Blood phosphate concentration of 257 postoperative patients in the ICU was determined through the molybdenum-blue developing method. The results showed that 136 cases had hypophosphatemia (blood phosphate concentration < 0.8mmol/L), bearing an incidence of 52.9%, which indicated that the occurrence of hypophosphatemia was associated with patients' age, primary disease and postoperative intravenous hypertonic glucose solution transfusion. Meanwhile, the existance of hypophosphatemia was often complicated by hypocalcemia (83.1%), hyponatremia (76.4%), and hypokelemia (37.5%), and furthermore, was in connection with organ dysfunction or failure. Of the 40 death cases among all 257, 31 endured hypophosphatemia (77.5%). Hence, prompt diagnosis and active prevention and treatment of hypophosphatemia are essential to raise the cure rate of surgical critical patients.

Phosphate retention is commonly associated with chronic renal failure. Previously we presented a technique to calculate the distribution of phosphate constituents in body fluids. We studied it in pre- and postdialysis serum of 62 patients. After hemodialysis, there was some decrease in the concentration of each constituent, with significant changes in the molar fraction of H2PO4- and CaHPO4. There is a significant negative correlation between the molecular mass of the constituent (X) and the effect of dialysis (Y): urea and H2PO4- 50%; NaHPO4-, HPO4(2-), and creatinine 41-44%; MgHPO4 39%, and CaHPO4 28%. Y = 69.7 - 0.26x, r = -0.833, p less than 0.001. This limited removal of CaHPO4 may contribute to the vascular calcification and peripheral ischemic necrosis of chronic uremic patients.

Leucocyte count, blood pH and gases, plasma concentrations of total and ionised calcium, of phosphate and PTH were studied in 11 stable patients on chronic haemodialysis alternatively, the sequence being randomised, during acetate (AHD) and bicarbonate (BHD). Initial leucopenia and its subsequent correction were comparable with both dialysis types whereas pO2 and pCO2 transitory decreased only in AHD so that the mechanism of hypoxemia with AHD does not seem to be related to the pulmonary leucostatis but rather to the reflex hypoventilation secondary to the decrease of pCO2 into the acetate dialysis. As regards the bone metabolism, no difference was observed between HDA and HDB considering the increase of total and ionised calcium, the decrease of plasma phosphate and the absence of decrease of PTH, the initial levels of which being however only slightly elevated. However blood pH was significantly (but slightly) higher before the subsequent dialysis in the case of BHD. This more sustained correction of acidosis would be the only theoretical advantage of BHD for the prevention of bone demineralisation.
