Visualized basting sutures in the application of full-thickness skin grafts.

This paper describes a simple technique for inserting basting sutures to secure full-thickness skin grafts. By applying the suture under visualized conditions before fully securing the perimeter of the graft, exact placement and knot security of the basting suture can be enhanced, and injury to underlying structures can be avoided. Furthermore, by securing the center of the graft early in the repair, alignment on and conformation to the underlying wound bed is improved.

Bioactivation of 7-hydroxymethyl-12-methylbenz[a]anthracene by rat liver bile acid sulfotransferase I.

The bioactivation of 7-hydroxymethyl-12-methylbenz[a]anthracene (HMBA) to an electrophilic sulfuric acid ester metabolite has been shown to be catalyzed by rat liver bile acid sulfotransferase I (BAST I). The sulfation and activation of HMBA by BAST I was determined by the ability of sulfated HMBA to form DNA adducts. The BAST I was also shown to react with rabbit anti-human dehydroepiandrosterone sulfotransferase antisera and to represent a major form of hydroxysteroid/bile acid sulfotransferase in female rat liver cytosol. Higher levels of BAST I activity and immunoreactivity as well as HMBA-DNA adduct formation were detected in female rat liver cytosol than in male rat liver cytosol. The bioactivation of HMBA by pure BAST I was dependent on the presence of 3'-phosphoadenosine 5'-phosphosulfate (PAPS) in the reaction and was inhibited by dehydroepiandrosterone, a physiological substrate for BAST I. Glutathione, a cellular nucleophile with important protective properties, decreased DNA adduct formation in the HMBA sulfation reaction in the absence of glutathione S-transferase activity. These results indicate the usefulness of BAST I to investigate the sulfation and activation of HMBA and probably other hydroxymethylated polyaromatic hydrocarbons to electrophilic and mutagenic metabolites under defined reaction conditions.

Difference in enzymatic sulfation of bile acids between the mouse and rat.

Species difference in bile acid-sulfotransferase (BAST) activity was studied between the mouse and rat. Cytosol fractions of the liver, kidney, small intestine and large intestine were incubated with bile acids and 3' phosphoadenosine-5'-phosphosulfate. The mouse liver showed BAST activity for lithocholic acid, taurolithocholic acid and taurochenodeoxycholic acid, whereas the rat liver and kidney had the activity for taurodeoxycholic acid in addition to these compounds. The highest activities were found in the mouse liver and rat kidney. The mouse small intestine showed weak activity only for lithocholic acid. No activity was found in other organs. BAST was inactive towards taurocholic acid, 7 alpha- or 12-monohydroxy-5 beta-cholanoic acid. Optimal pH of liver BAST in the two species was different from that of the rat kidney. BAST of the mouse liver showed the highest activity without addition of Mg2+, whereas that of the rat liver and kidney showed enhancement by exogenous Mg2+. These results indicated that the distribution and characteristics of mouse BAST was different from rat BAST. Such difference should be reminded in any animal study involving the two species.

Bile acid sulfotransferase I from rat liver sulfates bile acids and 3-hydroxy steroids: purification, N-terminal amino acid sequence, and kinetic properties.

A bile acid:3'phosphoadenosine-5'phosphosulfate:sulfotransferase (BAST I) from adult female rat liver cytosol has been purified 157-fold by a two-step isolation procedure. The N-terminal amino acid sequence of the 30,000 subunit has been determined for the first 35 residues. The Vmax of purified BAST I is 18.7 nmol/min per mg protein with N-(3-hydroxy-5 beta-cholanoyl)glycine (glycolithocholic acid) as substrate, comparable to that of the corresponding purified human BAST (Chen, L-J., and I. H. Segel, 1985. Arch. Biochem. Biophys. 241: 371-379). BAST I activity has a broad pH optimum from 5.5-7.5. Although maximum activity occurs with 5 mM MgCl2, Mg2+ is not essential for BAST I activity. The greatest sulfotransferase activity and the highest substrate affinity is observed with bile acids or steroids that have a steroid nucleus containing a 3 beta-hydroxy group and a 5-6 double bond or a trans A-B ring junction. These substrates have normal hyperbolic initial velocity curves with substrate inhibition occurring above 5 microM. Of the saturated 5 beta-bile acids, those with a single 3-hydroxy group are the most active. The addition of a second hydroxy group at the 6- or 7-position eliminates more than 99% of the activity. In contrast, 3 alpha,12 alpha-dihydroxy-5 beta-cholan-24-oic acid (deoxycholic acid) is an excellent substrate. The initial velocity curves for glycolithocholic and deoxycholic acid conjugates are sigmoidal rather than hyperbolic, suggestive of an allosteric effect. Maximum activity is observed at 80 microM for glycolithocholic acid. All substrates, bile acids and steroids, are inhibited by the 5 beta-bile acid, 3-keto-5 beta-cholanoic acid. The data suggest that BAST I is the same protein as hydrosteroid sulfotransferase 2 (Marcus, C. J., et al. 1980. Anal. Biochem. 107: 296-304).

