifications and closed-circuit air re-breathing. HS responses were correlated with age, whereas the roles of either multiple ischemic brain lesions or carotid occlusions were studied in the Pt group. Results were as follows: i) age did not prove to affect vasomotor response to PaCO2 variations; ii) in hypercapnia, patients with multifocal ischemic lesions showed an impaired response in both hemispheres, whereas altered vasoreactivity was unilateral in patients with carotid occlusion (and ipsilateral to the thrombosis); iii) no significant differences were found between HS and Pt responses in hypocapnia. These findings confirm previous reports obtained with the 133 Xenon technique, stressing the usefulness of cerebrovascular reactivity investigation in CVD pts for diagnostic and prognostic purposes.

Under examination there were alcoholics aged 25 to 45 years, 44 of whom later died. At lifetime the addicts showed various clinical syndromes of acute vascular disorders, such as, hemorrhages to the brain matter, subarachnoidal hemorrhages, subdural hematomas, general disturbances of the cerebral circulation, as well as alcoholic cardiopathies. On postmortem examination both acute and chronic vascular disorders were revealed. The acute disorders manifested themselves in a pronounced plethora of the brain, the meninges and the internal organs; disturbances of the vascular wall permeability, and hemorrhages (large and small) to the brain, the subarachnoidal and subdural space and to the internal organs. In chronic vascular disorders, cerebrovascular fibrosis and atherosclerosis of the cerebral vessels, the aorta and the coronary arteries as well as residua of large and small hemorrhages sustained were recorded.

We studied polymorphisms of mitochondrial DNA 5178cytosine/adenine (mt5178C/A) and angiotensin I-converting enzyme (ACE) genes (DCP1) in 127 cerebrovascular disorder (CVD) patients and 294 age-matched normal controls to clarify the genetic background of Japanese patients with CVD. Mt5178C was predominant in CVD patients compared with controls (P<0.01). The frequency of DCP1 insertion (I) and deletion (D) alleles showed no significant difference between the CVD patients and controls or between each CVD subgroup. Although the number of CVD patients in the present study was too small to make a final conclusion, mt5178C might be one of the genetic factors to be considered in Japanese patients with CVD.

Chronic cerebrovascular disorders (CCVD), as defined by the 1980 Ad Hoc Committee in Paris, constitute both clinically and pathogenetically an extremely complex entity, characterized by a protean symptom pattern. The effects of a daily dose of 10 mg flunarizine orally on CCVD have been evaluated with a neuropsychological methodology during a three-month treatment period in a randomized double-blind study compared with a placebo. The results confirmed the effectiveness of the drug in the improvement of neurological, amnesic, attentive and behavioural symptoms, without evident side-effects even after a long-term treatment.

Patients with cerebrovascular disorders generally undergo computerized tomography (CT), even though magnetic resonance imaging (MRI) is available. We investigated the efficacy of three dimensional CT angiography (3D-CTA) for a first diagnosis of occlusive cerebrovascular disorders. Sixty-two patients, who were suspected of occlusive cerebrovascular disorder on the basis of neurological findings or initial plain CT findings, were reviewed. 3D-CTA was performed at the first medical examination. As a result, two patients were diagnosed with ICA (internal carotid artery) occlusion, nine with MCA (middle cerebral artery) occlusion, and one with BA (basilar artery) occlusion. All twelve patients underwent cerebral angiography immediately after those 3D-CTA in every case. Angiographical findings were the same as 3D-CTA. Interventional superselective thrombolytic therapy was performed in ten of the twelve patients, and recanalization resulted in four of them. 3D-CTA may also be able to determine whether the occlusive vessel is the chief cerebral artery or the perforating artery.
