[Methods of cardiac synchronization in assisted circulation].

In assisted circulation cardiosynchronization should be regarded as a process of monitoring the arterial blood pressure within the limits of the cardiac cycle and changes of its form with the aim of improving the energy balance of the heart. The coordination of the pumping function and that of the cardio-vascular system comes to a problem of shaping the monitoring action that depends upon three variables, viz. the time lag tepsilon, actuation of the unit relative to the base cardiosignal, duration of the pumping phase tpi and the signal level at the pump's inlet U(t). A number of researchers consider tepsilon and tpi to be a function of the cardiac contractions period and determine them in different ways, employing for this purpose extrapolation, stochastic characteristics, etc. Of late, in addition to the period of cardiac contractions extremal, mean integral values of the systolic and diastolic pressure, the level of pressure at the instant of the cardiac valves closure and others have also come in use. There are shortcomings in the proposed cardiosynchronization algorhythms: their empiric nature and also the fact that the monitoring action includes merely variables tepsilon and tpi and does not carry function U(t), which reflects the intensity of the pump's operation.

[Feasibility of increasing the high-speed response of the electromechanical drive in assisted circulation devices].

The conditions in assisted circulation, especially with ballooned aorta, place high demands on the speed of the intraortal pump-balloon response. The salient feature of the described assisted circulation unit consists in that the intraortal pump-balloon is connected to the pneumotract via a commutative air-operated valve. A block diagram of the assisted circulation unit "Biopulso-4" based on the use of the electric drive, designed at the All-Union Scientific-Research Institute of Medical Equipment and Instruments, stress diagrams and a scheme explaining the operation of the apparatus, are described. The results obtained show the possibility of the balloon's resetting and opening in 70 ms, with the triggering synchronous pulse delay of 10 ms prior to response (with air use as power gas). These results are not any worse than those obtained with the pneumatic drive-based assisted circulation apparatus.

[Methods and modern apparatus for assisted circulation. The basic trends in their development].

A systematization of methods for assisted circulation, based on the formulation of a concrete procedure, the nature of the deranged or lost function substitution and conditions for effective realization of such a substitution, is offered. This made it possible to classify the equipment for assisted circulation and to analyze the current trends of its further development. The most characteristic automation levels of the equipment under consideration and of generations of some apparatus, differing in the degree of automation, control and monitoring the process of assisted circulation, are presented in the form of tables and pictures.

Business ends of operating mechanisms for assisted circulation, based on the principle of contrapulsation, must satisfy a number of requirements on which the safety and the effectiveness of their performance depend. Among these of particular importance are the working volume of the arrangement, correct conformity of the balloon and aorta diameters and also the duty cycle of the arrangement.

[Power units of implanted artificial heart and assisted circulation system].

The existing and presently planned systems of power supply for an artificially implanted heart and assisted circulation devices are reviewed. A comparative analysis as to their conformability to biological, functional and technical demands placed on the implanted systems is given. In an implanted artificial heart and assisted circulation systems most promising is shown to be the use of nuclear fuel as a source of power and as converters -- that of thermal engines with gas and steam cycle.

[Trauma to the blood during assisted circulation].

Trauma inflicted to blood in different method of assisted circulation was studied in experiments and in the clinic. Morphological disturbances of red blood cells and changes in their aggregation properties were revealed. Left ventricular shunt and aorto-aortic counter-pulsation are attended with increased aggregation properties of blood platelets. Aorta-aortic counter-pulsation causes a greater damaging effect than shunting of the left ventricle. Counter-pulsation by means of a balloon pump is attended with least hemolysis.

[Hydrodynamic stands for testing assisted circulation and artificial heart apparatus].

Problems arising in devising the assisted circulation apparatus and artificial heart can be solved only with investigations conducted on hydrodynamic models of the circulation system--the stands. An analysis of literature sources dealing with test stands justifies their classification according to the purpose, the types of tests, the specific structure and basic characteristics. It is necessary to proceed with further improvement of the stand equipment in the direction of creating a sufficiently accurate imitation of the anatomic structure and resilient properties of major arteries and veins, of constructing a geometrical and elastic model of the aorta, of pressure and flow control instruments at different points of the vessels, of the ones regulating the energy parameters, as well as of working out criteria for effectiveness of the devices to be used.

[Gas volume stabilization in the pneumatic transmission unit of the assisted circulation apparatus].

A compensator connected to the section consisting of the pump-main line-operating member and including a pneumatic resistance and a flaxid non-elastic container enables it in combination with the feedback to maintain through the volumetric displacement of the gas, or changing the pump diaphragm position, the stability of the gas volume in the pneumatic transmission element of the assisted circulation apparatus. The stabilization of the gas volume in the pneumatic transmission element creates favourable conditions of exploitation and heightens the effect of the cardiac insufficiency treatment, expecially in cases of protracted assisted circulation.

[Changes in the myocardial catecholamines in experimental stress and during assisted circulation].

After two hours of immobilization stress in rats, the authors observed significantly decreased noradrenalin values in the myocardium, returning to normal values later. The concentration of adrenalin in the myocardium of rats showed a contrary behavior under stress conditions. Myocardiac catecholamin shifts in dogs with experimental stress induced by coronary occlusion were registered. After 120 minutes of mechanical circulatory support for the left-sided bypass system the catecholamin level in the left atrium decreased. On the other hand, catecholamin levels in the left ventricular myocardium, injured by experimental infarction, were unchanged after 120 minutes of assisted circulation. In correspondence with hemodynamic and metabolic findings, these results could support the method of assisted leftside bypass. Myocardial catecholamines seem to be another possible parameter for the assisted circulation.

[Myocardial ultrastructure in the presence of assisted circulation].

Experiments were conducted on dogs; a study was made of the influence of a method of assisted circulation--contrapulsation, on the myocardial ultrastructure. A sharp glycogen accumulation in the muscle cells of the heart, the presence of mitochondria with a marked osmophilic microgranular matrix, and also a high pinocytic activity of the capillary endothelial cells were revealed by electron microscopy. The data obtained pointed to the metabolic reconstruction in the myocardium, and primarily to the reduction of the energy level of the myocardial muscle cell function. The changes in the myocardial ultrastructure revealed apparently provided the positive therapeutic effect.

[Massive mitral insufficiency due to spontaneous and isolated rupture of a posterior papillary muscle. Echocardiographic study, treatment by assisted circulation and prosthetic valve replacement].

The authors report a case of a patient of 72 years of age in whom the sudden onset of severe left ventricular failure led to the discovery of massive mitral incompetence. The diagnosis of rupture of the posterior papillary muscle of the mitral valve, which was suggested on clinical and echocardiographic grounds, was confirmed at operation. This was carried out after 72 hours of assisted circulation by counter-pressure, and consisted of a replacement by a prosthetic valve. The authors emphasise, in the light of this case, the importance of echocardiography by ultrasound in the early diagnosis of this type of acute mitral incompetence, clarify the signs, and emphasise the part played by assisted circulation with an intra-aortic balloon in the treatment of this condition.

[Certain technical aspects of assisted circulation].

The paper treats of issues concerned with the control, designing of the drive and operating mechanisms of the equipment for assisted circulation, working on the principle of an oriented presure change in the cardio-vascular system. Problems relating to the cardiosynchronization of the therapeutic action, constructing a drive based on the use of a linear direct current motor and to the demands placed on the actuating mechanisms are discussed.