Leukemia serum reactive with retrovirus-related antigen in normal human placenta.

Serum antibody reactive with a retrovirus-related p30 antigen in human normal term placenta was investigated and characterized by immunohistologic and immunoblotting methods. Sera obtained from patients with acute leukemia and malignant lymphoma were used as first antibody, and cryostat sections of placenta were the target antigen. An IgM antibody that reacted mainly with the basal aspect of syncytiotrophoblast of chorionic villi, where a putative human endogenous retrovirus p30 antigen is located, was directed by indirect immunofluorescence. This antibody activity, termed anti-basal aspect of syncytiotrophoblast (anti-BAST), was detected in the sera of many patients with acute leukemia (AML, ALL) and malignant lymphoma, and less frequently in sera of pregnant women and normal controls. As shown by immunoblotting analysis, the main reactive antigen recognized by anti-BAST was a non-glycosylated 32-kDa placental protein which was antigenically related to SSAV p30. A non-glycosylated 19-kDa protein was also considered to be one of the anti-BAST-corresponding antigens. This suggests the presence of a new antigen-antibody system of human retrovirus(es) revealed by subinfectious antigenic expression and by specific antibody activity in conditions of human health and disease, particularly, acute leukemias and malignant lymphomas of common types.

4CMenB vaccine coverage of invasive serogroup B meningococci collected in Belgium between 2016 and 2022.

Neisseria meningitidis infections can cause life-threatening meningitis and septicemia. In Europe, serogroup B (MenB) is the leading cause of invasive meningococcal disease (IMD), particularly in young children. Genomic surveillance of circulating MenB strains through whole genome sequencing (WGS) provides a powerful tool to assess the potential impact of vaccination strategies, including the 4CMenB vaccine, which is available for infants from 2 months of age. Here, we present a retrospective WGS-based analysis of clinical MenB IMD cases (n = 311) recovered in Belgium from 2016 to 2022 by the Belgian National Reference Center. High-quality WGS data were obtained for 281 of these strains, demonstrating high genetic diversity of the antigen targets included in the 4-component meningococcal serogroup B vaccine 4CMenB (fHbp, PorA, NHBA and NadA) and at the 4CMenB Antigen Sequence Types (BAST) level. Novel antigen combinations, not yet assigned a BAST ID, were detected in 23.5% of isolates. Vaccine coverage was predicted using the Genetic Meningococcal Antigen Typing System (gMATS) and the Meningococcal Deduced Vaccine Antigen Reactivity (MenDeVAR) index. Of the 281 strains, 79.5% (lower limit-upper limit: 68.0-91.5%) were predicted to be covered by the vaccine by gMATS, and 80.7% (lower limit-upper limit: 66.5-95.4%) by MenDeVAR. No evidence of variation in vaccine coverage was found throughout the study period nor between different age groups, demonstrating the broad applicability of 4CMenB. This study highlights the benefits of a pathogen surveillance program and the need for experimental characterization of continuously evolving antigenic subvariants of Neisseria meningitidis.

Serum antibody reacting with placental syncytiotrophoblast in sera of patients with autoimmune diseases--a possible relation to type C RNA retrovirus.

This study reports the immunohistological detection of serum antibody reacting with the basal aspect of syncytiotrophoblast of human chorionic villi, where SSAV/GaLV (simian sarcoma associated virus/gibbon ape lymphoma virus) type C retrovirus p30 related antigen was observed by an indirect immunofluorescent method using monospecific antibodies against SSAV p28 and GaLV p29. The immunoglobulin class of this antibody activity called 'the anti-basal aspect of syncytiotrophoblast (anti-BAST)', was exclusively IgM and detected in the sera of both female and male patients with SLE and other autoimmune diseases, but rarely in the sera of normal controls. Immunofluorescent absorption and blocking test revealed that anti-BAST specifically reacted with human placenta and cross-reacted with subhuman primate type C RNA retrovirus SSV/SSAV (simian sarcoma virus/simian sarcoma associated virus), but did not cross-react with ATLV (adult T cell leukaemia virus) and BaEV (baboon endogenous retrovirus). These results suggest the presence of a new antigen-antibody system for another human type C retrovirus related antigens(s) and a participation of retrovirus in autoimmune diseases.

Refinements in split-thickness skin grafting technique.

Modifications for split-thickness grafting techniques are discussed, including the use of incontinuity, semipermeable, biologic dressings during harvesting, spiral basting sutures for graft attachment, and meshing to increase graft coverage.

[An experimental-morphologic study of the regenerative capacity of the nematode Pontonema vulgaris (Bastian, 1865)].

In free esea nematodas Pontonema vulgaris (Bast). end portions were cut off and internal organs were damaged without disturbing the integrity of the cuticle (at the level of the oesophagus, anterior and medial parts of the gut and at the tail level). Reparative processes were registrated under conditions of "closed" traumas only. Removal of degenerating cells and a closure of the wound in the gut was brought about due to the active shift of the intestinal epithelium edges towards one another. The rest of degenerating cells found themselves in the gut lumen and were gradually adsorbed by entodermal cells. A deficiency of the intestinal epithelium cells is made up due to mitotic division of these cells in the epithelium areas adjacent to the site of the trauma. Regeneration in females was found to be more rapid than in males.

[The preparation and clinical use of a radioimmunoassay CA125 kit for the diagnosis of epithelial ovarian cancer].

A self-made radioimmunoassay CA125 kit (using OC125 monoclonal antibody ascites offered by Dr. Bast Laboratory which was purified, solidified and labelled with 125I) was used for serum determination in 80 patients with epithelial ovarian cancer and in 40 standard antigen samples. The results demonstrated a statistically significant correlation between our self-made CA125 kit and an imported CENTOCOR CA125 kit (P < 0.001). Clinical experience before operation in 40 patients with epithelial ovarian cancer, 34 with endometriosis and 39 with other malignant ovarian tumors suggested that the self-made CA125 kit was more sensitive in diagnosing epithelial ovarian cancer than the other two diseases. Post-operative follow-up in 86 non-recurrent cases and 101 recurrent cases showed a 96% positive predictive rate and 91% negative predictive rate. It is concluded that self-made CA125 kit is very useful in monitoring epithelial ovarian cancer.

Paget's disease of bone--current thinking and management.

OBJECTIVES: The objective of this article was to critically review the current knowledge on Paget's disease of bone, focusing primarily on possible etiologies and trends in management. Current courses of treatment are described, including calcitonin, diphosphonate and mithramycin therapy and the efficacy of each drug regime is analyzed. Finally, newer, more experimental pharmaceutical agents, such as gallium nitrate, are described. DATA SOURCES: Information was obtained from English language medical and scientific journals and medical/physiology text books. Index Medicus (1983-1989) and MEDLINE computerized data base were used. Paget's disease of bone, paramyxovirus, osteoclast, calcitonin and diphosphonate were the terms indexed directly. Key authors indexed included Rebel, Baste and Paget. Further information was obtained from the National Association for the Relief of Paget's Disease (England). CONCLUSIONS: Current research focuses primarily on histological and hybridization studies over the past 15 yr in favor of a slow virus as the causative agent in the disease, and the evidence supporting this is analyzed in some detail. In particular, the discovery of nuclear and cytoplasmic inclusion bodies within the osteoclasts of patients with Paget's disease and their similarity to respiratory syncytial virus and measles virus are discussed. Some form of environmental or physiological trigger may be involved, but the exact mechanism remains obscure. The current treatment of Paget's disease is based around the prevention of bone resorption using various diphosphonates and calcitonins, but many of these are associated with severe side effects. Newer pharmaceutical agents to control bone turnover, including gallium nitrate, have given promising results in preliminary medical trials.

[Diagnostic value of suspect epilepsy by means of 24 hour of ambulatory electroencephalogram].

106 cases of suspect epilepsy random observed by means of 24 hr tape recording of AEEG were given in this paper. Among them 57 cases. (accounting for 53.77%) are lightly abnormal including 42 cases (accounting for 39.62%) recorded as epilepsy burst. Comparing with EEG: Among 93 cases 25 cases as lightly abnormal by EEG (accounting for 26.88%), 52 cases recorded as lightly abnormal by AEEG (accounting for 55.91%) (P less than 0.005); arising rate of epilepsy burst 11 cases were recorded by EEG (accounting for 11.82%), 41 cases by AEEG (accounting for 44.01%) (P less than 0.005). The record of epilepsy burst patients monitored showed that positive rates of transient consciousness loss (8/15, 53.33%) and convulsions (9/19, 47.39%) are the highest. Abnormal burst time by AEEG is during sleeping (54 cases observed accounting for 77.14%) with positive rate highest at the same time, most accorded in the front temporal area (29/92, 31.52%). The author therefore, suppose that AEEG is more accurate to find abnormal bast wave or epilepsy burst wave than EEG because of superior condition in recording time to EEG and could offer more objective diagnosis reference for suspect epilepsy patients who are difficult to be checked clinically because of low positive rate of EEG.

The quality of life of the dentist as perceived by society.

Research dealing with the public's perception of the quality of life of the dentist is not well developed. Generally, the approach of researchers to this topic has been to examine the public's image of the dental profession. Specifically, the degree of prestige attributed to the dental profession by the public has attracted major attention. A review of studies reveals that while the profession enjoys relatively high prestige, it is extrinsic factors rather than the intrinsic value of the service that determine the public's image. The dental health care system and dental education as presently structured do not appear to be serving the bast interest of the public. The paper concludes with a discussion of what are considered to be major issues that the dental profession both in Canada and elsewhere will have to deal with in the coming years, in order to bring about change both in its image and the generally low utilization of the services it provides.
